Autoantibodies.

The number of autoantibodies associated with chronic liver disease continues to burgeon and characterization of these immunoreactivities will undoubtedly enhance understanding of the autoantigens that are targeted by cytodestructive immunocytes. Assays for the majority of these immunoserological species are not generally available and in most instances, assessments are restricted to individual laboratories with vested interests in characterizing a particular species. For clinical diagnosis and management of autoimmune liver disease, assays for ANA, SMA, anti-LKM1 and AMA are essential. This co…

Thyroid autoantibodies.

Thyroid-stimulating hormone (TSH) receptor antibody (TSH-R-Ab or TRAb) testing plays a pivotal role in arriving at the aetiological diagnosis in patients with thyrotoxicosis. A positive test establishes the diagnosis of Graves' disease (GD) while a negative result in conjunction with imaging studies supports other possible aetiologies. In patients with GD, TRAb levels at diagnosis and at the time of withdrawal of antithyroid drugs can identify patients who are unlikely to achieve remission and guide clinical management decisions. We provide an algorithm that incorporates TRAb in the decision-m

Myositis autoantibodies.

PURPOSE OF REVIEW: To review recent advances in our understanding of autoantibodies associated with dermatomyositis and the autoimmune necrotizing myopathies. RECENT FINDINGS: Autoantibodies preferentially associated with dermatomyositis include those recognizing Mi-2, MDA5, TIF1γ, and NXP-2. Each of these is associated with a distinct clinical phenotype. Autoantibodies found in patients with autoimmune necrotizing myopathies recognize signal recognition particle and 3-hydroxy-3-methylglutaryl-coenzime A reductase (HMG-CoA) reductase. The latter are found in patients with statin-associated au…

Autoantibodies and Cardiomyopathy: Focus on Beta-1 Adrenergic Receptor Autoantibodies.

Antibody response to self-antigens leads to autoimmune response that plays a determinant role in cardiovascular disease outcomes including dilated cardiomyopathy (DCM). Although the origins of the self-reactive endogenous autoantibodies are not well-characterized, it is believed to be triggered by tissue injury or dysregulated humoral response. Autoantibodies that recognize G protein-coupled receptors are considered consequential because they act as modulators of downstream receptor signaling displaying a wide range of unique pharmacological properties. These wide range of pharmacological prop…

Autoantibodies in myositis.

The discovery of novel autoantigen systems related to idiopathic inflammatory myopathies (collectively referred to as myositis) in adults and children has had major implications for the diagnosis and management of this group of diseases across a wide range of medical specialties. Traditionally, autoantibodies found in patients with myositis are described as being myositis-specific autoantibodies (MSAs) or myositis-associated autoantibodies (MAAs), depending on their prevalence in other, related conditions. However, certain MSAs are more closely associated with extramuscular manifestations, suc…

Autoantibodies and pain.

PURPOSE OF REVIEW: Over the last 20 years, several neurological conditions have been identified which appear to be caused directly by autoantibodies targeting receptors, ion channels and related proteins on neuronal or glial cells. Neuroimmune interactions are now accepted contributors to chronic pain conditions. Autoantibodies might be one such cause and here we highlight their potential role in pathological pain. RECENT FINDINGS: Recent studies have given more weight to the idea that autoantibodies can be directly related to pain; this is suggested by the success of immunotherapy in patient…

Myositis specific autoantibodies.

Recent studies have continued to examine the clinical associations of the group of autoantibodies that occurs predominantly in patients who have myositis (antibodies to aminoacyl-tRNA synthetases, to signal recognition particle [SRP], and to the nuclear helicase Mi-2). The anti-synthetase syndrome has been further studied, and the value of tacrolimus in treatment of the associated interstitial lung disease has been noted. The low frequency of myositis specific autoantibodies in non-myositis neuromuscular disorders has been more clearly demonstrated. The clinical associations of anti-Mi-2 and a…

[Anti-TPO autoantibodies].

Thyroid peroxidase (TPO) is a member of the thyroid autoantigens. Anti-thyroid autoantibodies, anti-TPO,-thyroglobulin and -TSH receptor autoantibodies, are important in diagnosing autoimmune thyroid diseases and for judging treatment efficacy. To diagnose chronic thyroiditis, anti-TPO autoantibody detection is the most sensitive among the three anti-thyroid autoantibodies. Eighty-five to ninety percent of patients with chronic thyroiditis have anti-TPO autoantibodies. Using recombinant human TPO protein, a kit measuring anti-TPO antibodies specifically have been developed. The developmental p…

Autoantibodies in polymyositis.

A group of autoantibodies have been identified that are found almost exclusively in patients with polymyositis and dermatomyositis (myositis-specific antibodies). Most have been associated with characteristic clinical subgroups. Five of the myositis-specific antibodies are directed at aminoacyl-tRNA synthetases and have been associated with a similar clinical syndrome marked by myositis, interstitial lung disease, arthritis, and Raynaud's phenomenon (antisynthetase syndrome). Myositis-specific antibodies can help with patient diagnosis, subgroup classification, and possible prognosis. Their ro…

[Dermatomyositis and Autoantibodies].

Recent studies have identified novel dermatomyositis-specific autoantibodies and revealed that disease-specific autoantibodies become positive at a high rate in this disease. Moreover, these autoantibodies have been demonstrated to show a strong correlation with distinct clinical manifestations and complications such as interstitial lung disease and malignancy. Thus, these autoantibodies are now recognized as useful tools to classify this varied disease into more homogeneous subsets. In this review, the clinical significance of five dermatomyositis-specific autoantibodies, anti-Mi-2, anti-MDA5…

[Myositis-specific autoantibodies].

Idiopathic inflammatory myopathies are a group of acquired skeletal muscle diseases that include polymyositis, dermatomyositis, and inclusion body myositis. Studies have shown many myositis-specific autoantibodies (MSAs) that are useful for the diagnoses as well as classification of idiopathic inflammatory myopathies, because they have been shown to correlate with distinct clinical phenotypes. Anti-Jo-1, anti-PL-7, anti-PL-12, anti-EJ, anti-KS, anti-OJ, anti-Ha, and anti-Zo antibodies target aminoacyl tRNA synthetases, and represent anti-synthetase syndrome. Anti-synthetase syndrome is charact…

TSH receptor autoantibodies.

Thyrotropin receptor autoantibodies (TSHR-Abs) of the stimulating variety are the hallmark of Graves' disease. The presence of immune defects leading to synthesis of TSHR-Abs causes hyperthyroidism and is associated with other extrathyroidal manifestations. Further characterization of these antibodies has now been made possible by the generation of monoclonal antibodies with this unique stimulating capacity as well as similar TSHR-Abs not associated with hyperthyroidism. Their present classification divides TSHR-Abs into stimulating, blocking (competing with TSH binding) and neutral (no signal…
