Dexmedetomidine with lidocaine topical anesthesia reduces emergence agitation and postoperative pain after pediatric tonsillectomy.

Tonsillectomy is among the most frequently performed surgical procedures in pediatric patients. The incidence of postoperative pain in children after tonsillectomy is high. Patient age and postoperative pain are significant factors that increase the incidence of emergence agitation in children. The study aims to evaluate the impact of spraying dexmedetomidine and lidocaine on the bilateral tonsillar fossa before tonsillectomy on postoperative pain and agitation in children. A total of 140 children aged 4-12 years, who underwent elective tonsillectomy were randomly assigned to four groups: Grou

Localized Tonsillar Amyloidosis Associated With a Plasma Cell Neoplasm Presenting as an Oropharyngeal Mass: A Diagnostic Challenge.

Amyloidosis is a rare and heterogeneous group of disorders characterized by extracellular deposition of misfolded proteins, which may present as either localized or systemic disease. In the head and neck region, the presentation of amyloidosis is uncommon and may clinically mimic neoplastic processes. We report the case of a 58-year-old man with a five-year history of progressive oropharyngeal symptoms, initially perceived as a foreign body sensation and intermittent odynophagia, later evolving to dysphagia to solids. Physical examination revealed marked asymmetry of the palatine tonsils due t

An Uncommon Extensive Bilateral Continuous Ossification of the Stylohyoid Chain Extending to the Hyoid Bone in Eagle's Syndrome: A Case Report.

Eagle's syndrome is a rare cause of cervicofacial pain and dysphagia arising from elongation of the styloid process or ossification of the stylohyoid ligament complex. While styloid elongation >3 cm is relatively common, extensive continuous ossification of the stylohyoid chain with continuity to the hyoid bone remains uncommon. We report a 42-year-old man with a six-month history of persistent throat pain and globus sensation unresponsive to conservative measures. Clinical examination showed bilateral tonsillar fossa tenderness with palpable bony projections, more pronounced on the left. Cone

Effect of tonsillar fossa suturing in adolescents and adults: a systematic review and meta-analysis.

PURPOSE: Tonsillectomy is one of the most frequently performed surgical procedures in otolaryngology. Postoperative hemorrhage (PTH) and pain remain major concerns, particularly in adolescents and adults, in whom complications are more severe. The role of tonsillar fossa suturing after tonsillectomy in reducing these complications is still controversial. METHODS: We conducted a comprehensive search of PubMed, Embase, and the Cochrane Library for studies comparing tonsillectomy with suturing (TS) versus without suturing (TNS) in patients older than 10 years. RESULTS: Seven studies involving 3

Glossopharyngeal nerve block versus tonsillar fossa ropivacaine infiltration within multimodal analgesia for pediatric post-tonsillectomy pain: a randomized controlled clinical trial.

PURPOSE: Pediatric tonsillectomy is associated with substantial postoperative pain. We compared glossopharyngeal nerve block (GNB) with peritonsillar ropivacaine infiltration (RI) within multimodal analgesia. METHODS: In this double-blind randomized trial, 90 children (4-10 years) undergoing day-case tonsillectomy were randomly allocated in a 1:1:1 ratio (30 per group) to receive bilateral GNB with 0.2% ropivacaine, RI with 0.2% ropivacaine, or sham control. The primary outcome was post-anesthesia care unit (PACU) pain measured by the Face, Legs, Activity, Cry, Consolability (FLACC) scale; se

Management of aerosol generating particles with a novel self-retaining intraoral suction device.

INTRODUCTION: Surgical aerosols generated during adenotonsillectomy (T&A) pose risks to operating room personnel. Although aerosol mitigation is legally required in 20 states, current strategies may poorly integrate into procedure workflow, impair visualization, prolong procedure duration, or be ineffective. Here, we evaluated the effectiveness of a novel self-retaining intraoral suction device (Smovac) compared with existing mitigation strategies. METHODS: T&A simulations utilizing chicken tissue and electrocautery were performed in an intubation manikin. Mitigation strategies included Smova

Unveiling a Rare Oropharyngeal Mass: A Case of Lymphangiomatous Polyp.

Lymphangiomatous polyps are rare benign lesions typically arising from nasal and laryngeal sites; however, oropharyngeal involvement is uncommon. We report the case of a 48-year-old male who presented with a stroke-like episode and was incidentally found to have a 3.0 × 1.4 × 2.4 cm pedunculated mass in the oropharynx. Flexible laryngoscopy revealed a smooth, pink, fleshy lesion near the anterior tonsillar fossa. Surgical excision was performed, and histopathology confirmed a lymphangiomatous polyp, supported by immunohistochemical stains (CD34 and ERG highlighting vascular structures). Althou

A large unilateral tonsillolith.

Tonsilloliths or tonsil stones are singular or multiple calculi found in tonsillar crypts that are located unilaterally or bilaterally. A review report determined the exact location of the tonsillolith, showing that in 69.7% of cases they are located in the tonsillar tissue, 21.2% in the tonsillar fossa, and 9% in the soft palate. Our research aims to document a case of unilateral tonsillolith that we removed under general anesthesia. His orthopantomography revealed a large radiopacity with respect to the anterior border of the left ramus and the angle area, measuring about 23.1 mm × 21.5 mm ×

Tonsillar Actinomycosis Simulating an Oropharyngeal Neoplasm in a 67-Year-Old Man With Diabetes Mellitus, Hypertension, and Ischemic Heart Disease: A Case Report From King Faisal General Hospital, Hofuf, Saudi Arabia.

Actinomyces species constitute part of the normal microbial flora of the oral cavity. When mucosal integrity is disrupted, these organisms can produce invasive cervicofacial infections. Tonsillar actinomycosis can closely mimic an aggressive oropharyngeal malignancy in elderly patients with significant comorbidities. A 67-year-old man with type 2 diabetes mellitus, hypertension, and ischemic heart disease presented with a chronic progressively enlarging right tonsillar mass accompanied by throat discomfort. Clinical assessment and computerized tomography imaging scan revealed a 42 mm × 24 mm ×

Cervicobrachialgia in a female in her fourth decade of life: Have Eagle's syndrome and thoracic outlet syndrome been considered?

Cervicobrachialgia is a common reason for consultation among women in the fourth and fifth decades of life. However, the diagnostic approach is often incomplete, leading to delays in both diagnosis and treatment. We report the case of a 44-year-old woman presenting with left craniocervical pain of several years´ duration, radiating to the left arm and the pectoral and dorsal regions, accompanied by paresthesia. Physical examination revealed findings compatible with Eagle's syndrome (tonsillar fossa) and thoracic outlet syndrome (TOS) (positive Adson, Wright, and Roos tests). Assessment using t

The hidden seasonality of pharyngitis and tonsillitis: a recurring early-summer wave of unclear aetiology.

Limited studies on the seasonality of pharyngitis and tonsillitis suggest subtle but unexplained fluctuations in case numbers that deviate from patterns seen in other respiratory diagnoses. Data on weekly acute respiratory infection diagnoses from 2010-2022, provided by the Polish National Healthcare Fund, included a total of 360 million visits. Daily mean temperature and relative humidity were sourced from the Copernicus Climate Data Store. Seasonal pattern was estimated using the STL model, while the impact of temperature was calculated with SARIMAX. A recurring early-summer wave of an unspe

Complete second branchial cleft fistula in a fifteen-year-old boy: A case report.

INTRODUCTION AND IMPORTANCE: The branchial or pharyngeal apparatus, crucial in embryological development, consists of clefts, arches, pouches, and membranes. Anomalies arising from this apparatus particularly involving the second branchial arch, are rare. Among these anomalies, complete second branchial cleft fistulas, with both external and internal openings, are exceptionally uncommon. This case report presents such a rarity in a fifteen-year-old boy, highlighting the clinical presentation, diagnostic approach, surgical management, and outcome.
