Temporal and local appearance of alkaline phosphatase activity in early stages of guided bone regeneration. A descriptive histochemical study in humans.

Alkaline phosphatase (ALP) catalyzes the hydrolysis of phosphate esters and it seems to be a prerequisite for normal skeletal mineralization. Also, ALP is the most widely recognized marker of osteoblast phenotypes. By a tissue regenerative technique called Guided Bone Regeneration (GBR), it is possible nowadays to regenerate small bony defects. The aim of the present study was to investigate early events in bone healing and neogenesis by studying histochemically the temporal and local appearance of the marker Alkaline Phosphatase (ALP) in a GBR model system. Nine healthy volunteers (5 males, 4

Intraoral distraction osteogenesis of the mandible in hemifacial microsomia.

PURPOSE: Lengthening of the mandible by distraction osteogenesis is the preferred method for treatment of hemifacial microsomia in children. Use of an intraoral distraction technique and horizontal oblique ramus osteotomy in such patients is presented. PATIENTS AND METHODS: Mandibular ramus lengthening was performed in 11 patients aged 6 to 12 years with hemifacial microsomia. During the age of mixed dentition in hemifacial microsomia patients with a hypoplastic mandible, the unerupted molars buds are located high in the retromolar region and are in danger of being damaged by the osteotomy. Intraoral distribution of Actinobacillus actinomycetemcomitans in young adults with minimal periodontal disease.

The aim of the present study was to investigate the intraoral distribution of Actinobacillus actinomycetemcomitans in young adults with minor signs of periodontal disease but harboring the organisms in the oral cavity. 17 healthy volunteers, 20 to 27 years of age, participated. Samples from mucosal surfaces of the oro-pharyngeal cavity and saliva (n = 221) as well as subgingival plaque from every tooth (n =477) were selectively cultivated for A. actinomycetemcomitans. Species identity and presence of the leukotoxin encoding gene, ltxA, were checked by multiplex polymerase chain reaction. Moreo

Cellular neurothekeoma of the oral mucosa.

Cellular neurothekeoma is an unusual benign neoplasm which, despite its name, is of uncertain origin. This report describes a cellular neurothekeoma of the cheek mucosa, the first at this site. The tumour presented in a 29-year-old man as a discrete mucosal thickening. Histology showed a generally well circumscribed, but unencapsulated, solid tumour which replaced the entire lamina propria and permeated between minor salivary glands and bundles of striated muscle in the submucosa. There was a sub-epithelial Grenz zone. The tumour was composed of nodules of pale, epithelioid cells separated by

The hamster cheek pouch as a model of oral cancer for boron neutron capture therapy studies: selective delivery of boron by boronophenylalanine.

Herein we propose and validate the hamster cheek pouch model of oral cancer for boron neutron capture therapy (BNCT) studies. This model serves to explore new applications of the technique, study the biology and radiobiology of BNCT, and assess the uptake of boron compounds and response of tumor, precancerous tissue, and clinically relevant normal tissues. These issues are central to evaluating and improving the therapeutic gain of BNCT. The success of BNCT is dependent on the absolute amount of boron in the tumor, and the tumor:blood and tumor:normal tissue boron concentration ratios. Within

Trends and spatial distribution of oral cancer mortality in São Paulo, Brazil, 1980-1998.

We assessed oral cancer mortality data in São Paulo to study trends of the disease and its distribution by areas of the city. We standardized death rates by gender and age group, and also supplied complementary information regarding oral cancer incidence. Oral cancer mortality remained stationary at high levels during the study period. Site-specific death rates revealed high figures for two categories of site: tongue and unspecified parts of mouth. Gum cancer death rates--the most easily diagnosed site-specific oral cancer at a routine dental examination--fell sharply, possibly as a consequenc

Case report of a pigmented dentigerous cyst and a review of the literature on pigmented odontogenic cysts.

This paper reports the first case of a dentigerous cyst containing melanin-pigment and melanocytes in the lining epithelium, and the first case of a odontogenic cyst with macroscopically visible pigmentation in the cyst wall. The patient was a 29-year-old Japanese male with a cystic lesion in the left retromolar area of the mandible. Pathologic examination revealed the lesion to be a dentigerous cyst with or without mild surface keratinization, and numerous granules of melanin-pigment were distributed in the basal cells of the epithelial lining. Furthermore, dendritic melanocytes were scattere

Superficial arteriovenous hemangioma of the oral cavity.

OBJECTIVE: Superficial arteriovenous hemangioma (AH) is a benign vascular lesion that often affects the head and neck, but only 3 histologically proven intraoral lesions have been previously reported. The aim of this study was to analyze the clinical and histologic features of 36 oral AHs from 35 patients. STUDY DESIGN: All vascular lesions, other than pyogenic granulomas, accessioned between 1952 and 2000 were retrieved, and clinical details were gleaned from the request forms or, when available, from the case notes. Histologic sections stained with hematoxylin-eosin were reviewed, and all s

G protein activation by endomorphins in the mouse periaqueductal gray matter.

The midbrain periaqueductal gray matter (PAG) is an important brain region for the coordination of mu-opioid-induced pharmacological actions. The present study was designed to determine whether newly isolated mu-opioid peptide endomorphins can activate G proteins through mu-opioid receptors in the PAG by monitoring the binding to membranes of the non-hydrolyzable analog of GTP, guanosine-5'-O-(3-[(35)S]thio)triphosphate ([(35)S]GTPgammaS). An autoradiographic [(35)S]GTPgammaS binding study showed that both endomorphin-1 and -2 produced similar anatomical distributions of activated G proteins i

Implant placement and simultaneous ridge augmentation using autogenous bone and a micro titanium mesh: a prospective clinical study with 20 implants.

This prospective clinical study evaluated bone regeneration around 20 dental implants placed in 15 patients (mean age 39.7 years). Peri-implant bone defects were augmented with autogenous bone grafts harvested intraorally from the mandible (chin or retromolar area). Augmented sites were covered with an individually trimmed micro titanium mesh which was rigidly affixed with microscrews to the residual jaw bone. Height of implant exposure (mean 6.5 mm), i.e. dehiscencies (80%) or fenestrations (20%), and graft height (mean 6.2 mm) were measured in an apico-coronal direction using a periodontal p

A clinicopathologic study of 196 intraoral minor salivary gland tumours.

We present a retrospective study of 196 patients with intraoral minor salivary gland tumours, 128 malignant and 68 benign, diagnosed from 1954 to 1993 in the A. C. Camargo Hospital, São Paulo, Brazil. Sixty-five percent of the cases occurred in the palate, followed by tongue (9.7%) and retromolar area (6.1%). Pleomorphic adenoma was the most common benign tumour, and mucoepidermoid carcinoma was predominant among the malignant tumours. Surgery was the main treatment method and postoperative radiotherapy and radiotherapy alone were used in 40 and 15 patients, respectively. Local recurrence was

Isolation of bacteria from cervical lymph nodes in patients with oral cancer.

Thirty patients with oral mucosal cancer were studied in relation to oral mucosal damage and bacterial translocation to the regional lymph nodes in the neck. All 30 patients (gingiva 11, tongue 13, cheek mucosa four, oral floor two) underwent extensive, clean-contaminated, head-and-neck surgery (including neck dissection) with free flap reconstruction. A total of 153 lymph nodes was harvested for microbial and histological examination. Viable bacteria were isolated from 70 lymph nodes (45.8%) from 25 patients (83.3%). Bacterial cells in the nodes were detected by scanning electron microscopy.

Recurrent TIPS failure associated with biliary fistulae: treatment with PTFE-covered stents.

INTRODUCTION: Sagittal split osteotomy is a standard procedure performed by maxillofacial surgeons. Patients usually present with either aesthetic or functional concerns. Surgical landmark identification is crucial to a successful surgical procedure. Various diagnostic preoperative tools have been used to identify these landmarks, such as imaging studies or cadaveric dissections. The cone-beam computed tomography (CBCT) is currently the most common preoperative tool used. AIM: This study aims to identify whether the deepest part of the ascending ramus (coronoid notch) distance to the mandibul

Horizontal anchorage in the ascending ramus--a technical note.

The ascending ramus provides a volume of bone that can receive horizontal screws of large sizes in the retromolar area, between the internal and external oblique lines. This area is not prone to disuse resorption and the only vital structure to consider is the neurovascular bundle. In this article two systems that make use of this anatomic opportunity are presented. One is an intraoral distractor, used for mandibular lengthening by gradual distraction. The other is a screw anchor that provides absolute anchorage for orthodontic tooth movement.

A surgical approach to extensive tumors in the pterygopalatine fossa extending into the maxillary sinus.

PURPOSE: This article describes a surgical approach to extensive tumors in the pterygopalatine fossa extending into the maxillary sinus. TECHNIQUE: The Barbosa approach was modified by adding a lateral incision in the mandibular gingivobuccal fold from the canine tooth to the retromolar area. RESULTS: This technique allowed a large, inferiorly based flap to be raised, which includes the parotid gland. The masseter and temporalis muscles was divided horizontally, and the ascending ramus of the mandible was osteotomized between the mandibular angle and the sigmoid notch and reflected to expose
