Fine needle aspiration cytology including the analysis of human papilloma virus (HPV) DNA enhances the diagnostic workup of solitary cystic neck lesions in a population with a high incidence of HPV positive oropharyngeal cancer.

BACKGROUND AND PURPOSE: Distinguishing branchial cleft cysts (BrCCs) from cystic metastases of human papillomavirus (HPV) positive tonsillar or base of tongue squamous cell carcinoma and cancer of unknown primary (CUP) is challenging. Fine needle aspiration cytology (FNAC) from cystic metastasis can be nonrepresentative, while reactive squamous cells from BrCC can be atypical. It is unclear whether benign characteristics and the absence of HPV-DNA in FNAC can enhance distinguishing BrCC from a cystic metastasis; therefore, we investigated here. PATIENTS/MATERIALS AND METHODS: Morphology and H

Patient-Specific Factors to Differentiate Between Branchial Cleft Cysts and CUP Syndrome: A 10 Year Unicenter Study.

OBJECTIVES: Due to its generally-poor prognosis and varied clinical presentation, cervical cancer of unknown primary (CUP) poses particular challenges for clinical and imaging diagnosis. Differential diagnoses of CUP syndrome may also include lateral cervical cysts. When a benign neck cyst is initially suspected, squamous cell carcinoma is often diagnosed after receiving histopathology. Our study investigates the risk factors to differentiate between CUP syndrome and lateral neck cysts (LNC) to possibly improve the prognosis. METHODS: Between 2013 and 2023, 244 patients with an initial diagno

Skull Base Squamous Cell Carcinoma Presenting as Collet-Sicard Syndrome: A Rare Cause of Multicranial Nerve Dysfunction.

The lower cranial nerves (CNs) - IX, X, XI, and XII - are affected by lesions of the skull base that impact the hypoglossal canal and jugular foramen in Collet-Sicard Syndrome (CSS), a rare disorder. Loss of posterior tongue taste sensation (IX nerve), paralysis of the vocal cords and difficulty swallowing (resulting from the X nerve), weakness in the sternocleidomastoid and trapezius muscles (due to the XI nerve), and atrophy and weakness of the tongue muscles (caused by the XII nerve) are all signs of this condition. The purpose of this report is to describe a case of squamous cell carcinoma

Papillary Thyroid Microcarcinoma Revealed by Contralateral Cystic Metastasis Mimicking a Branchial Cleft Cyst.

INTRODUCTION: Papillary thyroid microcarcinomas (PTMC) can exhibit invasive behavior with lymph node metastasis despite their small size. In some cases, they may present as cystic lymph node metastases, clinically and radiologically mimicking benign lesions such as branchial cleft cysts. These misleading presentations may delay the diagnosis of an underlying thyroid malignancy. We report a rare and atypical presentation of PTMC as a large contralateral cystic lymph node metastasis initially diagnosed as a branchial cleft cyst, highlighting the need to consider metastatic thyroid carcinoma in t

Cervical Thymopharyngeal Duct Cyst Mimicking a Thyroid Nodule in an Adult: A Rare Case Report.

Thymopharyngeal duct cysts are rare congenital lesions resulting from the persistence of the embryonic thymopharyngeal tract, typically presenting in pediatric populations. Adult cases are exceedingly uncommon and often misdiagnosed due to their rarity and nonspecific clinical features. We report the case of a 34-year-old woman who presented with a slowly-enlarging, painless left cervical mass initially suggestive of a thyroid pathology. Imaging revealed a well-defined cystic lesion at the lower pole of the thyroid gland. Surgical excision via a low cervicotomy was performed, and a left lobo-i

A Serendipitous Histopathological Revelation: An Intriguing Case Report of a Cystic Schwannoma of the Lateral Neck.

Schwannomas are rare, benign tumors that arise from Schwann cells, which produce the myelin sheath around peripheral nerves. Cystic degeneration is an uncommon feature of schwannomas, occurring in approximately 4% of cases. The diagnosis of cystic schwannomas can be challenging due to their rarity and nonspecific clinical and radiological features. A 45-year-old female presented with a two-year history of a gradually enlarging neck mass, which was initially suspected to be a branchial cleft cyst. Computed tomography (CT) scans showed a well-defined cystic lesion in the left supraclavicular reg

An unanticipated case report of squamous cell carcinoma arising from a submandibular branchial cleft cyst.

INTRODUCTION AND IMPORTANCE: Cervical area tumors, especially squamous cell carcinoma (SCC), present important clinical challenges, accounting for almost 90% of all head and neck malignancies. Risk factors such as tobacco use, alcohol consumption, and human papillomavirus infection increases the incidence of these malignancies. While SCCs often arise from primary tumors in the oral cavity or other regions, they can also observe in atypical sites like branchial cleft cysts. CASE PRESENTATION: This report introduces a rare case of SCC arising from a submandibular branchial cleft cyst in a 60-ye

Adult-Onset Xanthogranulomatous Inflammation Presenting as a Cervical Branchial-Cleft Cyst.

Xanthogranulomatous inflammation (XGI) is a rare benign histiocytic reaction in the head and neck. Adult-onset XGI within a branchial-cleft cyst (BCC) is exceptionally uncommon, with only two cases previously reported. We present the oldest documented patient and review the literature. A 71-year-old man presented with a painless 3-cm right lateral-neck mass. Ultrasound and MRI suggested a benign thin-walled cyst; fine-needle aspiration revealed foamy macrophages without atypia. Intraoperatively, dense fibrosis and firm adhesion to the carotid sheath raised concern for metastatic carcinoma. Fro

Fistulograms for the management of recurrent and atypical congenital neck anomalies.

BACKGROUND: Fistulous and cystic neck lesions that cannot be categorized into traditional classification schemes at presentation are challenging to manage and often manifest as recurrently draining fistulas after primary surgery. Work up with traditional cross-sectional imaging techniques with computed tomography (CT) or magnetic resonance imaging (MRI) may not provide adequate fine details of small channels. Characterization of fistula tracts is necessary for identification and definitive management of atypical or recurrent congenital neck anomalies. METHODS: A retrospective review of the el

Surgical treatment of first branchial cleft anomalies using retrograde facial nerve dissection technique.

PURPOSE: First branchial cleft anomalies (FBCAs) are infrequent congenital malformations. In FBCAs removal surgery, due to the previous infection history and the anatomical proximity of the FBCAs tract to the facial nerve, postoperative recurrence and facial paralysis are not uncommon. This study aimed to assess the clinical feasibility and outcomes of FBCAs resection using the retrograde facial nerve dissection technique. METHODS: This retrospective study included 19 patients (mean age, 6.3 ± 4.4 years) who underwent FBCAs excision via retrograde facial nerve dissection between 2017 and 2023

Quality of life (QoL) following transoral robotic surgery (TORS) resection of parapharyngeal space tumours.

INTRODUCTION: Parapharyngeal space tumours (PSTs) can be challenging to manage in view of their distinct location and proximity to vital neurovascular structures. Numerous surgical approaches have been described including minimally invasive techniques utilising transoral robotic surgery (TORS). This study aims to evaluate the role of TORS in the management of PSTs with a special focus on quality of life (QoL) outcomes. METHODS: A systematic search was performed to evaluate the existing literature surrounding the management of PSTs with TORS. In addition, all patients at our tertiary unit were

Right-Sided Type IV Branchial Cleft Anomaly in a Seven-Year-Old Boy: A Case Report.

Type IV branchial cleft anomalies, which can present as a cyst, sinus, or fistula, are the rarest type of branchial cleft anomalies. Although they can present at any age, they typically present during childhood with a history of frequent neck abscesses. Type IV branchial cleft anomalies usually occur on the left side near the medial lower border of the sternocleidomastoid (SCM) muscle and can extend to the pyriform sinus in the larynx. They can also present with thyroid involvement. Imaging with ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), sinogram, and direct laryng

Management of a First Branchial Cleft Fistula in an Adult Patient: A Case Report and Literature Review.

First branchial cleft anomalies (FBCAs) are uncommon congenital malformations that develop due to incomplete closure of the first branchial cleft during embryonic development. They represent a small proportion of all branchial cleft anomalies. These lesions, which may present as cysts, sinuses, or fistulas, pose significant diagnostic and therapeutic challenges due to their nonspecific clinical manifestations and complex anatomical relationships, particularly with the facial nerve and parotid gland. This case report describes a 47-year-old male patient presenting with recurrent infections and
