PURPOSE: To evaluate the thumb dorsal aponeurosis anatomy. Consideration of structural differences between the fingers and thumb will provide an improved clinical understanding of the thumb dorsal aponeurosis anatomy.

A case of bilateral upper eyelid ptosis in an HIV-infected patient with severe antiretroviral-associated facial lipodystrophy is presented. The features, treatment, and outcome are discussed. Dehiscence of the levator aponeurosis may be directly associated with highly active antiretroviral therapy-induced lipodystrophy.

This article represents a comparative research between the normal palmar aponeurosis seen with the electron microscope and adulterated palmar aponeurosis in Dupuytren disease. Lower lid retractor aponeurosis disinsertion is a well-recognized etiologic factor in many involutional entropion cases, but to our knowledge it has not previously been reported as a cause of congenital entropion. Four congenitally entropic lower eyelids in three patients with no history of birth trauma were all found to have retractor disinsertion during exploratory procedures. The aponeurosis lengthening described in this study was performed on 156 patients (219 procedures) with spastic equinus deformities. There was only one wound complication and no calcaneus deformities from overlengthening. The recurrence rate requiring relengthening was high (48% of the procedures), particularly if the initial operation was performed before the age of 5 years. Seven patients with acquired ptosis were treated with a tuck in the aponeurosis of the levator palpebrae superioris muscle. Although the etiology for the ptosis was not always clear, associated conditions included progressive external ophthalmoplegia, prior ocular irradiation, prior ocular surgery and enucleation.
