Arachnoid cysts are fluid-filled duplications or splittings of the arachnoid layer with a content which is similar but not equal to the cerebrospinal fluid. Arachnoid cysts are not actual neurodegenerative disorders, rather the underlying defect of the texture of the arachnoid layer is probably congenital in nature. They can occur sporadically or can be associated with other malformations or diseases. Arachnoid cysts may be discovered in early childhood. However, they can develop de novo, grow or decrease in size. They may be diagnosed by ultrasound screening in the fetal period or be discover…

PURPOSE OF REVIEW: The present review summarizes the current theories on arachnoid cyst formation, the common presentations of cysts surrounding or eroding the temporal bone from the middle and posterior cranial fossae, the diagnostic strategies and the management considerations and options.
RECENT FINDINGS: Arachnoid cysts are most common in the middle cranial fossa and rarely present in association with the petrous apex. They are frequently found incidentally on imaging studies performed in the workup for unrelated symptoms. When they do cause symptoms, these are usually nonspecific. Thus i…

Thirteen pathologically documented intrasellar arachnoid cysts treated by transsphenoidal cyst excision and marsupialization were analyzed. Presenting symptoms included headache (7), visual loss (7), galactorrhea (4), and anterior pituitary dysfunction (2). Postoperatively, the 7 patients with visual loss and 4 with hyperprolactinemia improved. The 2 patients with anterior pituitary dysfunction did not improve and 2 additional patients required hormonal supplements. Based on this operative experience, the pathophysiology of these cysts is discussed. The indications for transsphenoidal explorat…

The infratentorial compartment represents the second most common location of arachnoid malformations. Ten arachnoid cysts of the posterior fossa, operated on between 1970 and 1983, are reviewed. These lesions, although congenital and developmental in nature, may present at any age, and males are more frequently affected. A high rate of birth-related trauma (50% in this series) is conceivably due to fetal macrocranium, and the enlarged head and psychomotor retardation prevail in infancy and childhood. In arachnoid cysts occurring during adulthood, symptoms and signs more clearly indicate a dysf…

PURPOSE OF REVIEW: Arachnoid cysts are incidentally found on neuroimaging at an increasing rate in the paediatric population. Hence, learning their clinical implications to better inform families is important. The purpose of this review is to outline the current understanding of epidemiology, disease and clinical features based on location, imaging and management of arachnoid cysts in paediatrics.
RECENT FINDINGS: The prevalence of arachnoid cysts is 2.6% in paediatrics, with greater than 90% being asymptomatic. When they do cause symptoms, it is based primarily on their location and size. Th…

AIM: The management of arachnoid cysts (AC) remains controversial. An additional problem derives from the management of hydrocephalus associated with an AC. In this work, we discuss existing procedures proposed in the current literature for their treatment.
METHODS: We reviewed selected reports on intracranial ACs placing special interest in those about the association of hydrocephalus and ACs. We also briefly surveyed data of our patients with this association.
RESULTS AND DISCUSSION: Hydrocephalus is often found in midline and posterior fossa ACs. Interhemispheric lesions may also evolve w…

Arachnoid cysts of the posterior fossa are rare lesions that are considered to be mostly congenital in origin. In this article, we retrospectively review 12 patients who underwent surgical treatment for their symptomatic posterior fossa arachnoid cysts. The most common presenting symptoms were gait disturbances and headache. The diagnosis was established on computed tomography or magnetic resonance imaging. Surgery consisted of cyst wall excision with fenestration in nine cases and shunting procedures in three cases. In all cases except one who-died, the postsurgical follow-up neuroradiologica…

Arachnoid cysts very rarely occur within the ventricular system, where no arachnoid tissue exists. We present three cases of intraventricular arachnoid cyst with special reference to its origin. The first patient was a 5-year-old boy who complained of headaches and enlargement of his head. A CT scan revealed obstructive hydrocephalus and a large cystic lesion in the right lateral ventricle. The symptoms resolved after fenestration of the cyst and cystoperitoneal shunt. The second patient was a 49-year-old woman who complained of headache and numbness in her left upper extremity. A CT scan and …

Intracranial arachnoid cysts (ACs) are benign lesions. The incidence in children is 2.6%. ACs are often diagnosed incidentally. Because of the broad use of CT and MR imaging, the frequency of AC diagnosis has increased. In addition, prenatal diagnosis of ACs is becoming more common. This places clinicians in a difficult situation with regard to the optimal treatment, since the presenting symptoms are often vague and operative management includes not negligible risks. It is generally accepted that conservative management is indicated in cases with small and asymptomatic cysts. In contrast, pati…

There may be several kinds of pathological conditions in the cystic lesion which are clinically diagnosed as benign intracranial cysts on CT scan. Light and electron microscopic studies on cyst walls were important in the differential diagnosis of benign intracranial cysts. We have studied 5 cases of intracranial arachnoid cysts and two epithelial cysts using the light and electron microscopy. Five cases of intracranial arachnoid cysts included two children and three adults (three females and two males). Three cases of them were localized in the middle cranial fossa, one case in the anterior a…

The association of intracranial arachnoid cysts and pneumosinus dilatans is considered to be relatively rare, and has previously been described in only a limited number of cases. In this study the CT and/or MRI examinations of a total of 30 cases with middle cranial fossa arachnoid cysts were retrospectively evaluated in an attempt to note any association with pneumosinus dilatans of the paranasal sinuses. In four patients with surgically treated, large, middle cranial fossa arachnoid cysts, obvious paranasal sinus changes, consistent with pneumosinus dilatans, were noted. The frequency of the…

Twenty supratentorial and 10 infratentorial arachnoid cysts are reported. The patients were from 0 to 15 years of age. The commonest presenting symptoms in children were cranial enlargement, epileptic seizures, and psychomotor retardation. Neuroradiological evaluation included CT, metrizamide CT, cisternography, and angiography. Echography was performed in 5 newborns. Therapeutic criteria according to the clinical and neuroradiological findings are reviewed. Cystoperitoneal shunting in combination with ventriculoperitoneal shunting for associated hydrocephalus is considered the treatment of ch…

We report 2 cases of arachnoid cysts related to a head trauma. The first case involved a 1-year-old girl who developed an arachnoid cyst 7 months after suffering a head trauma due to a motor vehicle accident, and the second case involved a 1-year-old boy whose arachnoid cyst was first noted 2 months subsequent to his premature birth. We present the serial imaging for both cases. Choi and Kim [Pediatr Neurosurg 1998;29:260-266] postulated that head trauma in infancy may contribute to the pathogenesis of arachnoid cysts in some cases. In presenting our cases, we support this hypothesis, and furt…

BACKGROUND: Extradural arachnoid cysts in the spine are uncommon causes of spinal cord compression in the pediatric population that are thought to arise from congenital defects in the dura mater. Most reports describe such cysts as communicating with the intrathecal subarachnoid space through a small defect in the dura. Excision of the cyst with obliteration of the communicating dural defect is the mainstay of treatment in symptomatic patients. Solitary extradural arachnoid cysts have been reported in several studies, but multiple extradural arachnoid cysts are very rarely reported in children…
