Radiation Therapy Target Objectives for Tonsillar Cancer Treated With Unilateral Radiation Therapy-A Replanning Study From TROG 12.01.

PURPOSE: Unilateral radiation therapy (URT) is an effective treatment strategy in selected patients with lateralized tonsil cancer. However, there is a lack of established planning guidelines for URT treatment, leading to suboptimal optimization of contralateral and midline organs at risk (OARs). This study aimed to reoptimize URT plans to maximize sparing of midline and contralateral OAR's while maintaining target coverage, providing dosimetric guidelines for URT planning. METHODS AND MATERIALS: Treatment plan data for patients treated with URT on TROG 12.01 were reoptimized using Eclipse V1

Quantitative magnetic resonance imaging analysis of salivary glands: effects of age and number of remaining teeth.

The purpose of this study was to assess the influence of aging and number of remaining teeth on apparent diffusion coefficient (ADC) values of the submandibular and parotid glands. A total of 481 participants (230 men, 251 women) aged 20-89 years (mean: 53.70 years) were included. ADC values of the salivary glands were compared between men and women using the Mann-Whitney U test. Spearman's correlation coefficients were calculated using patient ages and number of remaining teeth as the explanatory variables and the ADC values as the outcome variables. In addition, multiple linear regression

Ultrasonographic Evaluation of Salivary Gland Stiffness in Primary Sjögren's Disease and Idiopathic Xerostomia.

BACKGROUND: Ultrasonography is widely used to evaluate salivary gland involvement in primary Sjögren's disease (SjD). However, distinguishing SjD-related dry mouth from idiopathic xerostomia (IX) remains challenging. This study evaluates whether elastography, an advanced ultrasonographic technique measuring gland stiffness, can improve differentiation. METHODS: In this retrospective study, 192 patients diagnosed with SjD (n = 83) or IX (n = 109) were assessed for xerostomia severity and underwent conventional, Doppler ultrasonography, and shear wave elastography (SWE) of salivary glands. Addi

Modulating O-GlcNAcylation Alters Salivary Acinar Cell Differentiation.

O-GlcNAcylation is a post-translational modification involved in various cellular processes, including cell cycle progression, signaling, transcription, and stress response. Mouse salivary gland morphogenesis shows specific localization patterns of O-GlcNAc transferase (OGT) and O-GlcNAc in developing acinar cells, suggesting a potential involvement of O-GlcNAcylation in acinar cell differentiation-related signaling molecules. To define its underlying mechanisms, this study used an OGT inhibitor, OSMI-1, and small interfering RNA (siRNA) targeting OGT, during in vitro cultivation of submandibu

Hedgehog-Wnt Crosstalk Orchestrates Submandibular Gland Morphogenesis in Mice.

The development of the mouse submandibular gland (SMG) is a classic model for branching morphogenesis, orchestrated by key signaling pathways such as Hedgehog (Hh) and Wnt. While Sonic Hedgehog (Shh) is known to be essential for early SMG development, its downstream mechanisms and potential crosstalk with other pathways remain unclear. This study investigates the functional interaction between Hh and Wnt signaling during SMG morphogenesis. Using both in vivo (pharmacological inhibition in pregnant mice) and ex vivo (organ culture) models, we inhibited Hh signaling with vismodegib from embryoni

Clinical and Prognostic Factors of Submandibular Gland Malignancies: A SEER-Based Analysis : Analysis of Submandibular Gland Tumors.

OBJECTIVE: While neoplasms of the submandibular gland (SMG) are rare, there is a nearly 50% risk of malignancy and its neoplasms are associated with poorer prognoses. Due to the rarity of these tumors, clinical and prognostic information relating to these tumors is limited; we aimed to elucidate these factors using an analysis of the Surveillance, Epidemiology, and End Results (SEER) database. METHODS: A retrospective population-based analysis was performed with data regarding primary SMG malignancy from 2000 to 2019 as extracted from the SEER database. Kaplan-Meier curves and log-rank tests

First-in-human study to investigate the safety and efficacy of effective-mononuclear cell therapy for radiogenic xerostomia.

BACKGROUND: Salivary gland (SG) hypofunction is the most common complication following radiotherapy for head and neck cancer. Currently, there are no effective therapies for radiation-induced xerostomia. We recently developed a novel therapy for preclinical studies using effective-mononuclear cells (E-MNCs) induced from autologous peripheral blood mononuclear cells (PB-MNCs) via a new primary culture system for treatment of radiation-induced xerostomia. However, the safety and effectiveness of E-MNC therapy have not been assessed in humans. The objective of this first-in-human study was to eva

Weakly supervised treatment selection: Machine learning models for appropriate surgical planning of submandibular stones.

There is a gap in real-world clinical adoption of machine learning (ML) solutions due to the inherent uncertainty and variability in treatment outcomes. To bridge this gap, we present a novel approach to the problem of medical treatment selection using ML models and we apply it to the case of submandibular sialolithiasis treatment. The study introduces a weakly supervised learning framework which allows for the inclusion of imprecise, incomplete, or noisy ground truth data. By applying this methodology to the specific medical problem of submandibular stone treatment, we demonstrate the potenti

Modulatory effects of Withania somnifera supplementation on p38 MAPK/NF-κB/COX-2 and TRPC1/CHOP signaling pathways in methotrexate-induced parotid and submandibular salivary gland toxicity in rats.

Methotrexate (MTX), a chemotherapeutic and immunosuppressive drug, is well known to cause salivary gland (SG) toxicity. Although Withania somnifera (WS) exhibits strong antioxidant and cytoprotective properties, its potential to counteract MTX-induced SG injury remains poorly explored. The current study aimed to assess the protective effect of WS supplementation against MTX-induced toxicity in rat parotid and submandibular glands and to explore the underlying mechanisms. Forty adult male albino rats, each weighing ~ 200 g, were randomly allocated into four groups (n = 10): Control (distilled w

Dysphagia After Cosmetic Submandibular Gland Botulinum Neurotoxin Type A Injection: A Case Report.

Background: Cosmetic injection of botulinum neurotoxin type A (BoNT/A) into the submandibular glands is increasingly performed to enhance jawline contour. Although generally considered safe, unintended diffusion of the toxin can impair pharyngeal musculature and lead to dysphagia. Severe aspiration-prone dysphagia after esthetic submandibular gland injection has rarely been described. Case Presentation: A healthy 37-year-old woman developed acute oropharyngeal dysphagia the day after receiving cosmetic contouring injections with incobotulinumtoxinA (Xeomin®), administered to both submandibular

Granzyme A-producing type 1 innate lymphoid cells promote salivary secretion in the aged submandibular gland.

BACKGROUND: Salivary gland function declines with age, contributing to periodontitis and aspiration pneumonia. However, the cellular mechanisms that preserve secretion during aging remain unclear. METHODS: Mice were stratified by aging stage, and saliva output and tissue histology were assessed. To infer cell-cell communication, regions surrounding lymphocyte clusters were microdissected and subjected to site-specific single-cell RNA sequencing. Epithelial cell responses were evaluated in a coculture system using a neutralizing antibody. Finally, physiological relevance was assessed by in viv

Pancytopenia and splenomegaly as uncommon manifestations in a patient with immunoglobulin G4-related disease.

Haematological manifestations of immunoglobulin (Ig)G4-related disease (IgG4-RD) are atypical, and few studies have addressed cytopenia in this condition. We present here a case of IgG4-RD with marked pancytopenia and splenomegaly in the absence of bone marrow abnormalities. A 67-year-old woman developed diplopia, dyspnoea, pancytopenia, and hypocomplementemia. Contrast-enhanced computed tomography showed bilateral lacrimal and submandibular gland swelling, enlargement of the cervical, mediastinal, inguinal, and para-aortic lymph nodes, splenomegaly, and granular shadows in both lungs. She had

Sialography and endoscopy-based classification of salivary duct stenosis.

OBJECTIVES: Salivary duct stenoses account for approximately 15-25% of salivary gland obstructions. A well-recognized classification criterion for these stenoses has yet to be established. The present study aims to classify and characterize salivary duct stenoses based on sialography and sialendoscopy. STUDY DESIGN: The sialograms and endoscopic findings in 307 patients presenting with primary salivary duct stenosis were respectively reviewed. The location, grade of luminal narrowing, length, and number of the stenoses were assessed, and stenosis severity of each gland was categorized. RESUL

3D Neck Lift: A Dynamic Approach to Submental Anatomy.

BACKGROUND: Traditional neck rejuvenation-liposuction, platysmaplasty, and skin redraping-overlooks intrinsic mandibular-cervical asymmetry, digastric insertions, platysmal discontinuity, submandibular gland descent, and age-related hyoid retrodisplacement. These influence submental contour and require a tridimensional, anatomy-based correction. METHODS: A retrospective study of 177 patients (99 women, 78 men; mean age 59 y, range 26-78 y) undergoing isolated neck rejuvenation (2003-2024) assessed outcomes of the 3D neck lift, which has been used since 2018 in Knize stage II-III and stage IV c
