Aneurysms arising from the extracranial carotid arteries are uncommon lesions. Clinically, they may present as lateral masses in the superior cervical area, as medial masses in the tonsillar fossa or parapharyngeal region, or as a mass palpable in both areas. Tenderness or pain may or may not be present. Pulsation is usually detectable. Embolic neurological episodes are a common event leading to discovery of these lesions. Etiologies include trauma, atherosclerosis, fibromuscular dysplasia, infection, and congenital predisposing factors. We report the CT appearance of this condition in three p

The response of two human tumour xenografts to single dose and fractionated X-rays has been tested using regrowth delay as the assay. The tumours were line transplanted cells from a moderately well-differentiated squamous carcinoma of the tonsillar fossa (XJ) and an undifferentiated carcinoma of the floor of the mouth (XR). Comparison of the dose response curves for single doses in air, clamped, or after misonidazole administration, led to estimates of the hypoxic fraction (approximately 15%) and the sensitizer enhancement ratio (less than or equal to 1.6). When 5 daily fractions were used, th

Tonsillectomy continues to be a commonly performed operation in the pediatric age group. The postoperative period is often protracted and characterized by throat and ear pain, intermittent fever, foul odor from the oral cavity, and poor oral intake. Consequently, antibiotics are frequently prescribed in an effort to minimize these symptoms and/or avoid complications such as dehydration or secondary infection of the operative site. However, to our knowledge, no study to date has been performed to demonstrate the efficacy of antibiotic therapy in this setting. At the Children's Hospital of Phila

Between 1976 and 1982, we systematically evaluated 33 patients with a diagnosis of metastatic squamous carcinoma of the neck (excluding the supraclavicular fossa) from an occult primary tumor. In 11 patients the primary lesion was identified by physical and indirect mirror examinations, in four by examinations done under anesthesia and panendoscopies, and in an additional three by random biopsies. Primary lesions were in the nasopharynx (five patients), hypopharynx (five), tonsillar fossa (three), epiglottis (two), lateral oropharyngeal wall (one), the base of the tongue (one), and the anterio

An elongated styloid process may be a source of craniofacial and cervical pain. This condition is characterized by a dull, nagging, pharyngeal pain and a palpatory finding in the tonsillar fossa. Sometimes the pain is localized, or radiates to the jaw and ear and may simulate pain of dental origin. Radiographic demonstration of styloid elongation is readily made in most instances. The only effective treatment is surgical shortening of the styloid process. Eight patients undergoing surgery for elongated styloids are reported. Six were previously treated under an incorrect diagnosis such as oral

In attempts to minimize the cost of health care, physicians are reducing the duration of hospital stay. Traditionally, at the Hospital for Sick Children, Toronto, otherwise healthy children undergoing adenoidectomy, tonsillectomy or adenotonsillectomy have been admitted the morning of surgery and discharged from hospital at 7 am the next day. The nursing records of 9409 patients aged 17 years or less who were managed in this way between 1980 and 1984 were reviewed to determine the occurrence of complications during the observation period. A total of 202 patients (2.15%) bled during the observa

Recently microvascular decompression surgery has been performed for trigeminal neuralgia or glossopharyngeal neuralgia with remarkable success. In differential diagnosis of such neuralgia, a lot of atypical facial pain must be taken into consideration, one of which is so-called Eagle's syndrome. This syndrome is characterized by elongated styloid process or calcified stylohyoid ligament compressing the Vth and/or IXth cranial nerve. In the present paper we report three cases of Eagle's syndrome and discuss the mechanism, differential diagnosis and surgical treatment of the facial pain in this

The records of 160 patients with squamous cell carcinoma of the tonsillar fossa treated from 1968 through 1979 were reviewed. Biologic behavior, local and regional findings, and other prognostic factors were evaluated. patients were treated by either radiation alone (112 patients), radiation followed by planned neck dissection (31 patients), surgery alone (11 patients), or surgery combined with radiation (6 patients). Analysis of the data supports the recommendation that radiation be used as treatment for T1, T2, and early T3 lesions, whereas surgery alone or combined with radiation is best em

From 1963 to 1978 119 patients with a carcinoma of the tonsillar fossa received cobalt-60-therapy from 60 to 70 Gy to the primary lesion and the lymph nodes. The 5-year-survival-rate was 34%, with a combined therapy of surgery and irradiation it was 43.6%. For 44 patients with a sarcoma of the tonsillar fossa the 5-year-survival was 32%. For the patients with a carcinoma chemotherapy was only given for palliation.

Pain in the neck, radiating towards the ear, together with a foreign body sensation in the throat is a common complaint. A less common finding upon physical examination is an elongated styloid process, palpable in the tonsillar fossa and provoking the symptoms for which the patient is seeking medical advice. The diagnosis of this so-called Eagle's syndrome is described and the case history and treatment of four patients are presented.

Guillotine tonsillectomy without anesthesia is a new and safe operative procedure. From 1958 to 1973, 11,140 tonsillectomies were performed in our clinic. Being safe, simple, convenient and time-saving, this method gives fairly satisfactory results. Besides, it is less traumatic and has a lower incidence of postoperative hemorrhage. More than 93% of the patients experienced no pain or slight pain during the operation. For assessment of the therapeutic results, we made an analysis of 4,190 cases. There was no much difference in cure rate and size of lymphoid tissue in the tonsillar fossa betwee

From 1960 through 1976, 353 consecutive patients with carcinoma of the tonsillar region were primarily treated with radiation therapy. The mean age of patients was 58 years with males predominating (78%). Patients were classified according to a TNM and stage classification. Most of the tumors were large, and the specific site of origin could not be determined in 33% of the patients. The predominant identifiable sites were the soft palate and uvula, 18%, the anterior tonsillar pillar, 18%, and the tonsillar fossa, 13%. Patients were treated with radiation to doses of 6700 rad given in 48 days a

A spontaneous rupture of the common carotid artery occurred in a 56-year-old, apparently healthy, man. The patient suffered a sudden, painful swelling on the left side of the neck and bulge of the tonsillar fossa simulating a parapharyngeal abscess. Emergency surgical exploration of the neck revealed a 5-mm rent in the common carotid artery 15 mm proximal from the bifurcation. Such abnormalities as aneurysm, thickness, or thinness of the arterial wall were not detected. The rent was closed by sutures during temporal ligation of the carotid artery. The duration of the temporal interruption of t

All clinicians responsible for diagnosing and treating diseases of the head and neck should be familiar with the possible clinical manifestations of a mineralized stylohyoid or stylomandibular ligament. Many patients with Eagle's syndrome have been misdiagnosed as having neuralgias, TMJ problems, psychosomatic disorders, or other vague, ill-defined diseases of the head and neck. Unfortunately , patients have been treated for these conditions with negative results. Extraction of teeth, especially third molars, has been performed unnecessarily in an attempt to alleviate the symptoms caused by a

The development, comparative anatomy, and function of the oropharynx are briefly described. The text book descriptions of the anatomical boundaries of the oropharynx are commented on and an attempt is made to define these boundaries more rigidly and accurately. Although the advantages and limitations of the UICC classification of oropharyngeal cancer is accepted, an alternative classification is suggested based on the function of the site involved rather than its descriptive anatomy. This has resulted in a necessary rearrangement of the tumor sites and subsites. Subdivision into three sites is
