BACKGROUND: Tuberculous meningitis (TBM) is the most severe and fatal form of tuberculosis (TB). Linezolid (Lzd), a key oxazolidinone with excellent central nervous system penetration, is widely used for TBM but is limited by significant adverse events (AEs). Contezolid (Czd), a next-generation oxazolidinone structurally optimized from Lzd, has shown promising potential. However, clinical evidence for its use in TBM remains limited to isolated case reports.
METHODS: We conducted a retrospective study of TBM patients treated with Czd- or Lzd-containing regimens. Patients were categorized into

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

INTRODUCTION: Multiple sclerosis (MS) is a chronic autoimmune disease of the central nervous system, influenced by both genetic and environmental factors. Among the environmental factors, early childhood self-medication may be relevant in MS development. This study aimed to evaluate the association between MS risk and self-medication practices before the age of 15.
METHODS: Under a case-control approach, 260 demographically matched healthy controls from the Azeri community and 469 MS patients completed a comprehensive questionnaire on childhood self-medication history. Regarding the kind and

BACKGROUND: Multiple sclerosis (MS) is an autoimmune disease affecting the central nervous system. Following the elucidation of the role of B cells in MS pathogenesis, B-cell-depleting agents such as Rituximab have become increasingly utilized. However, the relationship between rituximab therapy and susceptibility to tuberculosis (TB) remains a subject of ongoing investigation, with conflicting evidence in the literature.
CASE PRESENTATION: A 55-year-old Iranian woman with multiple sclerosis who had received rituximab for five years presented with high-grade fever, chills, and suprapubic pain

Diffuse leptomeningeal glioneuronal tumors (DLGNT) are rare central nervous system neoplasms characterized by the absence of a primary brain or spinal cord tumor. Patients exhibit common radiological and histopathological features; however, the clinical progression varies. We present the case of a 4-year-old girl initially diagnosed with constipation and failure to thrive, who subsequently developed neurological signs during hospitalization. Subsequently, she was referred with suspected tuberculosis meningitis, despite negative cerebrospinal fluid (CSF) culture results, and the patient presuma

Central nervous system (CNS) lymphoproliferative disorders comprise a heterogeneous group of rare entities that present major diagnostic and therapeutic challenges. Primary CNS large B-cell lymphoma (LBCL) is the most common form and the second most frequent primary CNS tumor after gliomas. The 5th edition of the World Health Organization Classification of Hematolymphoid Tumours introduced updated terminology, including primary LBCL of immune-privileged sites and lymphomas arising in immunodeficiency/dysregulation, along with less common variants such as intravascular large B-cell lymphoma, ly

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Some plants popularly employed for the treatment of peptic ulcers have proved to be attractive sources of new drugs. Despite extensive research, the pharmacological and toxicological potentials of these plants are not fully understood. In this context, the aim of this work was to analyze the multielemental composition of the methanolic extracts of three of those plants, Alchornea glandulosa (AG), Davilla elliptica (DE) and Davilla nitida (DN), with the intention of contributing to the understanding of the mechanisms of action of these extracts. For this purpose, we used the analytical techniqu

During and after the COVID-19 pandemic, Tuberculosis (TB) has reestablished with higher figures due to interruptions in the Directly Observed Treatment Short course (DOTS) despite underreporting. The rising consequences would have extended to extra-pulmonary forms of TB as well, including Tuberculous Meningitis (TBM). Considering the fact that TBM is the most dangerous and worst form of TB, we found the need to scan the literature to highlight various aspects of TBM. Epidemiology of TBM is proportionally less frightening, but the consequent mortalities and morbidities are more alarming than pu

BACKGROUND: Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. The current treatment protocols for pulmonary tuberculosis are quite effective, even though the treatment requires 3-6 months. The current treatment protocols for extrapulmonary tuberculosis are based on the same drugs that are used for pulmonary tuberculosis. However, the success rates are much lower for certain types of extrapulmonary tuberculosis, such as tuberculous meningitis. Tuberculous meningitis is one of the very few diseases attributable to bacteria that have a very high short-term mortality rate

A 39-year-old man received empiric treatment for pulmonary tuberculosis (TB). After developing sensory seizures he was restarted on anti-TB drugs when a brain MRI showed a 4.3 cm left parietal enhancing lesion with extensive edema. After TB treatment, imaging showed a reduction in size and edema. Later, he developed headache and seizures, and MRI showed recurrent edema and an enlarging lesion. Neurosurgery decided to biopsy the lesion to obtain a diagnosis. At craniotomy, a frozen section was reported as granulomatous inflammation. However, he returned postOP with a new right hemiparesis and M

OBJECTIVES: Complete removal of the tumor and surrounding tissue is the most important prognostic factor such as survival after surgery. When the tumor invades the phrenic nerve, the impact of intraoperative phrenic nerve sacrifice on the short- and long-term prognosis of patients is not clear. This study aims to explore the differences in prognosis between patients with malignant thymoma with and without phrenic nerve sacrifice during surgery, as well as analyze related factors.
METHODS: A total of 209 patients who underwent thymoma resection in the Department of Thoracic Surgery at our hosp

BACKGROUND: HIV and tuberculosis (TB) co-infection poses a significant health challenge, particularly when involving the central nervous system (CNS), where it leads to severe morbidity and mortality. Current treatments face challenges such as drug resistance, immune reconstitution inflammatory syndrome (IRIS), and persistent inflammation. Glutathione (GSH) has the therapeutic potential to enhance treatment outcomes by improving antibiotic efficacy, reducing inflammation, and mitigating immune dysfunction.
METHODS: Relevant studies were identified through systematic searches of PubMed, Elsevi

Bacterial meningitis is a severe and life-threatening infection of the central nervous system (CNS), primarily caused by Streptococcus pneumoniae and Neisseria meningitidis. This condition carries a high risk of mortality and severe neurological sequelae, such as cognitive impairment and epilepsy. Pain, a central feature of meningitis, results from the activation of nociceptor sensory neurons by inflammatory mediators or bacterial toxins. These nociceptors, abundantly present in the meninges, trigger neuroimmune signaling pathways that influence the host immune response. However, the mechanism

Tuberculosis (TB) remains a significant global health threat with high mortality and efforts to meet WHO End TB Strategy milestones are off-track. It has become clear that TB is not a dichotomous infection with latent and active forms but presents along a disease spectrum. Subclinical TB plays a larger role in transmission than previously thought. Aerosol studies have shown that undiagnosed TB patients, even with paucibacillary disease, can be highly infectious and significantly contribute to TB spread. Encouraging clinical results have been seen with the M72/AS01E vaccine. If preliminary resu
