Isoniazid (INH), a first-line antibiotic for tuberculosis treatment, is known for its efficacy but also for its potential to cause neurotoxic side effects, which include peripheral neuropathy, seizures, and in rare cases, psychosis. This study aimed to evaluate the neurotoxic effects of INH on memory function and its underlying mechanisms in male Swiss mice. Eighteen male Swiss mice (n = 6 per group) were treated orally with INH at doses of 5, 10, and 20 mg/kg for 28 days. The behavioral tests for memory using Y-maze and Morris water maze were conducted afterwards. The brain tissues of the ani

Central nervous system tuberculosis (CNS-TB) comprises tuberculous meningitis (TBM), parenchymal tuberculomas, and spinal arachnoiditis. It accounts for 5% to 10% of extrapulmonary TB and is more common in immunocompromised individuals, particularly those with HIV. Despite early diagnosis and treatment, mortality remains high, exceeding 30% in TB meningitis, and, as a consequence, many survivors develop cognitive, motor, or visual deficits. Management has proven to be difficult due to the blood-brain barrier and blood-cerebrospinal fluid barrier, which limit drug penetration, especially in ear

BACKGROUND: Dropped head syndrome (DHS) is a rare, disabling chin-on-chest deformity caused by severe cervical extensor weakness. The etiologic spectrum is dominated by peripheral myopathies, motor neuron disease, and myasthenia gravis; central nervous system causes are seldom considered, particularly in older adults where coexisting cervical spondylosis appears to offer a unifying structural explanation. To our knowledge, DHS as the sole presenting manifestation of brainstem encephalitis has not been previously reported.
CASE PRESENTATION: A 60-year-old immunocompetent woman presented with f

BACKGROUND: Increasing case reports and monocentric studies suggest that co-infections may be associated with poorer outcomes in patients with pulmonary cryptococcosis (PC), including both people living with HIV (PLWH) and HIV-negative individuals. However, systematic cohort-based evidence evaluating the prognostic impact of co-infections remains limited.
METHODS: Total of 454 patients with PC (239 PLWH and 215 non-HIV) were included to compare clinical characteristics, CT imaging features, and relevant prognostic factors according to their co-infection status.
RESULTS: Among non-HIV patient

BACKGROUND: All-oral shortened regimens for multidrug/rifampicin-resistant TB (MDR/RR-TB) commonly include cycloserine, which is associated with neuropsychiatric toxicity. Predictors of cycloserine-related adverse events remain poorly defined. We assessed whether baseline anaemia or hypoalbuminemia predict neuropsychiatric toxicity.
METHODS: We conducted a prospective cohort study of three 9-month standardised MDR/RR-TB regimens in Kazakhstan. Regimens shared a core of a fluoroquinolone, bedaquiline, and linezolid; one regimen included cycloserine. Neuropsychiatric adverse events of special i

Tuberculous meningitis is a devastating manifestation of tuberculosis, disproportionately affecting children and persons with HIV. Despite decades of clinical experience, diagnosis is often delayed and outcomes remain poor. At the US-Mexico border, management is further complicated by the high prevalence of Mycobacterium bovis, a member of the Mycobacterium tuberculosis complex intrinsically resistant to pyrazinamide, one of the few first-line agents with reliable central nervous system penetration. We present a case of M. bovis meningitis in a patient residing near the US-Mexico border, highl

Patients with acute leukemia are immunocompromised and highly susceptible to infections. Central nervous system (CNS) tuberculoma is a rare but serious complication in this population, particularly among those undergoing treatment for hematological malignancies. Early diagnosis is often challenging due to non-specific symptoms. We report the case of a 28-year-old female recently diagnosed with CALLA-positive B-cell acute lymphoblastic leukemia, who presented with a two-month history of low-grade fever. Induction chemotherapy was initiated; however, during the third week, she developed new-onse

INTRODUCTION: Mycobacterium tuberculosis infects nearly a quarter of the world population, while SARS-CoV-2 has infected hundreds of millions in recent years. The cellular and genetic impact of this coinfection remains unclear.
METHODS: Using a murine model, we studied SARS-CoV-2 superinfection during acute and chronic stages of high dose tuberculosis (TB).
RESULTS: In acute TB, coinfection increased mortality associated with higher brain viral loads. During chronic TB, survival improved slightly without affecting brain infection. Interestingly, histopathology showed disorganized inflammator

BACKGROUND: Central nervous system (CNS) infections are an important cause of morbidity and mortality among people living with HIV (PLWHIV), particularly in resource-limited settings. Cryptococcosis, toxoplasmosis and cerebral malaria often present with overlapping neurological symptoms, complicating diagnosis where confirmatory tests are unavailable. This study aimed to determine the prevalence and associated signs and symptoms of parasitic and fungal infections with neurological tropism in PLWHIV hospitalized in Libreville.
METHODS: A retrospective review was conducted at the Infectious Dis

BACKGROUND: Various neurological diseases can cause rapidly progressive dementia, which is a clinical syndrome characterized by a rapid decline in cognitive function over a short period, typically less than 1 or 2 years. It can be caused by various neurological diseases, including neurodegenerative, inflammatory, vascular, metabolic, and neoplastic central nervous system diseases. Rapidly progressive dementia is particularly associated with Creutzfeldt-Jakob disease, but other conditions such as immune-mediated encephalitis, rapidly progressive subtypes of Alzheimer's disease, and various othe

Central nervous system (CNS) tuberculoma is a rare but severe form of tuberculosis (TB) that can mimic other neurological disorders and poses major diagnostic challenges, especially with drug resistance. We report a case of a woman in her 60s who developed CNS tuberculoma four years after completing anti-tuberculosis therapy (ATT) for pulmonary TB. She presented with progressive left-sided weakness, facial asymmetry, and dysphagia. MRI revealed a ring-enhancing lesion in the right caudate nucleus with a lipid-lactate peak on spectroscopy, consistent with tuberculoma. Drug susceptibility testin

Extrapulmonary tuberculosis (EPTB) is an infectious disease characterized by the invasion of Mycobacterium tuberculosis beyond the lungs. Diagnosis is frequently delayed due to nonspecific clinical presentations that vary by organ system, making diagnostic imaging essential for disease detection, characterization, and treatment monitoring. The objective of this review is to examine and summarize imaging-based approaches for the diagnostic evaluation of EPTB across multiple body systems, including the central nervous system, spine, cardiovascular system, lymphatic system, abdominal and hepatic
