Ascariasis is a helminthic infection of global distribution with more than 1.4 billion persons infected throughout the world. The majority of infections occur in the developing countries of Asia and Latin America. Of 4 million people infected in the United States, a large percentage are immigrants from developing countries. Ascaris-related clinical disease is restricted to subjects with heavy worm load, and an estimated 1.2 to 2 million such cases, with 20,000 deaths, occur in endemic areas per year. Ascaris lumbricoides is the most common helminth to infect humans. Infection occurs when contaminated soil containing mature eggs is swallowed. Clinically, infection ranges from the asymptomatic carrier state to life-threatening pulmonary disease or intestinal obstruction. Diagnosis is most frequently established by identification of the eggs in stool specimens. Treatment with antiparasitic drugs is effective in preventing serious complications and eradicating the parasite from all members of the household.

Ascaris infestations may be found in California, particularly in patients who have migrated from endemic regions. Clinical manifestations include vague abdominal pains, unexplained fever, anemia, malaise and upper respiratory tract infections. Intestinal obstruction and infections are among the severe complications that can occur. Diagnosis is made by the observation of worms or ova in the feces, and occasionally by roentgenographic manifestations.
