Can 6-month long regimens become the standardized treatment for MDR-TB globally?

Treatment for multidrug-resistant or rifampicin-resistant (MDR/RR) tuberculosis (TB) was traditionally long and poorly tolerated. In 2022, the World Health Organization recommended the first 6-month regimen (BPaLM), followed in 2025 by a second short-course option (BDLLfxC) with potential drastic improvement of treatment outcome. The BDLLfxC regimen addresses key gaps in target populations, including groups for whom BPaLM is not indicated: children of any age, adolescents, and pregnant and breastfeeding women. A key difference between the two regimens is the replacement of pretomanid with dela

Invasive Aspergillosis with Intracranial Extension Initially Misdiagnosed as a Granulomatous Disease: A Case Report.

Background: Invasive aspergillosis with orbital apex and intracranial involvement is rare and often misdiagnosed due to nonspecific imaging findings. Misinterpretation may lead to inappropriate therapies, such as corticosteroids, which can exacerbate fungal infections. Case Presentation: A 50-year-old immunocompetent woman with diabetes mellitus presented with right ptosis and systemic malaise. Magnetic resonance imaging (MRI) performed three months prior had shown a subtle low-signal lesion in the right orbital apex. The lesion was small and thought to represent a granulomatous process, with

Concurrent Tuberculous Meningitis and Toxoplasma Encephalitis in an HIV-Positive Patient: An Exceptionally Rare Case.

Human immunodeficiency virus (HIV) infection is strongly associated with an increased risk of opportunistic infections affecting the central nervous system (CNS). Among these, tuberculous meningitis (TBM) and Toxoplasma encephalitis (TE) represent two of the most frequent and severe infections seen in individuals with advanced immunosuppression. Each condition alone is associated with significant morbidity and mortality, particularly in patients with profoundly diminished CD4+ T-cell counts. While TBM and TE are common individually in the context of HIV/AIDS, their concurrent occurrence within

Updates in the clinical management of tuberculous meningitis.

INTRODUCTION: Tuberculous meningitis is frequently associated with significant mortality and persistent neurological sequelae. Diagnosis is often delayed due to nonspecific symptoms and the low sensitivity of cerebrospinal fluid (CSF) tests. Standard pulmonary tuberculosis regimens are suboptimal for central nervous system involvement due to poor drug penetration and prolonged treatment. Emerging resistance, particularly to rifampicin and isoniazid, further complicates the management. AREAS COVERED: This review synthesizes recent data particularly on diagnostic tools, drug treatment, host-dir

An unusual triad: tuberculomas, tubercular meningitis, and hydrosyringomyelia in a child from a suburban region.

Central nervous system (CNS) tuberculosis is a rare but serious manifestation of Mycobacterium tuberculosis infection, especially in the paediatric population. We report a rare case of an eight year old male child from a suburban region presenting with intracranial tuberculomas, tubercular meningitis, and spinal hydrosyringomyelia (HSM). Magnetic Resonance Imaging (MRI) revealed multiple brain tuberculomas, basal exudates, abnormal meningeal enhancement, spinal HSM with features of arachnoiditis. The patient was operated for removal of tuberculomas and placement of ventriculo-peritoneal (V-P)

Ambient light alleviates retinal neurodegeneration in mice by powering mitochondria via the engineered optoenergetic rhodopsin.

The mitochondrial proton motive force (pmf) is a critical driver of cellular energy production and influences various cellular processes. Dysregulation of pmf is implicated in a range of diseases, including neurodegenerative diseases, mitochondrial diseases, cancer and aging-related pathologies. Currently, an efficient strategy to rescue ATP production and mitigate reactive oxygen species (ROS) generation under conditions of energy deprivation is lacking. Here, we engineered a light-sensitive, mitochondria-targeting proton-pumping rhodopsin (PPR), mt-EcGAPR, capable of generating an efficient

Primary intracerebral aspergillosis in an immunocompetent patient with no risk factors or clear cause: a case report.

INTRODUCTION: Intracranial aspergillosis (ICA) is an uncommon, life-threatening central nervous system infection that is caused by the Aspergillus species. Usually, ICA happens in immunocompromised patients. However, it can sometimes happen in immunocompetent patients, which poses a unique diagnostic and therapeutic challenge. Here we present a rare case of primary ICA in an immunocompetent patient. PRESENTATION: A 60-year-old woman presented with aphasia, blurry vision, nausea, and vomiting in addition to a 1-month history of eye pain and decreased vision. Examination showed disorientation,

Proteomic profiling of cerebrospinal fluid uncovers distinctive pathophysiological changes and potential biomarkers in pediatric tubercular meningitis.

INTRODUCTION: Tubercular meningitis (TBM) is a serious pediatric infection with high mortality rates. This research aimed to characterize the alterations in cerebrospinal fluid (CSF) proteome in TBM and to identify biomarker panels that can distinguish it from other central nervous system infections. METHODS: We retrospectively analyzed the CSF proteome from 104 patients, including 7 TBM, 28 with purulent meningitis (PM), 20 with viral meningitis (VM), 9 with cryptococcus neoformans meningitis (CNM), 30 non-CNS infected controls (Ctrl), and 10 brain disease (BD) controls. RESULTS: The TBM pr

Epstein-Barr Virus Central Nervous System Infections and Mortality Risk in Patients Presenting With Suspected Meningitis: Results From the Botswana National Meningitis Survey and the Harare Meningitis Aetiology Study.

BACKGROUND: Epstein-Barr virus (EBV) central nervous system (CNS) infection in immunocompromised hosts and among meningitis cohorts is well recognized. The clinical significance of EBV CNS infection, however, is poorly understood. METHODS: Data were collected as part of the Botswana National Meningitis Survey and the Harare Meningitis Aetiology Study. The prevalence of EBV CNS infection (EBV DNA detected in cerebrospinal fluid [CSF] by means of quantitative polymerase chain reaction) was determined, alongside associations with baseline covariates and the in-hospital mortality rate. RESULTS:

Levofloxacin-induced focal seizure in a patient with multidrug-resistant tuberculosis and chronic kidney disease.

Levofloxacin, a fluoroquinolone antibiotic, is essential in the treatment protocols for multidrug-resistant tuberculosis (MDR-TB). Generally well tolerated, it infrequently induces central nervous system adverse effects, especially in patients with renal impairment. We present a woman in her early 50s with chronic renal disease and hypothyroidism who experienced focal seizures after 48 hours of commencing levofloxacin as part of her multidrug-resistant tuberculosis treatment regimen. The seizures exhibited unilateral aberrant movements with initially intact consciousness, thereafter followed b

Behçet's Disease Mimicking Drug-Induced Mucocutaneous Lesions in a Patient on Anti-tuberculous Drugs.

Behçet's disease is an inflammatory disease characterized by recurrent oral aphthous ulcers and numerous potential systemic manifestations. These include genital ulcers, skin lesions, and diseases affecting the eyes, nervous system, blood vessels, joints, and gastrointestinal tract. The underlying cause of the disease is unknown. As with other autoimmune diseases, the disorder may represent aberrant immune activity triggered by exposure to an agent, perhaps infectious, in patients with a genetic predisposition to develop the disease. We present the case of a 30-year-old male who developed feat

Disseminated talaromycosis with pulmonary tuberculosis coinfection in an immunocompromised patient mimicking disseminated histoplasmosis.

Opportunistic infections in patients with human immunodeficiency virus (HIV) can mimic each other, complicating diagnosis. Here, we report a rare case of disseminated Talaromyces marneffei in a 39-year-old immunocompromised male with HIV and pulmonary tuberculosis. The patient had respiratory symptoms, systemic manifestations, and skin lesions. Clinical and pathological investigations suggested disseminated histoplasmosis; however, microbiological culture confirmed the diagnosis of disseminated talaromycosis. Imaging confirmed central nervous system involvement. Treatment with amphotericin B l
