Methods and functions: Breath tests.

Breath tests provide a valuable non-invasive diagnostic strategy to in vivo assess a variety of enzyme activities, organ functions or transport processes. Both the hydrogen breath tests and the (13)C-breath tests using the stable isotope (13)C as tracer are non-radioactive and safe, also in children and pregnancy. Hydrogen breath tests are widely used in clinical practice to explore gastrointestinal disorders. They are applied for diagnosing carbohydrate malassimilation, small intestinal bacterial overgrowth and for measuring the orocecal transit time. (13)C-breath tests non-invasively monitor…

Breath tests and irritable bowel syndrome.

Breath tests are non-invasive tests and can detect H₂ and CH₄ gases which are produced by bacterial fermentation of unabsorbed intestinal carbohydrate and are excreted in the breath. These tests are used in the diagnosis of carbohydrate malabsorption, small intestinal bacterial overgrowth, and for measuring the orocecal transit time. Malabsorption of carbohydrates is a key trigger of irritable bowel syndrome (IBS)-type symptoms such as diarrhea and/or constipation, bloating, excess flatulence, headaches and lack of energy. Abdominal bloating is a common nonspecific symptom which can negatively…

Manipulation of Breath Alcohol Tests: Can Specific Techniques Alter Breath Alcohol Content?

OBJECTIVE: The most common form of measurement of breath alcohol content (BrAC) is through the use of a diode catheter. This study aims to test the accuracy of breath alcohol analysis through different manipulations. METHODS: BrAC was measured after individuals consumed each standardized beer until they reached a 0.1 BrAC. Then, the individuals were breath analyzed while not providing full effort, using the side of their mouths, immediately after hyperventilating, 5 and 10 min after hyperventilation, immediately after a sip of water, and 5 min after that water. RESULTS: There were 54 individ

[Application of laser spectroscopy for 13C-breath tests].

A new device Tunable Diode Laser Spectroscopy (LS) was developed for the analysis of isotope ratios of 13CO2/12CO2. Its applicability for breath tests was validated. The exhaled breath is collected in a 2 l aluminum bag, of which CO2 is separated by cryogenic system and introduced into LS. Repeat measurements (8 times) of two kinds of gases (delta 13C:-2.62 and - 1.14%) revealed 0.045% and 0.065% variation (IS.D.), from which precision of measurement was estimated as 0.2% (3S.D.). Seven healthy volunteers were given orally different dose of 13C-glycine (50 mg-200 mg in 5 subjects) and 13C-meth…

[Breath analysis tests in gastrointestinal disorders (author's transl)].

From various breath tests up to now mainly measurement of respiratory 14CO2 and hydrogen have attained clinical application. Breath tests are easily performable, without discomfort for patient or in investigator and do hardly require active support of the patient. On the other hand absorption tests, as 14C-tripalmitate- and 14C-lactose-breath test, are influenced by alterations of the metabolism in a considerable degree, and clinical significance and specificity of the 14C-cholylcycline-breath test remains questionable hitherto. Nevertheless, obtained values which are independent from metaboli…

Single time point diagnostic breath tests: a review.

The metabolism of ingested xenobiotics is clinically significant to minimize risk and optimize therapeutic benefits. A majority of the drugs approved by the FDA are metabolized by phase I enzymes. Stable isotope-labeled xenobiotics can be used to provide rapid in vivo phenotype assessment of phase I enzymes. In this paper, we describe three simple, noninvasive phenotype breath tests using [13C]-dextromethorphan and [13C]-pantoprazole for assessing polymorphic CYP2D6 and CYP2C19 enzyme activity and [13C]-uracil to assess the enzyme activity of DPD, DPHD and β-ureidopropionase for identifying py…

Methane breath tests and blood sugar tests in children with suspected carbohydrate malabsorption.

Carbohydrate malabsorption and subsequent gastrointestinal symptoms are a common clinical problem in pediatrics. Hydrogen (H2) and methane (CH4) breath tests are a cheap and non-invasive procedure for diagnosing fructose and lactose malabsorption (FM/LM) but test accuracy and reliability as well as the impact of non-hydrogen producers (NHP) is unclear. CH4 breath tests (MBT), blood sugar tests (BST) and clinical symptoms were compared with H2 breath tests (HBT) for FM/LM. 187/82 tests were performed in children (2 to 18 years) with unclear chronic/recurrent abdominal pain and suspected FM/LM. …

Reliability of symptom analysis during carbohydrate hydrogen-breath tests.

PURPOSE OF REVIEW: Relevance of symptom analysis during hydrogen breath test (HBT) for establishing a clinical diagnosis of sugar intolerance is reviewed. RECENT FINDINGS: Evaluation of symptoms developed in response to the ingestion of 50 g lactose could represent a simple screening test to select patients for lactose intolerance testing. Patients who do not develop symptoms do not need to be referred for HBT. In addition, symptoms reported by patients during a negative HBT cannot be at all times attributed to a false-negative test; instead, a 'nocebo' effect is likely to be implicated. On t

[Breath tests in intestinal diseases and functional gastrointestinal diagnosis].

Among the numerous breath tests described for gastroenterological applications, breath hydrogen (H2) tests have emerged during the past two decades as a most sensitive, reliable and feasible method for detecting carbohydrate malabsorption and maldigestion (e. g. lactose maldigestion). Hence they are regarded time honored standards of contemporary gastroenterological function tests. For the diagnosis of the small bowel bacterial overgrowth syndrome the glucose H2 breath test is a feasible tool with moderate sensitivity (approximately 65%), which, however, is not higher with alternative breath t…

Modification of baseline status to improve breath tests performance.

Breath tests used to evaluate carbohydrates malabsorption require baseline H2 and CH4 levels as low as possible. Test cancellation is recommended when exceeding certain cut-offs (H2 ≥ 20 ppm and CH4 ≥ 10 ppm). Although following preparation protocols, many patients have baseline levels above those cut-offs. We investigated if light walking can reduce baseline H2 and CH4 levels. We retrospectively analyzed baseline H2 and CH4 levels from 1552 breath tests. Baseline levels (B1), especially in H2, were lower when obtained at later hours of the day. In those with baseline levels above cut-off, re-…

Non-(13)CO2 targeted breath tests: a feasibility study.

Breath tests allow a non-invasive and fast diagnostic of different specific enzymes' phenotypic functionality. Over the last decade several 13C-breath tests were successfully tested, with the (13)C-urea breath test being approved by the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA). The use of other targets than labeled (13)CO2 in exhaled breath offers additional possibilities. High sensitivity analytical technologies, such as proton-transfer reaction time-of-flight mass spectrometry, enable the detection of different volatile targets in the low ppb (parts per b…

Carbohydrate malabsorption: quantification by methane and hydrogen breath tests.

BACKGROUND: Previous studies in small series of healthy adults have suggested that parallel measurement of hydrogen and methane resulting fr … rbohydrate malabsorption. The status of methane production should, therefore, be known to interpret breath tests semiquantitatively. The weak correlation between hydrogen and methane excretion and gas-related abdominal complaints suggests that other factors than net production of these gases may be responsible for the symptoms.
