BACKGROUND: Central nervous system (CNS) infections represent serious medical emergencies. The development of syndromic tests has improved clinical management. The FilmArray® Meningitis/Encephalitis (ME) panel (BioFire Diagnostics, Salt Lake City, Utah) is an automated multiplex polymerase chain reaction (PCR) assay capable of simultaneously detecting 14 CNS pathogens from small cerebrospinal fluid (CSF) volumes. This study aimed to assess the diagnostic performance of this panel compared with conventional microbiological methods.
METHODS: A retrospective diagnostic accuracy study was conduct

BACKGROUND: Immune aging may contribute to Alzheimer's disease. Bacillus Calmette-Guérin (BCG), a vaccine known to induce trained immunity, has been linked to reduced Alzheimer's risk in prior studies. However, whether trained immunity can be observed in the human central nervous system remains unclear. We assessed whether BCG induces trained immunity-like responses in adults with and without Alzheimer's-related changes.
METHODS: We conducted two related one-year, open-label clinical trials in adults aged 55 years or older (n = 12 without Alzheimer's-related pathology; n = 11 with Alzheimer's

RATIONALE: The study is aimed at exploring the complex clinical scenario of a patient with systemic lupus erythematosus who developed a rare coinfection with Listeria monocytogenes and Mycobacterium tuberculosis. The rationale is to highlight the diagnostic and therapeutic challenges in managing such a case, particularly in the context of immunosuppression and the need for effective antimicrobial therapy. This case underscores the importance of advanced diagnostic techniques like metagenomic next-generation sequencing in identifying coinfections and the critical balance required in treating bo

Background Tuberculosis (TB) continues to be a significant contributor to childhood morbidity and mortality in high-burden countries, including India. Host nutritional status plays an important role in susceptibility to Mycobacterium tuberculosis, particularly through the modulation of cell-mediated immunity. Beyond its classical role in calcium homeostasis, vitamin D has important immunomodulatory effects, including enhancement of macrophage activity, promotion of phagosome-lysosome fusion, and induction of antimicrobial peptides such as cathelicidin. Although several studies in adults have r

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB) impose a disproportionate burden on high-endemic countries, with India contributing the largest global share of rifampicin-resistant cases. The National TB Elimination Program (NTEP) introduced longer all-oral regimens to replace injectable-based therapy; however, prospective pharmacovigilance data characterizing the real-world adverse drug reaction (ADR) profile of these regimens from India remain scarce.
OBJECTIVE: To prospectively evaluate the incidence, nature, system organ class dis

INTRODUCTION: Brain parenchymal infections encompass a broad clinicopathological spectrum caused by bacteria, mycobacteria, fungi, and parasites, presenting non-specifically with headache, fever, seizures, focal neurological deficits, and altered sensorium. Mycobacterium tuberculosis remains the most prevalent causative organism in India, while opportunistic pathogens, including Cryptococcus, Mucor, and Zygomycetes species, are increasingly encountered in immunocompromised hosts. Although neuroimaging and microbiological investigations are integral to evaluation, imaging features frequently ov

Central nervous system (CNS) tuberculosis (TB) is an uncommon but severe manifestation of Mycobacterium tuberculosis infection and is associated with significant morbidity and mortality. Its clinical and radiologic presentations are often nonspecific and may mimic malignancy, inflammatory disorders, or fungal infections, frequently resulting in diagnostic delay. Conventional cerebrospinal fluid (CSF) testing has limited sensitivity, and definitive diagnosis may require histopathologic confirmation when noninvasive studies are inconclusive. We report the case of a 76-year-old woman with a histo

BACKGROUND: Bedaquiline, pretomanid, and linezolid (BPaL) is a 6-month regimen that has revolutionized treatment of multidrug-resistant (MDR) tuberculosis (TB). However, there is limited evidence of using BPaL to treat MDR tuberculous meningitis (TBM), a devastating illness that has high risk of mortality and permanent disability.
METHODS: A patient with fluoroquinolone-resistant pre-extensively drug-resistant (pre-XDR) TBM was cured with a 10-month regimen based on BPaL with clofazimine and cycloserine. Total concentrations of bedaquiline, pretomanid, and linezolid were measured in plasma an

UNLABELLED: Distinguishing tuberculous meningitis (TBM) from other causes of meningitis remains challenging in high-burden settings because clinical features overlap and existing assays often delay etiologic confirmation. We evaluated MeltArray CNS, a highly multiplexed PCR assay targeting 85 meningitis-associated pathogens, for etiologic diagnosis in patients with suspected meningitis. Analytical performance was assessed using pre-characterized targets. MeltArray was compared head-to-head with metagenomic next-generation sequencing (mNGS) in 79 cerebrospinal fluid (CSF) specimens, with discre

BACKGROUND: Tuberculous meningitis (TBM) is a severe central nervous system infection in which dysregulated host inflammation contributes to neurological injury. The cellular organization of the cerebrospinal fluid (CSF) immune microenvironment in TBM, particularly in comparison with other inflammatory meningitis conditions, remains incompletely defined. This study characterized CSF immune-cell heterogeneity in TBM and purulent meningitis (PM) using single-cell transcriptomic profiling.
METHODS: Pre-treatment CSF immune cells from five patients with TBM and five patients with PM were profiled

Intracranial tuberculomas are an exceptionally rare manifestation of tuberculosis in Qatar and the Gulf Cooperation Council (GCC) region; their presentation can closely mimic other intracranial pathologies, posing diagnostic challenges. A 24-year-old Indian woman presented with headache and new-onset seizures, with a prior history of mediastinal lymphadenopathy. Neuroimaging revealed multiple intracranial ring-enhancing lesions associated with surrounding edema and midline shift. Based on the clinical and radiological findings, anti-tuberculosis therapy was initiated promptly. Serial follow-up

BACKGROUND: Paragonimiasis is a food-borne parasitic disease caused by lung flukes belonging to the genus Paragonimus. It can produce pulmonary, pleural, and other organ manifestations that overlap clinically, radiologically, and histopathologically with malignancy, tuberculosis, and immune-mediated disorders. Elevated total serum immunoglobulin G4 (IgG4) concentrations and IgG4-positive plasma cell infiltration may mimic IgG4-related disease (IgG4-RD), but quantitative serum IgG4 levels in paragonimiasis have not been systematically characterized.
METHODS: We conducted a multi-database scopi

BACKGROUND: Tuberculous meningitis (TBM) is the most severe form of central nervous system tuberculosis, associated with high mortality and neurological sequelae. Microglia-driven neuroinflammation is a key contributor to TBM pathogenesis; however, its specific effects on ependymal cells-critical for cerebrospinal fluid dynamics and barrier function-and potential therapeutic strategies remain unclear.
METHODS: A murine TBM model was established by tail vein injection of BCG. Although the virulence of BCG, an attenuated strain of Mycobacterium bovis, is different from that of clinically isolat

BACKGROUND: Intravesical Bacille Calmette-Guerin (BCG) therapy with live-attenuated form of Mycobacterium bovis (M. bovis) is used as a treatment of early-stage bladder cancers. Documented adverse reactions are usually local and time limited, with severe central nervous system (CNS) complications being exceedingly rare. We report an 88-year-old patient with papillary urothelial carcinoma who presented with recurring neurological complications for over a year after treatment with intravesical BCG therapy.
CASE SUMMARY: MRI at initial presentation revealed a dural-based left frontoparietal mass

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. BACKGROUND: Systemic sarcoidosis can occur without pulmonary involvement and may present exclusively with extrapulmonary manifestations, thereby mimicking infections, autoimmune diseases, or malignancies.
CASE: The case of a 60-year-old female patient with pronounced cachexia (BMI 14 kg/m2), hypercalcemia, and multiorgan involvement is presented. Histology revealed non-caseating granulomas in gastric biopsies (granulomatous gastritis) as well as granulomatous interstitial nephritis with a tubular proteinuria pattern on renal biopsy. Imaging revealed no evidence of pulmonary involvement. Marke

BACKGROUND: Second-hand smoke (SHS) exposure remains a major source of morbidity and mortality among non-smokers. This study presents the first dedicated Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) publication to systematically quantify SHS prevalence and evaluate its effect as a global risk factor across a broader range of outcomes.
METHODS: Using the GBD 2023 comparative risk assessment framework, we estimated SHS exposure prevalence and attributable disease burden across 204 countries and territories from 1990 to 2023, stratified by age and sex. Exposure estimates wer

Concurrent infection of the central nervous system by Mycobacterium tuberculosis and Streptococcus pneumoniae is rare and poses substantial diagnostic and therapeutic challenges due to the overlap of clinical symptoms and cerebrospinal fluid (CSF) findings. A 45-year-old male presented with complaints of fever, headache, vomiting, generalized seizures, and altered sensorium. Magnetic resonance imaging of the brain was normal, and CSF analysis showed lymphocytic pleocytosis, protein elevation, and elevated levels of adenosine deaminase.
