Synergistic effect of interleukin-6 and endoplasmic reticulum stress inducers on the high level of ABCG2 expression in plasma cells.

ABCG2 is a transporter preferentially expressed in a primitive subpopulation of cells and recently reported as a surviving factor for trophoblasts. To date, manner of ABCG2 expression in lymphoid tissues is not known. Immunohistochemically, strong ABCG2 expression was found in a small proportion of plasma cells mainly located in the interfollicular space of lymphoid tissues. The number of ABCG2-high plasma cells increased in interleukin-6- (IL-6) rich lesions, such as Castleman's disease of plasma cell type. Plasma cells are subjected to endoplasmic reticulum (ER) stress when excess proteins a

Mortality from malignant diseases-update of the Baltimore union poultry cohort.

We previously studied mortality up to 1989 in 2,639 members of a local union who had ever worked in poultry slaughtering and processing plants, because they were exposed to oncogenic viruses present in poultry. In this report, cancer mortality was updated to the year 2003 for 2,580 of the 2,639 subjects who worked exclusively in poultry plants. Mortality in poultry workers was compared with that in the US general population through the estimation of proportional mortality and standardized mortality ratios separately for each race/sex group and for the whole cohort. Compared to the US general p

[Tonsillar actinomycosis manifested as expectorated debris].

Actinomycosis is a festering bacterial infection frequently affecting the cervicofacial area, for which the germs responsible are Gram-positive bacilli of Actinomyces sp. We present a case of atypical presentation of actinomycosis, in the shape of repetitive mass in the tonsillar fossa with complex therapeutic management.

Antibiotics to reduce post-tonsillectomy morbidity.

BACKGROUND: Tonsillectomy continues to be one of the most common surgical procedures performed in children and adults. Despite improvements in surgical and anaesthetic techniques, postoperative morbidity, mainly in the form of pain, remains a significant clinical problem. Postoperative bacterial infection of the tonsillar fossa has been proposed as an important factor causing pain and associated morbidity, and some studies have found a reduction in morbid outcomes following the administration of perioperative antibiotics. OBJECTIVES: To determine whether perioperative antibiotics reduce pain

Atypical chronic head and neck pain: don't forget Eagle's syndrome.

We report a case of an adult woman with an Eagle's Syndrome (ES) treated with medical therapy. ES is characterized by an aspecific orofacial pain secondary to calcification of the stylohyoid ligament or elongated styoid process. In about 4% of general population an elongated styloid process occurs, while only about 4% of these patients are symptomatic. We report a case of a 49-year-old lady with a 1-year history of oro-pharyngeal foreign body sensation localized at the left tonsillar fossa, associated with a dull intermittent pain. A bony projection was palpable with bimanual transoral explora

[Mycotic aneurysm of the internal carotid artery].

The detection of aneurysms of the internal carotid artery due to acute or chronic neck infections seems to be a rather rarely event. However, if aneurysms or pseudoaneurysms of the neck region occur, than mostly by bleeding complication or unclear significant soft tissue swelling. The case of a patient is reported, who underwent an abscess tonsillectomy. The intraoperative situation of the right tonsillar fossa was found to show suspicious pulsations under the scarred and inflamed peritonsillar tissue layer. The postoperative performed intraarterial angiography verified the diagnosis of the an

Rare post-tonsillectomy complication in human immunodeficiency virus positive patient: ulcero-necrotic lesion of tonsillar fossa.

OBJECTIVE: Post-tonsillectomy complications are common. The majority are easily recognised because of their frequent occurrence and can be managed as required. Infrequent complications may be misdiagnosed or managed inappropriately. We present a previously unreported complication of tonsillectomy. CASE REPORT: A 45-year-old Afro-Caribbean man underwent routine tonsillectomy. Eight weeks post-operatively, he developed an ulcero-necrotic lesion of the tonsillar fossa. Subsequent investigation revealed that he was human immunodeficiency virus positive. It is highly likely that the patient's unde

A Rare Cause for Cervical Pain: Eagle's Syndrome.

Patients with pharyngodynia and neck pain symptoms can lead to an extensive differential diagnosis. Eagle's syndrome must be taken in account. Eagle defined "stylalgia" as an autonomous entity related to abnormal length of the styloid process or to mineralization of the stylohyoid ligament complex. The stylohyoid complex derives from Reichert's cartilage of the second branchial arch. The styloyd process is an elongated conical projection of the temporal bone that lies anteriorly to the mastoid process. The incidence of Eagle's syndrome varies among population. Usually asymptomatic, it occurs i

Use of drugs for nephrolithiasis.

Renal stone disease often begins by renal colic. In order to manage this event adequately, several goals should be pursued: first, attenuate pain; second, favour progression and spontaneous expulsion of stones; third, prevent from obstructive and infectious complications. All of the aforementioned points pertain to medical management of this disease. Concerning prevention, it is widely agreed that pathogenesis of kidney stones is a consequence of abnormalities in urine environment, leading to a disequilibrium between promoters and inhibitors of crystallization. Therefore, the rationale for the

Extramedullary plasmacytoma of the gingiva.

Extramedullary plasmacytoma is defined as neoplastic proliferation of plasma cells in soft tissue. It accounts for up to 3% of all plasma cell tumors. Approximately, 90% of extramedullary plasmacytomas are found in the head and neck region commonly affecting the nasal cavity, paranasal sinuses, tonsillar fossa, and oral cavity. Radiotherapy is the common modality of treatment with or without adjuvant chemotherapy. We report a case of extramedullary plasmacytoma of the gingiva.

Solitary fibrous tumors in the head and neck: new insights and implications for diagnosis and treatment.

OBJECTIVES: The solitary fibrous tumor (SFT) is a potentially malignant spindle cell neoplasm of mesenchymal origin that was originally described as a thoracic lesion originating from pleural tissue. Recently, numerous extrapleural sites of origin have been described, frequently affecting the head and neck region. The purpose of this article is to focus on a formerly underestimated neoplasm and to highlight its appearance in the head and neck region. METHODS: The authors present 2 illustrative cases, including what is probably the first reported case of an SFT arising in the human tonsillar f

Positron emission tomography: poor sensitivity for occult tonsillar cancer.

PURPOSE: The aim of the study was to determine the sensitivity of preoperative positron emission tomography (PET) scans in the detection of primary tumors of the tonsils. MATERIALS AND METHODS: We conducted a retrospective review of 46 patients treated at a university medical center for occult tonsillar cancer during the years 2002 to 2004. We identified patients who underwent a preoperative PET scan to locate an unknown primary tumor. Fusion computed tomography with PET (PET/CT) was used to further delineate anatomic localization to the tonsil area. A positive PET/CT scan was defined as asym

Morbidity after ipsilateral radiotherapy for oropharyngeal cancer.

INTRODUCTION: Radiotherapy for oropharyngeal cancer often causes severe side effects. If the primary tumour is localized to the tonsillar region, elective irradiation may be limited to the ipsilateral neck, sparring the contralateral normal tissues. We wanted to study the consequences of volume sparring on all prospectively registered morbidity endpoints. METHODS AND PATIENTS: Medical records, treatment charts and database information were collected for all 158 oropharynx cancer patients treated from 1998 to 2002 at Aarhus University Hospital. Of the 139 patients treated with curative intent

F-18 FDG PET/CT following dental extraction in a patient with head and neck cancer.

A 48-year-old man with squamous-cell carcinoma of the left tonsillar fossa and cervical lymph node metastases was being staged before radiation and chemotherapy. The patient had periodontal disease, and extraction of 2 teeth was performed before therapy. A staging PET/CT was performed 1 week after extraction. This case demonstrates increased FDG uptake at the extraction sites, which could be potentially mistaken for metastatic lesions, especially without the fused PET/CT images.

Robotic radiosurgery vs. brachytherapy as a boost to intensity modulated radiotherapy for tonsillar fossa and soft palate tumors: the clinical and economic impact of an emerging technology.

As a basis for making decisions regarding optimal treatment for patients with tonsillar fossa and soft palate tumors, we conducted a preliminary investigation of costs and quality of life (QoL) for two modalities [brachytherapy (BT) and robotic radiosurgery] used to boost radiation to the primary tumors following external beam radiotherapy. BT was well established in our center; a boost by robotic radiosurgery was begun more recently in patients for whom BT was not technically feasible. Robotic radiosurgery boost treatment has the advantage of being non-invasive and is able to reach tumors in

Coblation tonsillectomy: a prospective, double-blind, randomised, clinical and histopathological comparison with dissection-ligation, monopolar electrocautery and laser tonsillectomies.

INTRODUCTION: Coblation tonsillectomy is a relatively recently introduced surgical technique which attempts to bridge the gap between 'hot' and 'cold' tonsillectomy methods. AIM: To compare coblation tonsillectomy with three commonly used surgical techniques: cold dissection-ligation, monopolar electrocautery and CO2 laser. MATERIALS AND METHODS: A prospective, randomised, double-blinded clinical trial was undertaken of 60 adult patients divided into three equal study groups. Patients in each group were randomly assigned to have one tonsil removed with coblation and the second with one of th
