20% of patients with multiple injuries have abdominal injuries. Patients with massive symptoms and signs indicating abdominal injury should receive high priority in the treatment of the multiple injury patient, second only to injuries to airways and thorax. The unstable critically ill patient must undergo laparotomy without further investigation. In a patient in extremis, however, left emergency thoracotomy and clamping of the descending aorta should be performed prior to the laparotomy. Extensive abdominal haemorrhage should be treated first with a tamponade to control the bleeding, after whi…

Abdominal vascular injuries remain rare in centers that primarily treat victims of blunt trauma, but when penetrating wounds of the abdomen are commonly treated, the incidence of abdominal vascular injuries is surprisingly high. With suitable management, many of these patients survive.

To improve our understanding of this frequently lethal, but potentially salvageable problem, the case records of 105 patients with 138 major intra-abdominal venous injuries seen over a 4 year period (1980-1984) were reviewed. The overall mortality rate was 54%. The most frequent abdominal venous injuries and their mortality rates were inferior vena cava, 54% (28/52); portal venous system, 51% (16/31); iliac veins, 71% (20/28); renal veins, 58% (11/19); and hepatic veins, 88% (7/8). Several important prognostic factors were identified. Of 48 patients who presented to the emergency department wi

Injuries to major abdominal arteries and veins frequently are associated with exsanguinating hemorrhage and visceral ischemia. Expeditious management is the key to survival and good outcome. Knowledge of anatomic relationships between viscera and vessels forms the basis for directed dissection, optimal exposure, and lasting repair of vessels. Although penetrating mechanism of injury remains the most common cause of these injuries, trauma surgeons must be familiar with patterns of blunt trauma-mediated injury to avoid the devastating consequences of delayed management.

In the period of July 1, 1991-December 31, 1992, 4292 wounded were treated in the Military Medical Academy (MMA) in Belgrade. Out of 410 (9.6%) wounded with abdominal war injuries, 156 (38%) received the primary treatment in MMA and the remainder in other hospitals. One fifth of the wounded with abdominal injuries suffered combined injuries of more organs and 82.7% of the cases had multiple injuries of one organ or combined injury of two or more abdominal organs. Wounding caused by explosive devices prevailed (61.5%), and the most frequently injured organs were small intestine in 40% and colon…

Treatment of major abdominal vascular injuries is a challenge. Since 1973, 93 patients have been treated at our institution for 147 abdominal vascular injuries. The majority of these injuries (67%) resulted from penetrating trauma in young men between 21 and 30 years of age. Upon admission 72% of the patients were in shock. Injured most frequently were the iliac vessels (36%) and the inferior vena cava (20%). Suture repair with preservation of function was possible in 61%; ligation of the damaged vessel was necessary in 24 cases (26%). Overall mortality was 29%. Military anti-shock (MAS) trous

Relaparotomies in the postoperative period were performed in 56 out of 529 patients (10,6%) operated upon for injuries of organs of the abdominal cavity. Twenty-two of them died. In most cases relaparotomies were caused by peritonitis, intraabdominal bleedings and abscesses.

OBJECTIVE: The surgical management and outcome of abdominal vena cava (AVC) injuries is presented. STUDY DESIGN, PATIENTS AND METHODS: A retrospective record review of patients with AVC injuries treated in the Trauma Unit at Groote Schuur Hospital between January 1999 and December 2003 was undertaken. Demographic data, mechanism of injury, surgical management, associated injuries, duration of hospital stay, complications and mortality were extracted from patient records. Patients with acute peritonitis and/or shock underwent emergency laparotomy.
RESULTS: Forty-eight patients with AVC injurie

Injuries missed at initial diagnoses or operations have the potential to cause disastrous complications in abdominal trauma patients. The aim of this retrospective study is to assess the causes and the outcomes of missed abdominal injuries. Twelve patients (2%) with missed injuries were identified among 607 abdominal trauma patients operated on from 1985 to 1993. Ten patients were male and two were female. The modes of the trauma were ten blunt injuries (83%) and two stab penetrating injuries (17%). Five cases had delayed operations because of clinical errors in the initial diagnosis. Their ca…

During the years of 1969 to 1982, 361 patients with blunt abdominal trauma were treated at the General Surgical Hospital of the University of Giessen. 46 out of these patients (12,7%) had also a fracture of the pelvis. The mortality was high with 43,5% (20 patients), however, all these patients had multiple injuries. In most cases hemorrhagic complications were responsible for death. Peritoneal lavage is an important diagnostic measure. In case of injuries to inner organs, generally several organs are concerned. Therapeutic methods should be time-sparing because of the great risks involved in
