Leading causes of death among children under 5 years of age in the various population groups of the RSA in 1970.

The mortality rates (MRs) of children under 5 years of age in the various population groups of the RSA were calculated as deaths/10(5) for various causes of death and groupings of causes of death as classified by the International Classification of Diseases. In 1970 the ten leading causes of death among Coloured and Black children under 5 years of age in the RSA were similar to those among children in developing countries. The rank order of causes of death (in MRs/10(5] among Coloured children was as follows: gastro-enteritis (1 733), pneumonia (725), immaturity (405), ill-defined causes of de

Infections and infestations of the central nervous system in India. A review.

Based on extensive personal experiences during the past three decades, a review is given on infections and infestations of the central nervous system, which are found in India and in other tropical and subtropical areas. Besides the main disease such as tuberculosis, pyogenic infections and parasitic infestations, rare lesions are also discussed. The literature has been cited in extenso and more especially the contributions by Indian neurosurgeons and neurologists.

Opportunistic infections and Kaposi's sarcoma among Haitians: evidence of a new acquired immunodeficiency state.

Twenty Haitian patients, hospitalized from 1 April 1980 to 20 June 1982, had Pneumocystis carinii pneumonia, central nervous system toxoplasmosis, esophageal candidiasis, cryptococcosis, disseminated cytomegalovirus, progressive herpes simplex virus, chronic enteric coccidiosis, or invasive Kaposi's sarcoma. Ten patients died. Opportunistic infections were frequently multiple and were recurrent in three patients. In seven patients disseminated tuberculosis preceded the other infections by 2 to 15 months. There was no evidence of an underlying immunosuppressive disease, and no history of homose

[Cerebromeningeal location of chronic lymphoid leukemia. Rapid immunochemical diagnosis and complete remission by intrathecal chemotherapy].

Central nervous system (CNS) involvement is a rare complication of chronic lymphoid leukaemia (CLL). The occurrence of lymphocytic meningitis in the course of CLL suggests either CNS involvement by the leukaemic process or infection (especially tuberculosis) related to continuous immunodepression. We report a case of CLL in which leukaemic cells had surface IgM and produced IgM kappa without significant depression of other immunoglobulins. When the patient developed meningitis, measurement of serum and CSF albumin IgG and IgM levels showed that 88% of CSF IgM resulted from local synthesis, whi

Demonstration of mycobacterial antigens in nerve biopsies from leprosy patients using peroxidase-antiperoxidase immunoenzyme technique.

Peripheral nerve biopsies from patients with leprosy were stained with anti-Mycobacterium bovis (BCG) in a peroxidase-antiperoxidase (PAP) system to demonstrate intraneural mycobacterial antigens. Most M. leprae antigens have been shown to cross-react with BCG. Of the 30 biopsies from borderline tuberculoid (BT) patients 18 had acid-fast bacilli while 26 of them had demonstrable mycobacterial antigens in their nerves. All borderline lepromatous (BL) and lepromatous leprosy (LL) nerve biopsies had both M. leprae and mycobacterial antigens within them. Most of the antigens in the BT patients wer

Diffusion of ceftriaxone (Ro 13-9004/001) in the cerebrospinal fluid. Comparison with other beta-lactam antibiotics in dogs with healthy meninges and in dogs with experimental meningitis.

After i.v injection of 50 and 100 mg/kg ceftriaxone in 1 h in dogs with healthy meninges, the cerebrospinal fluid (CSF) concentrations found between minutes 90 and 240 were an average of 0.37 (0.32-0.41) and 1.22 microgram/ml (1.02-1.56), respectively. In dogs with meningitis a dosage of 50 mg/kg yielded high concentrations from minutes 60 to 240: on average, 13.0 microgram/ml (9.9-14.9); 9.9 microgram/ml in the fourth hour. The concentrations obtained in dogs with healthy meninges were 10-100 times higher than the MICs for meningococci and Haemophilus influenzae. In the infected dogs, the con

Treatment of bacterial infections of the central nervous system.

Antimicrobial agents for the treatment of bacterial meningitis and other pyogenic intracranial infections have now been in use for approximately 45 years. The last decade, however, has been a period of tremendous advances in knowledge of the pharmacokinetics and other characteristics of the available drugs. The identification of factors that affect their in vivo metabolism and excretion, the importance of drug interactions, and the development of methods for estimation of serum and cerebrospinal fluid levels have added both complexity and sophistication to the clinical utilization of the antim

Relapsing experimental allergic encephalomyelitis in the SJL/J mouse.

A single injection dose-response curve was done in the SJL/J mouse using varying amounts of syngeneic spinal cord homogenate and Mycobacterium tuberculosis H37RA in a Freund's type of adjuvant. Inoculation with as little as 0.025 mg. of cord homogenate induced severe experimental allergic encephalomyelitis in some mice. Possible reasons for the great ease of induction of experimental allergic encephalomyelitis with this single injection regimen in comparison with other single injection schemes are discussed. No single injection dose combination was found that induced severe experimental allerg

[Experimental allergic encephalomyelitis in guinea pigs caused by meningococci].

The sensitization of guinea-pigs with the mixture of meningococci and heterologous-cerebral tissue commonly induces the development of the typical clinical and pathomorphological picture of allergic encephalomyelitis. Unlike Mycobacterium tuberculosis and other acid-resistant bacterial and much like Bordetella pertussis, meningococci are capable of inducing allergic encephalomyelitis when introduced in mixture with oil without cerebral tissue. The vaccinal strain induces allergic encephalomyelitis with a more moderate course of the disease.

[Therapeutic value of immunoglobulins in central nervous system infections].

From 218 patients aged 15 to 72 years suffering from inflammatory infectious diseases of the central nervous system, 162 patients were treated with high doses of immunoglobulins (Ig). The therapeutic results were compared with those of the 56 patients without an additive therapy, separately for bacterial and viral infections. If we compare recoveries and deaths or survivals but defective recoveries, the chi-square test reveals that treatment with Ig at the 0.1 percent level is superior to treatment without Ig in a highly significant manner in favor of the use of Ig. Using other criteria, such

Late generalized tuberculosis: a clinical pathologic analysis and comparison of 100 cases in the preantibiotic and antibiotic eras.

1. The clinical and pathologic findings in 100 patients with late generalized tuberculosis (LGT) are described and a comparison made between the findings occurring in the preantibiotic era with those in the early antibiotic period. The clinical presentation of LGT as seen in a general hospital has changed. Whereas, in the preantibiotic era, LGT was often the primary disease, occurring principally in young adults and frequently associated with pulmonary symptoms, in the antibiotic era, LGT commonly occurred together with and was frequently obscured by other diseases, often afflicted the elderly

Pyridoxine supplementation during isoniazid therapy.

Vitamin B6 (pyridoxine) supplementation during isoniazid (INH) therapy is necessary in some patients to prevent the development of peripheral neuropathy. In vivo pyridoxine is converted into coenzymes which play an essential role in the metabolism of protein, carbohydrates, fatty acids, and several other substances, including brain amines, INH apparently competitively inhibits the action of pyridoxine in these metabolic functions. The reported frequency of INH-induced neuropathy in various studies is reviewed and population groups at relatively high risk of developing this complication are ide

Congenital tuberculosis: critical reappraisal of clinical findings and diagnostic procedures.

The recent pattern of immigration from Indochina and Latin America to the United States suggests that tuberculosis will remain a significant public health problem. Two infants recently seen with probable congenital tuberculosis prompted critical evaluation of the 24 cases of congenital tuberculosis reported in the English literature since the introduction of isoniazid in 1952. Failure to thrive, jaundice, and central nervous system involvement, all reported in previous reviews and textbooks to be very common, were unusual presenting manifestations. In contrast, hepatomegaly, a finding not ment

Uncommon neurological presentations of tuberculoma.

The neurological manifestations and autopsy findings of 3 cases of tuberculoma have been described. A cerebello-pontine angle tuberculoma presented with a long history of trigeminal neuralgia. Intramedallary tuberculoma of the spinal cord presented with transverse myelitis, history of one complete remission and a normal myelogram. A tuberculoma of the medulla led to, apart from other features, severe orthostatic hypotension. Tuberculous meningitis, as a terminal process, developed in the first two cases and the third patient died from a vasomotor collapse on suddenly standing up from bed.

[A case of cerebral tuberculoma (author's transl)].

A 27-year-old woman was admitted to other hospital for acute pleuritis in May 1977. She suddenly had a focal epileptic seizure in the face with loss of consciousness on July 10, 1977. The same episodes of seizure occurred on Aug. 8, on Oct. 26, on Nov. 22, 1977. She was admitted to our hospital on Dec. 12, 1977. Neurological examinations showed no abnormality. Chest X-ray film showed bilateral severe thickening of the pleura. Plain skull films showed normal findings. Enhanced CT scanning showed a homogenous irregular contour of high density area surrounded by low density area in the right fron

Isoniazid therapy of Huntington disease.

We describe clinical and biochemical changes in seven patients with Huntington disease given isoniazid (INH) in dosages three to five greater than normally used in tuberculosis. Because INH inhibits the enzyme gamma-aminobutyric acid aminotransferase (GABA-T), and increases GABA content in the brains of experimental animals, it might correct the brain GABA deficiency characteristic of Huntington disease. Of six patients treated long enough to be clinically evaluated, one showed marked and two others showed signifciant improvement. High-dose INH therapy carries serious toxic risks, which are in

Metchnikoff-Schaumann bodies in experimental Yersin's type tuberculosis induced with Mycobacterium avium in hamster.

Structure and ultrastructure of Metchnikoff-Schaumann bodies occurring in macrophages, epithelioid cells and multinucleated giant cells in tuberculous granuloma were investigated. Their similitude with other reported intracellular calcareous bodies occurring in malakoplakia, infectious orchitis (Michaelis-Guttman' bodies or calcosphaerites), in beryllium granulomas (conchoid bodies) and sarcoidosis (Schaumann bodies) is discussed.

[Comparison of CT, scintigraphy, and angiography in the topographic and pathological diagnosis of 300 supratentorial tumors (author's transl)].

The results of CT, scintigraphy (CSS), and angiography in 300 supratentorial cerebral tumors were compared with regard to the topographic and pathological diagnosis proposed in the initial reports. CT elucidated the topography in 98% of the cases and the pathology in variable percentages, depending on the type of tumor. The use of scintigraphy provided a high accuracy of pathological diagnosis. Except for meningiomas, arteriography was less accurate than the two other methods of examination. A radiological management of supratentorial cerebral tumors is proposed based on clinical and CT data.
