Imaging Modalities in Tuberculosis.

Tuberculosis (TB) remains a major global health challenge, with increasing prevalence of multidrug-resistant and extrapulmonary forms complicating diagnosis and management. Imaging plays a pivotal role in the early detection, characterization, and treatment monitoring of TB, particularly when clinical or microbiological findings are inconclusive. Modalities such as chest radiography, computed tomography (CT), ultrasound, magnetic resonance imaging (MRI), and 18F-fluorodeoxyglucose positron emission tomography combined with CT (18F-fluorodeoxyglucose [18F-FDG PET-CT]) provide critical insights

Right Temporal Lobe Tuberculoma Presenting as Seizures in a Patient With Pulmonary Tuberculosis.

BACKGROUND Central nervous system (CNS) manifestations of tuberculosis (TB) account for approximately 1-2% of all TB cases worldwide. Tuberculomas, which are granulomatous lesions of the CNS diagnostically confirmed through biopsy, can mimic more common intracranial pathologies, including abscesses and neoplasms. This report describes a patient with pulmonary TB who presented with seizure and was ultimately diagnosed with a right temporal lobe TB mimicking a brain abscess. CASE REPORT A 30-year-old woman with alcohol use disorder, polysubstance use, and pulmonary TB previously treated with rif

Impact of High-Dose Rifampicin on Linezolid Pharmacokinetics in Tuberculous Meningitis.

BACKGROUND: Tuberculous meningitis (TBM), a severe form of extrapulmonary tuberculosis, requires sustained therapeutic drug concentrations in the central nervous system (CNS). Linezolid is a promising treatment for TBM due to excellent CNS penetration and bactericidal activity against Mycobacterium tuberculosis. However, co-administration with rifampicin may alter linezolid pharmacokinetics (PK), potentially reducing efficacy due to drug-drug interactions. METHODS: We utilized data from the Adjunctive Linezolid for the Treatment of TubERculous Meningitis trial, a phase II study evaluating hig

Meningeal tuberculoma presenting as a space-occupying lesion in an immuno-competent young adult patient: A case report and literature review.

Meningeal tuberculoma is a mass-like lesion formed by the immune response to Mycobacterium tuberculosis, involving the brain parenchyma, meninges, or both, that is characterized by central caseous necrosis surrounded by inflammatory cells and fibrosis. It is a rare but serious form of central nervous system (CNS) tuberculosis and can occur with or without tuberculous meningitis. We report a case of a 25-year-old female who presented with seizures and focal neurological deficits. Neuroimaging revealed a ring-enhancing lesion suggestive of a neoplastic process. The patient underwent left frontal

Non-leukemic myeloid sarcomas involving only the central nervous system: a case report and literature review.

A previously healthy 39-year-old male presented with a five-month history of dizziness and progressive numbness and weakness of the lower limbs for more than two months. Contrast-enhanced MRI revealed multiple enhancing soft tissue masses in the spinal canal, while PET/CT indicated numerous hypermetabolic lesions in the intracranial and spinal regions. Initial pathological findings suggested inflammatory lesions, and the diagnosis remained uncertain after consultations at multiple hospitals. The patient was admitted to our hospital on June 15, 2019. Subsequent examinations after admission rule

Intranasal administration in modulating depressive-like behavior and reconstructing treatment paradigms through neuroinflammation and neurotrophic pathways.

Intranasal administration, as a non-invasive brain-targeted delivery strategy, offers a promising approach for the treatment of central nervous system disorders, particularly depression. Traditional oral or intravenous drug delivery is often limited by the restrictive permeability of the blood-brain barrier and the hepatic first-pass effect, which hinder effective drug accumulation in the brain. In contrast, the intranasal route leverages the olfactory and trigeminal neural pathways to enable direct drug transport from the nasal cavity to the brain, effectively bypassing the blood-brain barrie

Mycobacterium abscessus Cervical Lymphadenitis Mimicking Tuberculosis in China: A Case Report.

Mycobacterium abscessus (Mab) is recognized as the most pathogenic mycobacterium affecting humans, comprising three phylogenetic subspecies: M. abscessus subsp. abscessus, M. abscessus subsp. bolletii, and M. abscessus subsp. massiliense. Its clinical manifestations are varied and frequently subject to misdiagnosis. Mab exhibits both intrinsic and acquired resistance to numerous antibiotics, which results in limited therapeutic options and poor clinical outcomes. This report details a case of cervical lymphadenitis caused by Mab, initially misdiagnosed as tuberculosis. During anti-tuberculosis

Comparative metabolic profiling of pediatric cerebrospinal fluid reveals disease-specific biomarkers in HIV and TB meningitis.

HIV enters the central nervous system (CNS) early, disrupting neurometabolism and inducing chronic neuroinflammation that results in the development of neurocognitive symptoms and neurodevelopmental delay in children infected with HIV. In children, neurocognitive presentation is often more severe than in adults, due to early and prolonged exposure to the virus. Tuberculous meningitis (TBM), another major pediatric CNS infection and often an opportunistic co-infection in HIV, carries high morbidity and mortality but remains difficult to diagnose with existing tests. Metabolomics provides a prom

Prevalence of Cervical Lymphadenopathy in Acute CNS Infections - Testing Glymphatics in Humans.

BACKGROUND: Glymphatics in the brain have recently been shown to drain cervical lymph nodes in animal models. Diagnosis of acute central nervous system (CNS) infections is limited by pauci-microbial nature of the cerebrospinal fluid (CSF). Evaluation of cervical lymphadenopathy in acute CNS infections may aid in diagnosis and provide evidence of glymphatics. OBJECTIVE: To study the prevalence of cervical lymphadenopathy in acute CNS infections and to assess diagnostic yield by using cervical lymphadenopathy biopsy/FNAC. DESIGN, SETTING AND PARTICIPANTS: A cross-sectional study was conducted

Multifocal tuberculosis with cerebral, spinal, pulmonary and abdominal involvement in a 16-year-old girl: A radiological case report.

Multifocal tuberculosis involving the central nervous system and systemic organs is uncommon but well documented, particularly in endemic regions and in the setting of incomplete treatment. We report the case of a 16-year-old girl with prior pulmonary tuberculosis and poor therapeutic adherence who presented with progressive paraparesis over 3 weeks. Brain Magnetic resonance imaging demonstrated multiple supratentorial enhancing lesions consistent with solid granulomatous tuberculomas, without significant surrounding edema. Associated leptomeningeal and pachymeningeal enhancement was also obse

Bedaquiline inhibits the ATP synthase leak channel and prevents glutamate-induced neuronal death.

FOF1-ATP synthase is one of the most abundant proteins of the mitochondrial inner membrane and the primary enzyme responsible for ATP production in eukaryotic cells. Nevertheless, it was recently reported to play a prominent role in cell death by forming a large-conductance leak channel in the mitochondrial permeability transition pore (mPTP), making it a promising therapeutic target. Bedaquiline (BDQ), a member of the diarylquinoline class of drugs, was shown to selectively inhibit the catalytic activity of Mycobacterium tuberculosis ATP synthase with no effect on the mammalian enzyme. Here,

Cerebellar Tuberculoma as a Rare Manifestation of Central Nervous System Tuberculosis: Literature Review and Clinical Case.

Central nervous system tuberculosis (CNS-TB) is a severe extrapulmonary manifestation of tuberculosis associated with high morbidity and mortality, particularly in children. While tuberculous meningitis remains the predominant form, focal parenchymal lesions such as tuberculomas and abscesses are less common. Cerebellar tuberculomas represent an exceptionally rare localization and may mimic posterior fossa tumors, leading to diagnostic delay. This article provides an updated review of CNS tuberculosis with special emphasis on pediatric epidemiology and cerebellar involvement, and presents a cl

Clinical characteristics of central nervous system talaromycosis: A comparative analysis with cryptococcal meningitis and tuberculous meningitis in patients with AIDS.

BACKGROUND: The incidence of central nervous system (CNS) talaromycosis, an emerging complication in patients with acquired immunodeficiency syndrome (AIDS), is increasing, accompanied by a high mortality rate. Limited understanding of whether its clinical manifestations differ from those of common AIDS-associated CNS infections, such as cryptococcal meningitis (CM) and tuberculous meningitis (TBM), makes diagnosis challenging. Therefore, we aimed to systematically elucidate the key differences in clinical manifestations, diagnostic approaches, and treatment strategies among CNS talaromycosis,

Clinical and hematological factors associated with neurological involvement in spinal tuberculosis: a retrospective study.

BACKGROUND: The association between peripheral blood immune-inflammatory markers and neurological involvement in patients with spinal tuberculosis, and to develop a predictive model for early risk assessment. Spinal tuberculosis with neurological impairment is a major cause of disability, yet simple, cost-effective early warning indicators remain lacking. Peripheral blood immune-inflammatory markers are valuable in infectious disease prognostication, but their predictive role in spinal tuberculosis neurological involvement remains unclear. METHODS: A retrospective study was conducted on 294 p

Bacillus Calmette-Guérin (BCG) Vaccination and the Immune-Brain Axis: Implications for Neuroprotection and Neurodegenerative Disease.

The Bacillus Calmette-Guérin (BCG) vaccine, originally developed for tuberculosis (TB) prevention, has recently attracted attention due to its broader immunomodulatory properties. In addition to its role in TB control, BCG induces trained immunity, a process involving epigenetic and metabolic reprogramming of innate immune cells that leads to altered systemic inflammatory responses. Increasing evidence suggests that these long-term immune adaptations may influence the central nervous system by modulating microglial activation and neuroinflammatory pathways implicated in neurodegenerative disea
