Ultrasonography of palpable masses in the pediatric head and neck.

Palpable soft-tissue masses in the pediatric head and neck are common and encompass a broad differential that includes congenital, infectious, inflammatory, and neoplastic processes. Owing to its ability to characterize lesion morphology, vascularity, and internal architecture without ionizing radiation, ultrasonography (US) remains the primary imaging modality for initial evaluation. This review focuses on pediatric head and neck masses, emphasizing practical US-based assessment and differential diagnosis. Entities discussed include common congenital lesions such as dermoid cysts, thyroglossa

Complete bilateral second branchial cleft fistula in an adolescent: a rare case report with literature review.

INTRODUCTION: Branchial cleft anomalies are congenital defects resulting from incomplete obliteration of branchial apparatus during embryogenesis. Second branchial cleft anomalies are the most common type, typically presenting as cysts or sinuses. Complete fistulae are rare, and bilateral complete second branchial cleft fistulae are exceedingly rare, with only a few cases reported in the literature to date. CASE PRESENTATION: We present a case of a 17-year-old female with bilateral mucopurulent neck discharge since birth, worsening in recent months. Clinical examination revealed two cutaneous

The diagnostic value of transfistular contrast-enhanced ultrasound in branchial cleft fistulas.

AIMS: This study aimed to assess the diagnostic value of transfistular contrast-enhanced ultrasound (CEUS) for branchial cleft fistulas (BCF), using surgical findings as the reference standard. MATERIALS AND METHODS: Fifty - six patients were initially suspected of having BCF. Among them, 35 patients who underwent two-dimensional ultrasound (2D-US), transfistular CEUS, and prior to surgery were retrospectively enrolled. The diagnostic accuracy of transfistular CEUS and 2D-US for BCF detection was compared. The agreement between CEUS and surgical findings in internal orifice localization and B

Third and fourth branchial cleft cysts - A 25-year institutional experience of imaging detection and pathologic diagnosis.

BACKGROUND AND PURPOSE: Third and fourth branchial cleft cysts (BCCs) are rare and often mimic other lower-neck cystic lesions. Tract conspicuity is frequently limited on single-time-point imaging, particularly during inflammation. We report a 25-year institutional experience evaluating Proportion Undergoing Surgery and Pathology-Confirmed BCC Rate. MATERIALS AND METHODS: We retrospectively queried radiology reports (January 2000-December 2024) to identify suspected third and fourth BCCs. Reports and images were reviewed to classify lesions; pathology was cross-referenced for operated cases.

Diagnostic challenges and perioperative management of a lateral cystic neck mass in an adult patient with hemophilia B: A case report.

Lymphatic malformations and branchial cleft anomalies are rare congenital lesions that can present similarly as cystic neck masses, often in childhood, but occasionally in adults. Differentiating between the two can be challenging, especially when clinical, radiographic, and cytologic findings overlap. We present the case of a 41-year-old male with hemophilia B and a right lateral cystic neck mass, underscoring preoperative diagnostic ambiguity and illustrating perioperative coordination required for safe surgical excision in a patient with a bleeding disorder. On examination, patient's neck h

From Sinus to Fistula: The Hidden Cost of Repeated Incision and Drainage in Paediatric Branchial Cleft Anomalies, a Case Series.

Branchial cleft anomalies commonly present as branchial cleft sinuses, which are epithelialized tracts that communicate with the skin or pharynx. While these anomalies are often asymptomatic, they can become complicated by infection, resulting in abscess formation. The standard management of an infected branchial cleft sinus involves prompt incision and drainage (I&D) to alleviate symptoms and control infection. However, repeated I&D procedures without early definitive surgical intervention to address the underlying anomaly may lead to significant preventable complications such as sinus-to-fis

Branchial cleft cyst of the cervicothoracic spine.

A branchial cleft cyst (BCC) is a neck mass, present from birth, caused by incomplete development of the branchial arches during fetal development. It commonly presents as a solitary, painless mass in the neck of a child or young adult. We present a case of a large intramedullary BCC in a patient with Klippel-Feil syndrome (KFS). A male in his 60s presented with long-lasting neck pain and fast-progressing paraparesis. MRI showed a large intramedullary cystic tumour in the cervicothoracic spine with accompanying cervical diastematomyelia and extensive cervical vertebrae fusion. The patient was

Endoscopic-Assisted Transcervical Excision of Second Branchial Cleft Anomalies in Young Children.

We describe an endoscopic-assisted transcervical technique for excision of second branchial cleft anomalies in young children. In three cases (ages 2-8), this approach allowed safe identification and ligation of the tracts while providing improved visualization and allowing smaller incisions. Endoscopic assistance may be an effective alternative to traditional wide transcervical excision.

Lymphoepithelial Cyst Presenting as a Lateral Neck Mass: A Diagnostic Pitfall in Adults.

Cystic lateral neck masses in adults are most commonly attributed to branchial cleft cysts. Lateral cervical lymphoepithelial cyst is a rare benign entity that may represent a diagnostic challenge because of its nonspecific clinical presentation. We report the case of a 32-year-old woman presenting with a slowly enlarging right lateral cervical swelling evolving for one year. Clinical examination revealed a painless, well-defined, and mobile mass. Cervical ultrasonography followed by CT demonstrated a well-circumscribed cystic lesion measuring 37 × 45 × 65 mm, initially suggestive of a branchi

A retrospective study of cervical chondrocutaneous branchial remnants.

BACKGROUND: Cervical chondrocutaneous branchial remnants (CCBRs) are rare developmental anomalies that arise during early embryogenesis. Because of their rarity, most published reports consist of small case series, and no standardized classification system has been established. The purpose of this study was to define the standardized clinical characteristics of CCBRs and to propose a comprehensive classification system based on the largest cohort reported to date. METHODS: We retrospectively reviewed 55 CCBRs in 53 patients who underwent surgical excision between 2014 and 2023. Data collected

Management of a First Branchial Cleft Cyst in a Pediatric Patient: A Case Report.

First branchial cleft cysts are rare congenital anomalies in the pediatric population. The variable clinical presentations, ranging from asymptomatic neck masses to recurrent infections or otorrhea, pose significant diagnostic challenges, especially in young children, where anatomical landmarks are less defined, and misdiagnosis is common. This case report presents a detailed analysis of a Work type II first branchial cleft cyst in a one-year-old girl, emphasizing diagnostic strategies, imaging modalities, and individualized management considerations to optimize outcomes while minimizing compl

Bilateral, non-syndromic second branchial cleft fistula in an 8-year-old child: a case report.

BACKGROUND: Second branchial cleft anomalies are the most common congenital cervical malformations, accounting for approximately 90% of branchial cleft lesions. They usually present unilaterally, while bilateral involvement is exceedingly rare and most often associated with genetic syndromes such as branchio-oto-renal (BOR) syndrome. CASE PRESENTATION: We report the case of an 8-year-old Moroccan boy with no relevant past medical history who presented with bilateral, low-lying paramedian cervical fistulas that had been recurrently infected since early childhood. [Case of concurrent second branchial cleft cyst and diffuse large B-cell lymphoma].

Second branchial cleft cyst and lymphoma are two common types of neck masses. The etiology, clinical manifestations, imaging findings, treatment options, and prognosis of these two entities differ. Clinically, the occurrence of both lesions at the same anatomical site is rare. Herein, we report a case of concurrent second branchial cleft cyst and diffuse large B-cell lymphoma in the same anatomical region. The final diagnosis was confirmed by clinical features, ima-ging findings, and ultrasound-guided biopsy. The patient received the R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristin
