The incidental finding of microscopic foci of acantholytic dyskeratosis, reproducing the histological pattern of Darier's disease, has been the subject of a number of papers in the last few years. Two patients developed a papulosquamous eruption in a widespread distribution which progressed with islands of sparing of uninvolved skin characteristic clinically of adult-onset pityriasis rubra pilaris (PRP). A case of a 70-year-old man with several dome-shaped tumors with acantholysis was reported. The histopathological findings of these black-brownish colored tumors on the back were compatible with seborrheic keratosis, consisting of basaloid and squamoid cells. Although three cases reported by Tagami et al. (1978) and Uchiyama et al. (1986) showed intraepidermal epithelioma-like tumor nests in the acanthotic lesions, our case was thought to correspond to another variant of seborrheic keratosis with acantholysis.

Ultraviolet irradiation produced an acantholytic phenomenon in normal skin of seven patients with pemphigus foliaceus. In six of the seven patients, this reaction was manifested by a positive Nikolsky sign and was confirmed by biopsy in four. Friction alone, infrared irradiation, occlusive dressing, and a rubefacient failed to produce a similar reaction. The thermal burn in the skin of patients having pemphigus was of subepidermal type.
