Transcriptomic signatures predict severe linezolid-induced peripheral neuropathy and reveal pre-treatment immune and antioxidant dysregulation.

Linezolid is an essential drug for treating multidrug-resistant and extensively drug-resistant tuberculosis, yet its long-term use is frequently limited by peripheral neuropathy, which in severe cases may be irreversible. To investigate biological factors underlying susceptibility, we analyzed transcriptomic profiles of peripheral blood mononuclear cells from 51 MDR/XDR-TB patients before and after six months of linezolid treatment, stratifying individuals into asymptomatic, mild, and severe neuropathy groups. Patients who later developed severe neuropathy already exhibited distinct transcript

Sepsis-Like Presentation of Tuberculous Meningitis With Rapid Neurologic Decline and Spinal Subdural Abscesses: A Case Report.

Tuberculous meningitis (TBM) is the most severe form of central nervous system tuberculosis and carries high morbidity and mortality. We report a 35-year-old man from Ecuador who presented with sepsis-like systemic symptoms and rapidly progressive neurological deficits. Initial imaging revealed cavitary pulmonary lesions and hilar lymphadenopathy, suggestive of pulmonary tuberculosis. Neurological deterioration prompted magnetic resonance imaging (MRI) of the brain and cervical spine, demonstrating communicating hydrocephalus, basilar leptomeningeal enhancement, diffuse cervical cord edema, an

Infection, vaccination and risk of dementia: a proposed immunological model.

With ageing populations, the prevalence of different types of dementias is increasing. The pathology of Alzheimer's disease (AD), the most common form of dementia, has been linked to the presence of plaques and neurofibrillary tangles in the central nervous system of patients. There are growing indications that risk of developing dementia correlates with several infectious agents, including human herpes viruses, flaviviruses and SARS-CoV-2. This has led to a proposition that AD and other dementias could be considered as having an infectious disease etiology. Whilst the mechanisms behind this r

I saw a different "bunch of grapes" sign in two patients with neurotuberculosis.

Neurotuberculosis is one of the most severe forms of tuberculosis, often posing diagnostic challenges due to its variable imaging presentation. We report two cases demonstrating the characteristic "bunch of grapes" sign on brain magnetic resonance imaging (MRI), representing coalescent ring-enhancing tuberculomas. The first patient was a 44-year-old woman presenting with fever and aphasia, while the second was a 28-year-old HIV-positive woman on irregular antiretroviral therapy with a history of impaired gait, dizziness and nausea. MRI revealed multiple T2-hypointense nodular lesions with ring

Common Adverse Reactions and Management Strategies of First-Line Anti-Tuberculosis Drugs.

This review synthesizes evidence from recent clinical and mechanistic studies published between 2015 and 2024 to provide updated insights into the prevention and management of adverse drug reactions (ADRs) associated with first-line anti-tuberculosis drugs (ATDs)-namely isoniazid (INH), rifampicin (RIF), pyrazinamide (PZA), and ethambutol (EMB)-which are essential for tuberculosis (TB) treatment but frequently cause significant ADRs that threaten therapeutic success. We examine four major toxicities: hepatotoxicity (primarily from INH and RIF, mediated by oxidative stress, mitochondrial dysfun

Brainstem tuberculoma mimicking brainstem stroke: Crossed syndrome in a young female.

INTRODUCTION: Tuberculosis (TB) involving the central nervous system (CNS) can present as tuberculoma and may mimic neoplasms or vascular lesions, particularly when the brainstem is involved. Early recognition is critical in endemic settings such as India. CASE PRESENTATION: A previously healthy female in late adolescence presented with a one-month history of headache followed by progressive left-sided weakness and multiple cranial nerve deficits, producing a crossed brainstem syndrome. Magnetic resonance imaging (MRI) of the brain revealed conglomerated ring-enhancing lesions in the midbrain

Host-directed nanotherapy for the treatment and imaging of tuberculous meningitis.

RATIONALE: Tuberculous meningitis (TB meningitis) is a devastating infection where the host immune response drives brain injury. Standard adjunctive corticosteroids often fail to prevent neurological sequelae or improve survival in many populations. Host-directed therapies that can cross the blood-brain barrier (BBB) and reduce neuroinflammation are urgently needed. We evaluated a hydroxyl-terminated polyamidoamine (PAMAM) dendrimer as a theranostic nanoplatform to visualize and treat microglia-mediated neuroinflammation in a young rabbit model of TB meningitis. METHODS: A novel radiolabeled

Enterovirus D68 and mycobacterial coinfection: case report.

The threat of viral epidemics to long-standing diseases, such as mycobacterial infection, is constantly evolving. Enterovirus D68 (EV-D68) is an emerging cause of respiratory infection and has raised great interest since its first outbreak in 2014. Very few studies have been done to describe the clinical aspects of the coinfection of EV-D68 and mycobacteria, so this study was conducted to help round out the understanding of this coinfection pattern. We observed three adult cases of EV-D68 and mycobacteria, who were admitted to the first affiliated hospital of Zhejiang University in August/Sept

Management of Coccidioidal Meningitis Outside the Endemic Region.

Leptomeningeal enhancement (LE) is an MRI neuroimaging characteristic associated with infection, malignancy, or cerebrovascular pathologies of the subarachnoid space. LE secondary to infection is most commonly a sign of meningeal tuberculosis. Fungal infections have a heightened presence in certain endemic regions in North America. We present the case of a 44-year-old male patient from Illinois with persistent headaches, hydrocephalus, and LE on MRI. He was empirically treated for tuberculosis, then blastomycosis, but later was found to have disseminated Coccidioides immitis infection via lymp

When Hiccups Speak Louder Than Cough: A Case Report of An Unusual Presentation of Lung Abscess.

Hiccups are commonly benign, self-limiting events often triggered by transient gastrointestinal or central nervous system irritants. However, when persistent, they may indicate underlying pathology involving the thoracic, gastrointestinal, or neurological systems. We report the case of a 65-year-old male smoker who presented with a 4-day history of persistent hiccups. He denied fever, chest pain, hemoptysis, or other systemic symptoms. Physical examination was largely unremarkable except for decreased air entry in the right middle and lower lung zones. Initial investigations, including gastros

Efficacy and safety of drug therapies for treating tuberculous meningitis: a network meta-analysis.

INTRODUCTION: The optimal treatment for tuberculous meningitis (TBM) remains inconclusive. This study intends to compare the efficacy and safety of available drug therapies for treating TBM from the perspective of mortality, neurological events, and adverse events. METHODS: PubMed (21/6/2025), Cochrane (21/6/2025), Embase (21/6/2025), and Web of Science (21/6/2025) were searched for randomized controlled trials (RCTs) of TBM. Risk of Bias 2.0 was used for the quality assessment of the included studies. R4.2.3 "gemtc" package was used for data analysis, and Stata 15.0 was utilized to assess pu

Central nervous system tuberculosis in Tunisia: clinical phenotypes from a 14-year cohort.

INTRODUCTION: Central nervous system (CNS) tuberculosis is associated with high mortality, neurological sequelae, diagnostic delay, and heterogeneous clinical spectrum. Beyond HIV infection, the role of broader immunosuppression and host inflammatory responses remains insufficiently characterized. In Tunisia, exposure to unpasteurized dairy and cattle raises concerns about possible zoonotic transmission. METHODS: All patients treated for CNS tuberculosis at La Rabta University Hospital (Tunis, Tunisia) from 2010 to 2023 were included. Patients were classified into mutually exclusive phenotype

[Tuberculosis of the Central Nervous System].

Tuberculous meningitis is a condition in which Mycobacterium tuberculosis affects the central nervous system. Although it does not present particularly distinctive symptoms, it often diverges from the classic signs seen in other forms of meningitis. Careful history-taking is essential for suspecting the diagnosis. Genetic testing has become increasingly important in diagnostic methods. Regarding treatment, not all recommendations from international guidelines can be applied domestically. Moving forward, there is a need to establish guidelines for tuberculous meningitis that leverage Japan's st

Adjunctive linezolid in patients with tuberculous meningitis for the prevention of mortality or neurologic disability: a meta-analysis of randomized controlled trials.

BACKGROUND: This systematic review aimed to evaluate the effectiveness of linezolid as an adjunct to the current tuberculous meningitis standard of care in preventing death and neurologic disability. METHODS: The MEDLINE, Embase, and CENTRAL databases were searched until 5 January 2024. We included randomized controlled trials evaluating the effectiveness of linezolid in individuals with clinically diagnosed tuberculous meningitis. For the statistical analysis, we used a Bayesian random-effects model with a weakly informative prior. RESULTS: Three trials were included in this review. Linezol

Efficacy and Safety of High-Dose Rifampicin in the Management of Tuberculosis Meningitis: A Systematic Review and Meta-Analysis.

Tuberculosis meningitis (TBM) represents the most severe type of tuberculosis (TB), and it is characterized by high mortality and neurological complications. Rifampicin is a major part of the standard treatment of TB; however, conventional doses often achieve subtherapeutic levels in the central nervous system. High-dose rifampicin has been suggested to enhance drug exposure and, possibly, improve the clinical outcomes. This study is a systematic review and meta-analysis evaluating the effectiveness and safety of high-dose rifampicin compared with standard-dose rifampicin in the treatment of T

Brainstem Encephalitis With Conflicting Autoimmune and Infectious Markers: A Case Report.

The differential diagnosis of inflammatory diseases of the central nervous system has a very wide spectrum, and the diseases may mimic one another. Although modern techniques help clinicians make a final diagnosis, there still can be challenges and pitfalls in some cases. We present an 18-year-old female patient who presented with signs of brainstem encephalitis, which was accompanied by a small hyperintense lesion in the frontal lobe. The radiological findings were nonspecific, and her cerebrospinal fluid findings were normal. However, she had positive centromere antibody in the serum and pos

Neurological and Systemic Manifestations of Melioidosis: A Three-Case Series From Eastern India.

Melioidosis, caused by Burkholderia pseudomallei, is an under-recognized infection in India that frequently mimics tuberculosis and pyogenic infections, leading to diagnostic delays. Neurological melioidosis is rare but associated with significant morbidity and mortality. We describe three culture-confirmed cases of melioidosis encountered over a three-month period at a tertiary care center in eastern India. This series is, to our knowledge, the first from eastern India to document a temporally clustered, culture-confirmed spectrum of melioidosis, ranging from systemic bacteremic to fulminant

Disseminated Tuberculosis Presenting As Scrofuloderma and Tuberculous Meningitis With Ischemic Stroke in the Absence of a Pulmonary Disease.

Extrapulmonary tuberculosis represents a significant proportion of tuberculosis cases, and scrofuloderma is an uncommon cutaneous manifestation caused by contiguous spread from underlying structures, such as lymph nodes, bone, or joints. Its association with central nervous system involvement, particularly tuberculous meningitis, is rare and carries a high risk of morbidity and mortality if not promptly recognized. We report the case of a 69-year-old man who initially presented with painful, indurated supraclavicular and anterior thoracic skin lesions that progressed to ulceration with seropur

Seeing the Unseen: A Rare Ocular Complication of Tuberculous Meningoencephalitis.

Tuberculous meningitis (TBM) is the most severe form of central nervous system (CNS) tuberculosis and carries significant morbidity, particularly when diagnosis is delayed. Cranial nerve involvement is a recognized complication, most frequently affecting the abducens nerve (CN VI); bilateral oculomotor nerve (CN III) palsy, however, is exceedingly rare and typically signifies pathology at the level of the midbrain. The oculomotor nerve originates from paired nuclei in the midbrain tegmentum at the level of the superior colliculus; any compressive or inflammatory lesion at this site can produce

Concurrent Multibacillary Lepromatous Leprosy with Autonomic Dysfunction and Cavitary Pulmonary Tuberculosis Complicated by Spontaneous Pneumothorax and Subcutaneous Emphysema.

The concurrent presentation of two distinct mycobacterial diseases - Mycobacterium tuberculosis and Mycobacterium leprae - in a single patient is exceptionally rare. Lepromatous leprosy (LL), the multibacillary form of Hansen's disease, is characterized by profound defects in cell-mediated immunity, which may predispose individuals to other intracellular infections, including tuberculosis. We present a unique case of a patient with undiagnosed lepromatous leprosy presenting with ichthyosiform scaling and autonomic dysfunction who developed a life-threatening pulmonary complication from reactiv
