OBJETIVE: To report a case of a woman in whom alopecia appeared after several months of treatment with anidulafungin.
CASE SUMMARY: A 34-year-old woman with chronic femoral osteomyelitis with the presence of persistent suppuration, developed a Candida albicans infection, isolated in the fistula exudate cultures. After initial failures of single therapy with azoles, it was decided to administer fluconazole and anidulafungin 100 mg/d. WHAT IS KNOWN AND OBJECTIVE: Anidulafungin is an echinocandin antifungal used to treat invasive fungal infections caused by Candida or Aspergillus species. Anidulafungin is the newest addition to the antifungal arsenal. It possesses fungicidal activity against Candida spp., including isolates that are azole and polyene resistant. In addition, it is fungistatic against Aspergillus spp. Anidulafungin is unique in that it possesses no clinically relevant drug interactions and does not require dosage adjustment in renal or hepatic impairment. Anidulafungin was well tolerated in clinical trials and its clinical efficacy has been demonstrated in the treatment of candidemia and other forms of candidiasis.

Concentrations of anidulafungin and micafungin were determined in eight different tissues obtained during autopsy of four deceased individuals who had been treated with anidulafungin and of seven who had received micafungin. The largest amounts were recovered from liver, with anidulafungin concentrations of 11.01 to 66.50 μg/g and micafungin levels of 0.36 to 5.53 μg/g (0.65 μg/g 30 days after the last administration). The lowest anidulafungin levels were measured in skeletal muscle, and the lowest micafungin concentrations were in kidneys.

The combination of platelets and anidulafungin at 0.03 μg/ml significantly (P < 0.05) reduced the germination rate and hyphal elongation in Aspergillus fumigatus compared to those with either anidulafungin only or an untreated control. Platelets decreased the expression of the fks gene, which plays an important role in cell wall synthesis. Our results suggest that human platelets plus anidulafungin might contribute to defense against A. fumigatus.

A two-compartment pharmacokinetic (PK) population model of anidulafungin was fitted to PK data from 23 critically ill patients (age, 65 years [range, 28 to 81 years]; total body weight [TBW], 75 kg [range, 54 to 168 kg]). TBW was associated with clearance and incorporated into a final population PK model. Simulations suggested that patients with higher TBWs had less-extensive MIC coverage. Dosage escalation may be warranted in patients with high TBWs to ensure optimal drug exposures for treatment of Candida albicans and Candida glabrata infections.

Whereas echinocandins are alternatives for the treatment of invasive aspergillosis, the efficacy of anidulafungin (AFG) against Aspergillus terreus infection has not yet been explored. We have evaluated the in vitro activity, as well as the in vivo efficacy of AFG in neutropenic mice infected by A. terreus species complex. Time-kill studies showed in vitro fungistatic activity of AFG against two strains. AFG at doses of 5 and 10 mg/kg/day significantly reduced the fungal load in kidney of mice, but only the higher dose was able to prolong survival.

Anidulafungin and micafungin were quantified in cerebrospinal fluid (CSF) of critically ill adults and in cerebral cortex of deceased patients. In CSF, anidulafungin levels (<0.01 to 0.66 μg/ml) and micafungin levels (<0.01 to 0.16 μg/ml) were lower than those in plasma concentrations (0.77 to 5.07 and 1.21 to 8.70 μg/ml, respectively) drawn simultaneously. In cerebral cortex, anidulafungin and micafungin levels were 0.21 to 2.34 and 0.18 to 2.88 μg/g, respectively. Thus, MIC values of several pathogenic Candida strains exceed concentrations in CSF and in brain.
