Twenty-one patients with right hemisphere damage were studied (11 men, 10 women; average age = 41.33; age range 19-65). Subjects were divide … neglect, constructional apraxia, spatial alexia, and spatial acalculia. It is proposed that spatial agraphia is related to: (1) left hemi-neglect, (2) constructional deficits, (3) general spatial defects, and (4) some motor disautomatization and tendency to perseverate. In cases of right frontal damage, motor-associated deficits (iterations of features and …

Alexia without agraphia (also called pure alexia or word blindness) was the first of the disconnection syndromes (syndromes caused by disconnection of the right from the left cerebral hemisphere through interruption of the communication pathways between them) to be described. Déjerine in 1892 reported a patient who developed this syndrome after an infarct of the left occipital lobe and splenium of the corpus callosum. We describe a patient who developed alexia without agraphia due to an embolic left occipital lobe infarct extending to the posterior commissure and splenium of the corpus callosu…

Two new cases of alexia without agraphia are presented. Pertinent clinical findings, anatomy, pathophysiology and differential diagnoses are reviewed. The importance of carefully examining the inferior portion of the left side of the splenium of the corpus callosum on CT and/or MR scans in patients who present with this clinical syndrome is stressed.

Study of neuropsychological sequelae of a focal acquired brain lesion may bring out and help delineate the features of a compensated developmental language disorder and its anatomical substrate. A left-handed man with a history of phonological developmental dyslexia and dysgraphia learned in early adulthood to read and write using a lexical system. Following a small posterior right parietal infarct when aged 56 yrs he developed a severe agraphia displaying features of phonological dysgraphia with impaired segmentation and features of lexical agraphia. Writing was severely impaired for all clas…

Eight patients with acquired agraphia were studied using the same writing tests and were compared with normal and brain-damaged controls. Four patients fulfilled the criteria for lexical agraphia and on CT scan had lesions of the posterior angular gyrus that spared the supramarginal gyrus. The other 4 fulfilled the criteria for phonological agraphia. They had lesions on CT scan that were similar to those found in previously described patients with phonological agraphia. Their lesions involved the supramarginal gyrus or insula deep to it and spared the angular gyrus. These studies support the h…

BACKGROUND: Every language has certain specific idiosyncrasies in its writing system. Cross-linguistic analyses of alexias and agraphias are fundamental to understand commonalities and differences in the brain organization of written language. Few reports of alexias and agraphias in the Spanish language are currently available.
AIMS: To analyse the clinical manifestations of alexias and agraphias in Spanish, and the effect of demographic variables.
METHODS & PROCEDURES: Spanish versions of the Western Aphasia Battery (WAB) and Boston Diagnostic Aphasia Examination (BDAE) were used for langua…

Damage to the left upper parietal lobule causes pure agraphia. However, we experienced a patient who exhibited musical agraphia following such a lesion after the agraphia improved. The patient was a 53-year-old female piano teacher. After surgery, she exhibited agraphia and musical agraphia. There was no expressive amusia, receptive amusia, aphasia, agnosia or apraxia. Fifteen months post-surgery, when her agraphia had resolved, her abilities to read, write, and copy music were evaluated. She could read and write single notes and musical signs, but her ability to write a melody was seriously i…

We report a right-handed man who developed selective Kana (phonogram) agraphia following an infarct in the non-dominant right cerebral hemisphere. His ability for comprehension, reading and writing of Kanji (ideogram) was unaffected. Kana errors consisted of substitution with another letter and the number of target words was well preserved. The lesion responsible for his Kana agraphia included the right Wernicke's area (the posterior one-third or one-half of the superior temporal gyrus) on MRI, but he did not have aphasia. Based on these findings, we conclude that the language function in some…

We report a patient who developed selective Kana (phonogram) agraphia following an infarct in the left middle frontal gyrus known as Exner's area. He had well-preserved ability for comprehension, reading, and writing Kanji (ideogram). Kana errors consisted of substitution with another letter while the number of target words was well preserved. It is suggested that a dominant middle frontal gyrus lesion can result in agraphia.

Hypothesizing that agraphia in Alzheimer's disease (AD) reflects disturbances in multiple cognitive domains, we evaluated writing samples from 33 patients meeting strict criteria for probable AD. We found agraphia to be common on a standard narrative writing task. When compared with 41 education- and age-matched normal control subjects, AD patients had significantly lower writing scores, wrote significantly fewer words, mentioned significantly fewer categories of information, and were significantly more likely to make writing errors. On stepwise regression procedures, neuropsychological measur…

BACKGROUND/OBJECTIVES: Patients with apraxic agraphia cannot correctly form the letters needed to write words but can correctly spell words orally. Apraxic agraphia (AA) is often associated with ideomotor apraxia and most commonly induced by stroke, but can be associated with degenerative diseases. In degenerative diseases, asymmetrical apraxic agraphia (AAA) has rarely been reported as one of the presenting features. The purpose of this report is to describe a case series of 3 patients who presented with an AAA, and also to elucidate some of the cognitive mechanisms of this disorder.

We describe a patient with left unilateral ideomotor apraxia without left-sided agraphia caused by a callosal lesion that was demonstrated by magnetic resonance imaging. The clinical features, together with data in the literature, suggest that the callosal fibers for writing are concentrated in the posterior corpus callosum, while those for praxis cross in the more rostral part of the posterior half of the callosum.

OBJECTIVES: Apraxic agraphia (AA) is a peripheral writing disorder generally considered to result from a causative lesion in the parietal and/or prefrontal lobe of the language dominant hemisphere (De Smet, Engelborghs, Paquier, De Deyn, & Mariën, 2011). De Smet et al. (2011), however, confirmed that AA might be associated with lesions outside the typical language areas such as the cerebellum or the thalamus. We report a 32-year-old ambidextrous patient with a left frontal lobectomy who following bilateral thalamic damage developed AA.

Studies of alexia and agraphia have played important roles in understanding how complex cognitive functions are related to brain structure and activity. Modern interests in brain-behavior relations began during the second half of the 19th century as an outgrowth of flawed correlative studies by neuroanatomist Franz Gall and subsequent clinical-pathological analyses by Jean-Baptiste Boulliaud on speech and the frontal lobes. In 1856, Louis Victor Marcé drew attention to writing disorders and postulated a cerebral faculty for writing. Following Paul Broca's epochal reports on aphemia, many Europ…

We report on a patient with pure agraphia due to perioperative stroke following left carotid endarterectomy. Her writing was illegible, slow and laborious, with impaired copying. Her oral spelling skills were normal. There was no limb apraxia. The CT brain scan revealed two hypodense areas--one anterior, the other posterior--at the level of the left centrum semi-ovale.

Two cases of alexia without agraphia, one due to a cerebrovascular accident which improved spontaneously and the other in a cerebral abscess partially recovered after surgical treatment are presented. Both were anatomically verified by computed tomography. The second was also verified at surgery. A small well localized lesion of the fusiform and lingual gyri in the dominant hemisphere can lead to alexia without agraphia. The lack of involvement of the optic radiation and calcarine fissure explain the abscence of visual field defects and hence the purest type of this syndrome as shown in our fi…

A 77-year-old composer had a left occipital lobe haemorrhagic infarct giving a severe reading disturbance with well-preserved writing and wi … -verbal images. His disorder fits the classic visual-verbal disconnection account of alexia without agraphia and the contemporary view that music involves a family of related but distinct skills probably involving many brain areas in both hemispheres, although different cortical areas make characteristic contributions to different musical behaviours.
