[Means for improving polyclinic medical care for workers in railroad transportation].

The paper is concerned with an organizational, structural and functional model of the diagnostic center (DC) in the train, developed by the authors. The DC in the train is a new form of mobile specialized polyclinical aid to the population living in the area of railway stations. The purpose of the model is to draw nearer high-qualified counselling and diagnostic services and prophylactic aid to railway workers. The DC in the train was tried and introduced into western Siberia railway practice. The results of DC in the train establishment are described as is the efficacy of the automated system

Psychological reactions of drivers to railway suicide.

Around 90 London Underground train drivers experience a person jumping or falling in front of their train each year. The majority of these incidents are suicides or attempted suicides. 76 drivers were interviewed in order to assess the range of responses to these incidents. The following psychometric instruments were used: Present State Examination (PSE9); Post-Traumatic Stress Disorder (PTSD) Interview; General Health Questionnaire (GHQ-28); Impact of Events Scale (IES); Post-Traumatic Symptom Scale; Recent Difficulties/Events scale; Perceived Stress Scale and Eysenck Personality Questionnair

Psychotherapy for train drivers after railway suicide.

In Denmark there are approx. 50 railway suicides and suicide attempts each year. Most of these take place on urban railways such as the S-train in Copenhagen. There is a 1 in 8 chance that any one driver will be involved in such an incident each year. Since 1986 the Danske Statsbaner has implemented a policy aimed at minimising the negative psychological effects of railway suicides on train drivers. This case history describes how these procedures are put into effect.

[Prevalence and etiology of occupational dermatoses in workers of railway transport in 1958-1992].

Dermatoses are among the most prevalent occupational diseases on the railway transport; in 1961-1975 their share in total morbidity was 40%. The structure of diseases was as follows (%): contact dermatosis 87.4, eczema 6.8, allergic drug-induced dermatitis 1.4, folliculitis, toxicoderma, chemical burns, radial dermatitis and other kinds of dermatosis 0.84. The main causes of diseases were the use of many chemicals in transport processes and of Diesel engines on trains.

Incidence of selected cancers in Swedish railway workers, 1961-79.

Among all Swedish men, 20 to 64 years of age and employed in 1960, railway workers were selected and compared with the population at large, concerning the incidence of leukemia, lymphoma, tumors of the brain, breast, and the pituitary gland. The study was a re-analysis of the 1961-79 incidence data previously showing no increase in risk for leukemia and brain tumors for railway workers. In the present study, follow-up was divided into two 10-year periods, and elevated relative risks (RR) were found for the first decade. For the first decade, engine drivers and conductors combined had an RR of

Research into hand-arm vibration syndrome and its prevention in Japan.

Research on vibration syndrome in Japan began in the 1930s with studies of the disorder among railway, mining and shipyard workers. In 1947, the Ministry of Labor decided vibration syndrome among operators of rock drills and riveters etc. was an occupational disease. Industrial developments in the 1950s and 1960s promoted the survey of vibration syndrome in mining, stone quarrying and forestry. The Ministry of Labor (1965) and the National Personnel Agency (1966) legally recognized vibration syndrome among chain saw operators as an occupational disease. Guidelines for prevention and early ther

The effect of major railway accidents on the psychological health of train drivers--I. Acute psychological responses to accident.

The acute psychological reactions of 101 train drivers to on-the-track accidents were studied by means of clinical interviews and questionnaires (Impact of Event Scale, GHQ-20 and a questionnaire addressing stress symptoms, pre-accident expectancies and worries). More than half of the train drivers reported moderate to high intrusive distress (mean 11.3) within hours to days after the accident but only 1/3 reported symptoms of acute psychophysiological arousal. Intrusive symptoms related to visual impressions were most frequently reported. Avoidance was less prevalent (mean 8.8). Clinical inte

The effect of major railway accidents on the psychological health of train drivers--II. A longitudinal study of the one-year outcome after the accident.

The psychological impact on 101 train drivers of accidents causing major injuries or death to persons was studied by means of clinical interviews and questionnaires (Impact of Event Scale, General Health Questionnaire and a questionnaire addressing stress symptoms plus past and pre accident expectancies of being involved in accidents). The drivers were examined within hours to a few days after the accident and later at 1 month and 1 year. One month after the accident the symptoms of distress were significantly reduced and most so among the drivers with no preaccident risk experience according

Public transport passenger accidents: an analysis of the structural and functional characteristics of passenger and vehicle.

An analysis was conducted of accidents occurring to passengers aboard transit vehicles of the San Francisco Municipal Railway in 1989. It was expected that passenger accidents were not distributed randomly across transit modes or passenger age cohorts but that strong associations would be found between type of accident and structural and functional characteristics of the vehicles and passengers. That expectation was realized, with passengers from the oldest age groups having the highest relative frequency of accidents and vehicles with three steps being involved in a disproportionately large s

Traffic volume and collisions involving transit and nontransit vehicles.

This study reports an analysis of collisions occurring between public transit vehicles operated by the San Francisco Municipal Railway System (Muni), the public transit agency for the City of San Francisco, and nontransit vehicles. The analysis, focusing on weekday collisions during 1987, demonstrated a strong association between hourly transit collisions rates and hourly traffic volume. The collision rate varied from 0.01 per 1,000 Muni vehicle-hours of operation during the interval 5 A.M. to 6 A.M., a time of very low traffic volume, to 0.93 (approximately 1 collision per 1,000 Muni vehicle-

[Risk assessment in professional drivers with coronary heart disease: experiences with the evaluation program for locomotive drivers by the Swiss Federal Railroad 1986-1991].

BACKGROUND: For operational reasons in 1985 the question arose for the Swiss Federal Railways (SBB) whether engine-drivers (ED) with coronary heart disease (CHD) could be licensed for unaccompanied driving on certain well-defined premises. Until that year, such ED were allowed to drive only when accompanied by an apprentice ED. No generally accepted directives have operated in Switzerland for licensing ED or other professional drivers with CHD. METHODS: Based on data in the recent literature and assuming an acceptable mortality risk of about 0.75% per year, admission criteria were devised for

Railway suicide: the psychological effects on drivers.

People have jumped (or fallen) in front of trains on the London Underground system in increasing numbers throughout the twentieth century. During the past decade there have been about 100 such incidents each year, of which around 90 would involve the train driver witnessing his train strike the person on the track. Most are suicides or attempts at suicide. They represent major unexpected and violent events in the lives of the train drivers and it might be expected that some of them would respond by developing a post-traumatic stress reaction of the type identified by Horowitz (1976) or other a

[Plasmodium falciparum travels by train from Ethiopia to Djibouti].

To investigate the role of the Djibouti-Ethiopian railway as a potential vehicle for inter-regional spread of malaria vectors and malaria parasites, we performed a double-sided study, both entomological and parasitological, during November 1989, at the frontier post of Guelile where the trains from Ethiopia enter the Republic of Djibouti. No malaria-transmitting mosquitoes were collected either from the daily passenger train or from the weekly vegetables train. One hundred and five passengers entering Djibouti by train from Ethiopia had a thick film examined for malaria parasites. Five smears

Pure-tone hearing thresholds and anamnestic data pertaining to nosocusis and exposure to gun noise were analyzed for 9778 male railroad train-crew workers. A major portion of losses in hearing sensitivity due to railroad noise are obscured in comparisons of hearing levels of trainmen with the hearing levels of the unscreened samples of United States males given in Annex B, ISO 1999 [ISO 1999 (1990), "Acoustics--Determination of occupational noise exposure and estimation of noise-induced hearing impairment" (International Organization for Standardization, Geneva)]. [Fatal accidents at train intersections in Denmark].

Material was collected concerning fatal level-crossing accidents in Denmark during the period 1.1.1980 to 31.12.1987. The epidemiological and traumatic circumstances are illustrated. A total of 51 fatal level-crossing accidents occurred which corresponds to 1% of all the annual traffic deaths in Denmark. The majority of the accidents occurred in good weather conditions and between 12 noon and 3.59 pm. Single-person accidents constituted 75%. The majority of the persons killed belonged to the local population and were familiar with the local conditions. The majority of the accidents were the lo

Safety at work for railway staff in Britain.

The recent series of railway accidents raised questions about the safety of British Railways. We have studied trends in accidents to railway staff over a 33-year period, 1954-86, which time has covered the pre- and post-modernization of the railways, in conjunction with large reductions in both staffing and traffic volume. The Railway Inspectorate, which has the duty of investigating and reporting on accidents occurring on railway premises, defines three kinds of accident: train, movement and non-movement. The numbers of railway staff killed and injured in each kind of accident have been studi

Occupational performance of a paced secondary task under conditions of sensory deprivation. I. Heart rate changes in train drivers as a result of monotony.

Twelve experienced train drivers were asked to operate the train function safety circuit (SIFA)--a paced secondary motor task which is expected to guarantee the driver's fitness for service on engines of the German Federal Railway--under laboratory conditions of extreme monotony. In spite of massive decreases in vigilance as shown by theta-activity in the EEG, all subjects were able to operate the device without major errors. A prerequisite for adequate performance is an EEG-defined arousal reaction, which is synchronized with the SIFA cycles. For 7 subjects the time-related cross-correlation

Occupational performance of a paced secondary task under conditions of sensory deprivation. II. The influence of professional training.

Twelve truck drivers operated the train function safety circuit (SIFA), a paced secondary task used as a job monitor on German railways engines, under laboratory conditions of extreme monotony, in a comparison with 12 train drivers who were well acquainted with SIFA. Alertness was determined by means of EEG evaluations. Heart rate was monitored as the parameter for physical load, and the precoded SIFA tasks as the active response parameter. In spite of significantly more frequent and more distinct decreases in alterness, the SIFA-trained subjects (TS) performed better. Nine out of 12 TS reache

[The status of travel by severely handicapped patients by railroads].

An essential prerequisite for integrating disabled persons is their mobility. The special transport approach, both in terms of handling capacity and cost involved, is increasingly turning out to be unable to achieve future-oriented solutions in this context. Disabled people wish to use the transport services available to the general public. They therefore demand public transportation policies to provide for their access to the mainstream system, ideally independent of outside assistance.
