Salivary Hyalinizing Clear Cell Carcinoma and Odontogenic Clear Cell Carcinoma: A Case Series and a Scoping Review Comparing Clinicopathological Presentations.

Background/Objectives: Hyalinizing clear cell carcinoma (HCCC) and clear cell odontogenic carcinoma (CCOC) are rare clear cell neoplasms with overlapping histopathological features. This study aimed to compare their clinicopathological characteristics, particularly in anatomically challenging sites such as the palate and maxilla. Methods: Three analyses were performed. First, an unpublished series of five HCCC and three CCOC cases was evaluated for diagnostic histopathological features. Second, a PRISMA-ScR-guided literature review of 58 HCCCs and 45 CCOCs restricted to tumours arising in intr

Head and neck manifestations of IgG4-related disease: current understanding.

IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory condition characterized by elevated serum IgG4 levels and dense infiltration of IgG4-positive plasma cells with fibrosis in affected organs. Since its initial recognition, clinical awareness of IgG4-RD has expanded, and head and neck involvement has become increasingly well described. Among these, IgG4-related dacryoadenitis and sialadenitis (IgG4-DS), historically referred to as Mikulicz’s disease, represents the most common manifestation. Patients typically present with persistent, painless swelling of the major salivary and lacr

Salivary gland fine-needle aspiration biopsy: quality assurance results from a tertiary cancer center.

INTRODUCTION: Fine-needle aspiration biopsy (FNAB) plays a critical role in the management of patients with salivary gland lesions. A specific diagnosis can be difficult due to the wide range of lesions with overlapping morphologic features, potentially leading to interpretation errors. We analyzed the cytologic-histologic discrepancies identified in the quality assurance program of a major cancer center in cases of salivary gland FNAB and performed a root cause analysis. MATERIALS AND METHODS: Salivary gland FNAB specimens performed during a 12-year period at a major tertiary cancer center w

Imaging of Major Salivary Gland Lesions and Disease.

Infectious and inflammatory disorders are the commonest pathologies to affect the major salivary glands however frequently overlap in clinical presentation. Imaging plays an important role in diagnosis, usually initially performed by CT or ultrasound. MRI, with its superior soft-tissue characterization compared with CT, provides a better evaluation of tumors and tumor-like conditions. Imaging features may suggest that a mass is more likely to be benign versus malignant, however, biopsy is often needed to establish a definitive histopathologic diagnosis. Imaging plays a key role in the staging

Results of histopathological revisions of major salivary gland neoplasms in routine clinical practice.

AIMS: Salivary gland neoplasms are rare and are characterised by overlapping histopathological aspects. Therefore, the assessment of the correct histopathological diagnosis can be challenging. This study evaluated the frequency of pathology consultations and revisions for salivary gland neoplasms during routine clinical practice in the Netherlands. Furthermore, the concordance and discordance rates of these revisions are presented. METHODS: The Dutch Pathology Registry (PALGA) was searched for patients that underwent a resection of a major salivary gland neoplasm between 2006 and 2016. Freque

Clear Cell Neoplasms of Salivary Glands: A Diagnostic Challenge.

This review focuses on the heterogenous group of clear cell neoplasms of salivary glands and attempts to identify major differential diagnostic features. Within the head and neck region, clear cells are found most commonly in salivary gland tumors, but may also be seen in tumors of squamous or odontogenic epithelial origin, primary or metastatic carcinomas, benign or malignant melanocytic lesions, or benign or malignant mesenchymal tumors. Clear cells occur fairly commonly among a wide variety of salivary gland neoplasms, but mostly they constitute only a minor component of the tumor cell popu

Pulmonary sialadenoma papilliferum and its mimics: what you need to know.

Pumonary salivary gland-type tumours (SGT) represent a small but distinct group of primary lung neoplasms. These types of tumours originate from the submucosal bronchial glands of the tracheobronchial tree. Pulmonary SGTs differ greatly in the incidence of individual tumours from salivary gland tumours of the head and neck. Additionally, the vast majority of pulmonary SGT are malignant. Recently, pathological diagnosis has significantly improved with the application of molecular diagnostic technologies. However, the current knowledge of benign SGTs is limited; moreover, tumour diversity and ov

A 10-Year Review of Intraoral Salivary Gland Tumor Diagnoses: Diagnostic Challenges and Inter-Observer Agreement.

BACKGROUND: Salivary gland tumors (SGT) are a diverse group of neoplasms arising from the major and minor glands. The oral cavity is the most common site for minor SGT (IMSGT), and these lesions frequently pose a challenge to the pathologist due to overlapping histopathological features and limited material for analysis. Our objective was to determine specific clinical and histopathological features associated with challenges in IMSGT diagnoses and pathologists' agreement. METHODS: We conducted a retrospective analysis of 248 IMSGT received between 2010 and 2019. We evaluated the diagnostic c

A Subset of Salivary Intercalated Duct Lesions Harbors Recurrent CTNNB1 and HRAS Mutations: A Molecular Link to Basal Cell Adenoma and Epithelial-Myoepithelial Carcinoma?

BACKGROUND: Intercalated duct lesions (IDLs) are benign salivary gland proliferations that resemble normal intercalated ducts and are subdivided into hyperplastic, adenoma or hybrid types depending on circumscription. While IDLs were historically regarded as non-neoplastic, frequent association with basal cell adenoma (BCA) and epithelial-myoepithelial carcinoma (EMC) has raised the possibility that they are neoplastic precursors. METHODS: In this study, we performed β-catenin immunohistochemistry and targeted molecular analysis on IDLs to clarify their pathogenesis. RESULTS: We identified 1

Benign tumors of the superficial lobe of the parotid gland.

UNLABELLED: Tumors of the parotid gland are a rare occurrence among head and neck tumors (only 3-8%), and the vast majority are benign. Most benign tumors of the parotid gland are pleomorphic adenoma (PA), followed by Warthin's tumor (WT) and they occur in the superficial lobe. The only reasonable treatment is considered to be total tumor removal, but the appropriate surgical approach remains a matter of controversy. Over time, it went from enucleation to superficial parotidectomy (SP) and then to extracapsular dissection (ECD). SP is often considered to be the "gold standard"; however, the ri

Human immunodeficiency virus and salivary gland pathology: an update.

Salivary gland disease is a common manifestation of human immunodeficiency virus (HIV) infection, with a significant increase in prevalence over the last two decades. This review summarizes contemporary knowledge of non-neoplastic salivary gland disease in HIV infection. The aim is to update information on and bring attention to those lesions, which are almost exclusive to the salivary glands in the HIV setting. The associated conditions include xerostomia or salivary gland hypofunction; Sjögren syndrome-like illness; salivary gland enlargements, including benign lymphoepithelial cysts (cystic

Pleomorphic adenoma-like tumour of the breast.

AIMS: Pleomorphic adenoma (PA) of the breast is a rare tumour seen usually in postmenopausal women. Although PA of the salivary glands (SG) is recognized to be a benign tumour, the nature and biology of similar tumours seen in the breast remains to be defined. The aim of this study was to describe PA of the breast that was reported on core biopsy as an invasive matrix-producing metaplastic breast carcinoma (MBC). METHODS AND RESULTS: A core biopsy from a clinically malignant retroareolar mass showed mildly atypical polygonal cells with surrounding myxoid stroma. Immunohistochemistry showed ex

Ultrasound-guided fine needle aspiration of major salivary gland masses and adjacent lymph nodes.

Ultrasound has an established role in the detection of masses in the major salivary glands and potentially malignant adjacent lymph nodes. Because there is overlap in their sonographic features, tissue diagnosis plays an important role in management. This review assesses ultrasound-guided fine needle aspiration biopsy as a diagnostic tool in the characterization of these lesions. The literature, and the authors' experience, suggests that ultrasound-guided fine needle aspiration is a safe and accurate technique, with definable implications for management, when performed in conjunction with cyto

Shear wave elastography of focal salivary gland lesions: preliminary experience in a routine head and neck US clinic.

OBJECTIVES: To evaluate shear wave elastography (SWE) for focal lesions in major salivary glands. METHODS: Sixty lesions (49 parotid, 11 submandibular) undergoing routine ultrasound (grey scale and Doppler) also underwent SWE before US-guided needle aspiration for cytology. Quantitative indices of the shear elastic modulus (stiffness) were compared with cytological results. RESULTS: Fifty-five lesions were benign (21 pleomorphic adenomas, 18 Warthin's tumours; 16 others) and 5 malignant (2 mucoepidermoid carcinomas, 1 myoepithelial carcinoma, 1 B-cell lymphoma, 1 nodal metastasis). Cytological features of cystadenocarcinoma in cyst fluid of the parotid gland: Diagnostic pitfalls and literature review.

Cystadenocarcinoma is a rare malignant tumor, with an estimated incidence of 2% of malignant salivary gland tumors. Cytological diagnosis of cystadenocarcinoma is important for differential diagnosis between benign lesions and malignant tumors with cystic growth. We report a case of cystadenocarcinoma causing difficulty in cytological diagnosis. A 23-year-old man presented with an asymptomatic mass in the left parotid gland that had been present for 2 years. The mass was elastic hard, measuring 30 x 35 mm in diameter. Preoperative fine-needle aspiration cytology (FNAC) showed a small number of

US of the major salivary glands: anatomy and spatial relationships, pathologic conditions, and pitfalls.

Ultrasonography (US) is useful for differential diagnosis of diseases of the salivary glands. In acute inflammation, salivary glands are enlarged and hypoechoic with increased blood flow; they may contain multiple small, oval, hypoechoic areas. In chronic inflammation, salivary glands are normal sized or smaller, hypoechoic, and inhomogeneous. Sialolithiasis appears as markedly hyperechoic lines or points with distal acoustic shadowing. Sialosis appears as enlarged hyperechoic glands without focal lesions or increased blood flow. The US features of advanced Sjögren syndrome include inhomogeneo

Gene expression screening of salivary gland neoplasms: molecular markers of potential histogenetic and clinical significance.

Salivary gland neoplasms comprise phenotypically and biologically diverse lesions of uncertain histogenesis. The molecular events associated with their development and clinicopathological heterogeneity remain unknown. To reveal these events, we performed microarray expression analysis using a nylon-filter membrane platform on 18 primary lesions representing the most common benign and malignant types. Our study identified a small set of genes that are differentially altered between normal salivary gland tissues and benign and malignant tumors. Of the 5000 genes arrayed, 136 genes were different

Review of fine-needle aspiration cytology of salivary gland neoplasms, with emphasis on differential diagnosis.

The widespread use of fine-needle aspiration (FNA) biopsy of salivary gland lesions in many centers is testimony to its usefulness and acceptance as a diagnostic technique. Many pertinent questions concerning a mass arising in the salivary gland can be answered by evaluation of FNA cytologic material, and these include whether the mass is truly of salivary gland origin, whether the lesion is inflammatory or neoplastic, and if neoplastic, whether benign or malignant. On diagnosis of a neoplastic salivary gland lesion, the next important issue is to correctly classify the tumor, particularly if

MRI of the parapharyngeal space.

Magnetic resonance imaging has replaced contrast-enhanced CT as the primary imaging modality for parapharyngeal space masses. It has several advantages over CT. MRI's superior contrast resolution enables the imager to better define normal anatomic structures and establish more precise tumor margins. Magnetic resonance's direct multiplanar imaging capability allows the diagnostician to offer his clinical colleagues a more accurate assessment of the total extent (in three dimensions) of disease and a better map of the spatial relationship of tumors to crucial vessels. This important information
