Is Short Therapy an Appropriate Regimen for Children and Young Adolescents with Drug-Susceptible Tuberculosis?

Background: Tuberculosis (TB) remains a major cause of morbidity and mortality among children worldwide, with approximately one million new pediatric cases annually. The conventional treatment for drug-susceptible TB has long relied on a 6-month multidrug regimen, which is highly effective but associated with challenges in adherence, toxicity, and healthcare burden. Objectives: To evaluate whether short-course therapy is an appropriate regimen for children and young adolescents with drug-susceptible TB, with particular focus on its efficacy, safety, and applicability in different clinical cont

Temporally Resolved Single-Cell RNA Sequencing Reveals Pathogenesis and Immune Responses in Intracerebral Bacille Calmette-Guérin (BCG) Infection.

Background: In some children with immunodeficiency, Bacille Calmette-Guérin (BCG) vaccination can lead to dissemination and severe infection, including severe intracranial infection, called disseminated BCG disease (BCGosis), which is characterized by high rates of disability and mortality. However, the specific routes by which BCG crosses CNS barriers and the patterns of temporal remodeling of the CNS immune microenvironment during infection have yet to be fully elucidated. Methods: Mice were infected with BCG through tail vein injection to construct an intracerebral mycobacterial infection m

[Chinese guideline for the chemotherapy of multidrug-resistant/rifampicin-resistant tuberculosis (2026 edition)].

Multidrug-resistant tuberculosis (MDR-TB) and rifampicin-resistant tuberculosis (RR-TB) pose significant challenges to global tuberculosis prevention and treatment efforts. At present, the chemotherapy of MDR/RR-TB is still hampered by prolonged treatment duration and suboptimal success rates. In 2019, the Chinese Medical Association Tuberculosis Branch issued the Chinese Expert Consensus on the Treatment of MDR-TB and RR-TB, which has provided essential guidance for standardizing the diagnosis, treatment, and clinical management of MDR/RR-TB in China. In response to recent advances in global

Ring-enhancing demyelinating lesions in the brain and optic neuritis: atypical presentation of paediatric neuropsychiatric SLE.

Paediatric-onset systemic lupus erythematosus (SLE) is a rare but clinically significant condition, often presenting with central nervous system (CNS) involvement. The most common CNS manifestations include headaches, seizures and psychosis. We present two cases of neuropsychiatric SLE with unusual clinical and imaging features. The first case involves an adolescent girl who presented with unilateral vision loss and elevated intracranial pressure. MRI revealed large well-defined ring-enhancing lesions in the bilateral subcortical white matter. She underwent extensive evaluation for infectious,

Brainstem Tuberculous Abscess in a 63-Year-Old Adult With Unrepaired Tetralogy of Fallot.

BACKGROUND: Tetralogy of Fallot (TOF) is a congenital heart anomaly usually corrected surgically in childhood. Reports of adults with unrepaired TOF indicate that they are at high risk of endocardial infections and sequelae such as distant abscesses. CASE PRESENTATION: A 63-year-old man born and raised in Vietnam presented with unrepaired TOF and 5 days of dizziness, dysarthria, and left-sided weakness. Magnetic resonance imaging (MRI) of the brain demonstrated a 2.9-cm rim-enhancing pontine lesion with associated edema and a punctate acute infarct in the right cerebellum. Echocardiogram of t

Radiographic resolution of presumed cerebellar tuberculoma during extended BPaLM-based therapy for disseminated multidrug-resistant tuberculosis.

PURPOSE: Central nervous system (CNS) involvement in multidrug-resistant tuberculosis (MDR-TB) is associated with high morbidity, and evidence guiding the use of standardized all-oral regimens in intracranial disease is limited. We describe radiographic evolution of a presumed cerebellar tuberculoma during BPaLM-based therapy for MDR-TB. METHODS: We report the clinical course, microbiologic data, treatment regimen, and serial neuroimaging of a man in his 30s with pulmonary MDR-TB, pleural involvement, and a small peripherally enhancing cerebellar lesion compatible with a tuberculoma. RESULTS

Concurrent Spinal Dural Arteriovenous Fistula and Varicella-Zoster Virus Meningoencephalitis Unmasked by Corticosteroid-Associated Deterioration: A Case Report on the Diagnostic Value of Serial mNGS.

BACKGROUND: Concurrent spinal dural arteriovenous fistula (SDAVF) and varicella-zoster virus (VZV) meningoencephalitis are exceptionally rare, and overlapping features can delay diagnosis. This case adds to the literature by illustrating how corticosteroid exposure before exclusion of vascular and infectious mimics may be followed by neurological deterioration, and by emphasizing the diagnostic value of serial metagenomic nextgeneration sequencing (mNGS). CASE PRESENTATION: A 48-year-old man developed insidious bilateral lower-limb weakness that progressed to numbness, sphincter dysfunction,

Comparison of three linezolid management strategies for peripheral neuropathy in multidrug- or rifampicin-resistant tuberculosis treatment: a target trial emulation.

BACKGROUND: Peripheral neuropathy frequently leads to linezolid dose reductions or interruptions during multidrug- or rifampicin-resistant tuberculosis (MDR/RR-TB) treatment. The implications of these modifications on treatment success are uncertain. METHODS: We conducted a target trial emulation using the endTB Observational Study among individuals who developed non-severe peripheral neuropathy while receiving linezolid 600 mg daily within 6 months of initiating an individualized MDR/RR-TB regimen. We examined three linezolid management strategies: immediate change (i.e., dose reduction, tem

Ethionamide versus ethambutol-containing first-line regimens for TB meningitis.

Ethionamide has emerged as an alternative to ethambutol for the treatment of tuberculous meningitis (TB meningitis). Here, we show that ethionamide- and ethambutol-containing first-line TB regimens achieved similar bactericidal activities in brain tissues and reduction in neuroinflammation in a mouse model of TB meningitis. However, ethionamide, but not ethambutol, achieves brain concentrations above the minimum inhibitory concentration for Mycobacterium tuberculosis.

Co-expression of MARCKS and GSDMD pathway genes in tuberculous meningitis: a multi-omics analysis of blood-brain barrier disruption.

INTRODUCTION: Tuberculous meningitis (TBM), the most severe form of Mycobacterium tuberculosis infection, is characterized by high mortality and neurological sequelae, largely attributed to blood-brain barrier (BBB) disruption. While recent studies identified GSDMD-mediated endothelial pyroptosis as a key mechanism of inflammatory BBB damage, the full molecular landscape in TBM remains unclear. METHODS: This study employed an integrated multi-omics approach, combining bulk and single-cell RNA sequencing of clinical and murine datasets with experimental validation, to identify central mediator

MRI markers of glymphatic dysfunction in tuberculous meningitis: associations with CSF proteins and cognitive impairment.

OBJECTIVES: Tuberculous meningitis (TBM), a central nervous system infection, is frequently complicated by mild cognitive impairment (MCI). Although imaging markers of glymphatic function have been shown to correlate with cognitive decline, its role in TBM remains unrevealed. This study aims to examine the associations among glymphatic function, cognitive performance, and cerebrospinal fluid proteins in patients with TBM using functional magnetic resonance imaging (fMRI). METHODS: This study enrolled 123 participants (61 healthy controls (HCs), 30 TBM-nonMCI, 32 TBM-MCI) who underwent MRI sca

Disseminated Tuberculosis With Cavitary Lung Disease in a Very Preterm Infant.

Tuberculosis (TB) is an important cause of poor health outcomes and mortality for pregnant women and their infants globally. Screening during pregnancy helps diagnose TB at a critical period to avert morbidity and mortality but may be overlooked. TB incidence in the United States is low but has been increasing since 2020, alongside a global resurgence. The Centers for Disease Control and Prevention (CDC) recommends TB screening for all individuals with epidemiologic risk factors, including birth, residence, or prolonged travel in medium- or high-prevalence settings; immunosuppression; and clos
