[Induction chemotherapy in T1 and T2 tumors of the tonsillar area].

Tumors of the tonsillar Fossa classed T1T2 are generally treated by irradiation with high rates of local control. Patient survival is related to the risk of developing distant metastatic disease and second head and neck cancers. The use of induction chemotherapy for squamous cell carcinoma of the head and neck gives a high rate of clinical tumor response and for this reason, it is increasingly used in standard treatment regimens. However, the long term results in terms of improved local control and improved survival (from a decreased rate of metastases) has not been clearly demonstrated. In ad

Postoperative radiation therapy for squamous cell carcinomas of the oral cavity and oropharynx: impact of therapy on patients with positive surgical margins.

PURPOSE: The presence of a positive or close margin after resection of a squamous cancer of the head and neck is associated with a significant risk of local recurrence. To determine the efficacy of postoperative radiation therapy for patients with advanced oral cavity and oropharyngeal cancers with inadequate margins of resection, the present retrospective analysis was undertaken. METHODS AND MATERIALS: One hundred and two patients were treated with surgery and postoperative radiation therapy for advanced squamous cell carcinomas of the oral cavity and oropharynx. The anatomic subsites treate

"Compensated" split-course versus continuous radiation therapy of carcinoma of the tonsillar fossa. Final results of a prospective randomized clinical trial of the Radiation Therapy Oncology Group.

The Radiation Therapy Oncology Group conducted a prospective comparison of a compensated split course radiotherapy technique (300 cGy x 10, 3 weeks rest, 300 cGy x 10), versus continuous radiotherapy (200-220 cGy up to 6000-6600 cGy), in 137 evaluable patients. The complete response (CR) was 57% in 63 patients, treated with the split-technique vs 61% in 74 patients submitted to continuous course radiotherapy. The completion of therapy as planned was better in the split-technique, but acute and late tissue reactions were the same. Locoregional control of tumor at 5 years was 25% for split and 2

A prospective study of factors which may predispose to post-operative tonsillar fossa haemorrhage.

Adeno-tonsillectomy and tonsillectomy are frequently performed and haemorrhage remains an important post-operative complication. A prospective study of 1090 patients undergoing surgery over a 12-month period was undertaken investigating whether the following factors influenced post-operative haemorrhage: age and sex of patients, method of tonsillectomy, haemostasis and premedication, month of operation and experience of surgeon. A significantly higher incidence of post-operative haemorrhage occurred in the group aged 16-25 years and significantly more males suffered post-operative haemorrhage.

A randomized comparison of three post-tonsillectomy diets.

Traditionally, soft cold foods have been recommended after tonsillectomy to aid comfort and haemostasis but, more recently, rougher foods have been advocated to promote physiologically normal deglutition. This trial was designed to discover whether post-tonsillectomy dietary advice has any influence on recovery. 150 patients due to undergo tonsillectomy were prospectively randomized to 1 of 3 diets: mainly rough food, mainly soft food, and no advice except to eat regularly. Food consumption, analgesia intake and pain levels were recorded daily by each patient. Tonsillar fossa slough and second

Limitations of radiotherapy in the definitive treatment of squamous carcinoma of the tonsillar fossa.

Between 1970 and 1990, 104 patients with squamous cell carcinoma (SCC) of the tonsil were treated. The median age was 58 years and 80% of patients were males. Distribution among clinical stages was: stage I, 19 patients; stage II, 12 patients; stage III, 23 patients; and stage IV, 48 patients. More than 70% of patients had initial radiotherapy as definitive treatment irrespective of stage, reflecting the treatment philosophy over much of this period. The overall survival rate was 26% at 5 years, with survival being significantly affected by T stage, clinical stage and age. Clinical node status

Complications of treatment of recurrent laryngeal papillomatosis with the carbon dioxide laser in children.

A retrospective study of the complications of treatment with the carbon dioxide (CO2) laser of 17 patients diagnosed to be suffering from recurrent laryngeal papillomatosis is presented. No immediate complications occurred except one case of laryngospasm and failure to intubate during anaesthesia leading to hypoxic encephalopathy. Three patients were completely free from disease and complications. Another patient was free from laryngeal lesions but developed a papilloma in the right tonsillar pillar. Five other patients showed one or more multiple sites of involvement in addition to the larynx

Malignant cyst of the lateral aspect of the neck: branchial cleft carcinoma or metastasis?

A 58-year-old man had a left jugulodigastric mass, which was found to be cystic by computed tomography, and no evidence of other lesions. Grossly and histologically, the surgical specimen consisted of a thin-walled, fluid-filled cyst lined by squamous epithelium that varied in appearance from benign to invasive squamous cell carcinoma. The findings supported a differential diagnosis of branchial cleft carcinoma (BCCA) versus cystic growth of a lymph node metastasis from an occult malignancy. On this basis, guided biopsies of the upper aerodigestive tract were performed, with strong suspicion o

[Parapharyngeal space lesions: with a reference to biopsy].

We received seven cases of parapharyngeal space lesions and discussed on necessity of biopsy. Tumors originating in the parapharyngeal space were represented by parotid pleomorphic adenomas which showed bulging of soft palate and tonsillar fossa, and benign neurogenic tumors which showed bulging of the lateral wall of the meso-pharynx. In these cases, preoperative biopsy was not necessary unless the lesion was suspected, of malignancy by CT, MRI and so on. In the case of bulging of soft palate and tonsillar fossa, if CT or MRI finding shows that the tumor is localized in the median portion of

The inferiorly based masseter muscle flap: anatomical basis for its use in head and neck reconstructive surgery.

The aim of this study was to investigate and elucidate the anatomical basis for the use of the inferiorly based masseter muscle flap in the reconstruction of oropharyngeal defects after ablative surgery. The anatomy of the masseter muscle, with particular emphasis on its blood supply, was studied using plain and coloured resin injected dissections and a series of digital subtraction angiograms and external carotid arteriograms. Both techniques were demonstrated to be reliable and effective means of anatomical investigation of blood vessels. The vascular anatomy of the masseter muscle and the a

[Elongated styloid process syndrome].

An elongated styloid process may be a source of craniofacial and cervical pain. The syndrome is characterized by a variety of symptoms including difficulty in swallowing, sore throat, glossodynia, headache and hemifacial pain. Sometimes, the pain is localized or radiates to the jaw and ear and may simulate pain of dental origin. Diagnosis is readily made by radiographic examination and palpating the tonsillar fossa. The only effective treatment is surgical shortening of the styloid process. Three patients, two women and a man, underwent surgery in our department for symptomatic elongation of t

Unusual presentations of lymphoma of the head and neck in childhood.

Lymphoma of the head and neck in children can pose a significant diagnostic problem, especially when histologic analysis indicates non-Hodgkin's lymphoma and the initial site of involvement is extranodal. This report describes 15 pediatric cases of lymphoma seen from 1981 to 1987 with an initial presentation in the head and neck. Cervical lymph nodes represented the initial site of involvement in 10 of the cases. The other five cases presented with disease in the tonsillar fossa; maxillary sinus and mandible; parotid; pharyngeal wall; trachea and thyroid gland; and ethmoid sinus, sphenoid sinu

The efficacy of cefaclor vs amoxicillin on recovery after tonsillectomy in children.

Tonsillectomy and adenoidectomy continues to be one of the most commonly performed operations in the pediatric age group. The morbidity from tonsillectomy can be severe and includes throat and ear pain, fever, lethargy, and poor oral intake. A previous study at the Children's Hospital of Philadelphia (Pa) demonstrated the efficacy of amoxicillin therapy in minimizing some of these postoperative symptoms. However, some children continue to have a prolonged recovery even while receiving this antibiotic regimen. Because of these children and the high incidence of Staphylococcus found in tonsillar

Elongated styloid process: an overview.

Stylalgia is a condition that causes a dull, nagging pain in the oropharynx, often referred to the ear and the mastoid region. The clinical diagnosis is certain if the elongated styloid process is palpated through the tonsillar area. Radiological investigations, such as orthopantomograms, give information about the medial angulation. The treatment of choice is surgical shortening of the process. Trans-tonsillar fossa excision is a simple technique and does not involve greater morbidity than a routine tonsillectomy.

Localization of the carotid artery within the tonsillar fossa by Doppler flow mapping.

Injury to the internal carotid artery during procedures performed in the tonsillar fossa can be catastrophic. Tonsillectomy or uvulopalatopharyngoplasty was performed on 32 patients at the Hospital of the University of Pennsylvania or the Children's Hospital of Philadelphia. Using Doppler ultrasonography, the course of the internal carotid artery in the postsurgical tonsillar fossa was mapped. The results indicate that, in the majority of patients, the internal carotid artery is located between 20% and 60% of the total width across the tonsillar fossa, as measured from the posterior pillar. The extended maxillotomy and subtotal maxillectomy for excision of skull base tumors.

An approach to tumors of the middle compartment of the skull base is described with three case reports. It is accomplished by extending the subtotal maxillectomy or maxillotomy to include removal of a part of the malar bone, coronoid process of the mandible, nasal turbinates, ethmoid and sphenoid sinuses, posterior nasal septum, and pterygoid plates. Extension of the incision through the anterior tonsillar pillar and lateral pharyngeal wall into the retropharyngeal space will assist to expose the craniocervical region from the sphenoid roof to the fifth cervical vertebra and the skull base bet
