Short-term compatibility of furosemide with crystalloid solutions.

Parenteral veterinary furosemide is a 50-mg/mL solution with a pH of 8.0-9.3. The purpose of this study was to determine whether a commonly used veterinary formulation of 50 mg/mL of furosemide solution could be diluted in vitro without precipitation. Furosemide 50 mg/mL was diluted to concentrations of 10 and 5 mg/mL with 5% dextrose in water (D5W), 0.9% saline, lactated Ringer solution (LRS), and sterile water. Acidic sterile water and basic sterile water solutions were made by the addition of hydrochloric acid and sodium hydroxide, respectively, for use as controls to assess the effect of p

Crystalloid Solutions in Hospital: A Review of Existing Literature.

Intravenous fluids (IVF) like normal saline (NS) and Ringer's lactate (RL) are often crucial in the management of hospitalized patients. Mishandling these fluids can lead to complications in about 20% of patients receiving them. In this review, we present the current evidence through the identification of observational studies and randomized trials that observed the optimal use of IVF. We found that NS may cause hyperchloremic metabolic acidosis in surgical patients, but there is no clear difference in mortality and long-term outcomes between NS and balanced crystalloids. Critically ill patien

Normal Saline Versus Balanced Crystalloid Solutions for Kidney Transplantation.

INTRODUCTION: The ideal crystalloid solution to be used during the perioperative period in patients undergoing kidney transplantation remains unclear. Normal saline (NS), the intravenous fluid commonly using during the perioperative period, contains a high chloride content, which may be associated with hyperchloremic metabolic acidosis and acute kidney injury. Balanced crystalloid (BC) solutions have a lower chloride content. The purpose of the study was to determine if a BC solution prevents the incidence of hyperchloremia and hyperkalemia during renal transplantation. METHODS: NS and BC giv

Small-volume resuscitation. Crystalloid solutions for intravascular volume replacement.

There is growing interest in the use of hypertonic crystalloid solutions for intravascular volume replacement. Such solutions rapidly restore nutrient blood flow to vital organs by a combination of increased perfusion pressure and vascular dilation. The addition of colloid to the solution appears to improve survival by limiting the tissue damage caused by ischemic injury and may avert the development of multisystem organ failure. Low cost and long shelf life make hypertonic solutions attractive for acute resuscitation after major trauma, but further investigations are warranted as to their eff…

An assessment of crystalloid solutions for donor heart preservation.

The optimal technique for donor heart protection remains controversial. One component of preservation is the transport solution. Although saline solution is most frequently used as a transport medium, other crystalloid solutions may be superior. Accordingly, human right atrial trabeculae contracting isometrically in vitro were used to assess five crystalloid solutions at two different temperatures (12 degrees and 4 degrees C): St. Thomas' Hospital solution, modified Krebs-Henseleit (K+ = 16 mEq/L), Krebs-Henseleit (K+ = 4.5 mEq/L, saline, and Euro-Collins. After a 24-hour preservation period t…

[Fluid and Volume Therapy with Crystalloid and Colloidal Infusion Solutions].

A rational infusion therapy orchestrates fluid- and volume therapy based on the individual indication and situation. The principle of fluid replacement is to substitute ongoing fluid losses such as insensible perspiration and urine output or to treat dehydration with balanced crystalloid solutions. Volume therapy in contrast is the quick restoration of intravascular losses such as an acute blood loss through application of balanced colloids or crystalloids. The goal of volume therapy is to maintain normal cardiac output and oxygen delivery by restoring intravascular normovolemia and cardiac pr…

Differential clearance of colloid and crystalloid solutions from the lung.

The hypothesis that alveolar fluid clearance depends on factors other than the alveolar-capillary oncotic gradient was tested by comparing lung clearance rates of three different colloid solutions and isotonic saline. The solutions (4 mL/kg) were instilled into the lower lobes of New Zealand rabbits. Colloid solutions were diluted to produce a fixed gradient of 8 mm Hg for each experiment. Fluid clearance was calculated at 3 hours, using lung gravimetrics. Surface tension, as a measure of surfactant inhibition, was measured in separate experiments using bubble surfactometry. Airway inflammator

Fluid resuscitation after an otherwise fatal hemorrhage: I. Crystalloid solutions.

One half of deaths among trauma victims occur within 1 hour of injury and are due to rapid hemorrhage or CNS trauma. We developed a rapid hemorrhage model in unanesthetized swine to simulate human exsanguination. We compared the ability of four crystalloid solutions to prevent death after an otherwise fatal hemorrhage: normal saline (NS), Ringer's lactate (RL), Plasmalyte-A (PA), and Plasmalyte-R (PR). Five days before hemorrhage swine received an aortic sideport and a central venous treatment catheter. Aortic blood (54 ml/kg) was removed in 15 minutes from 116 swine. The percentages of shed b…

Both blood and crystalloid-based extracellular solutions are superior to intracellular solutions for lung preservation.

OBJECTIVE: Lung transplantation remains limited by donor organ ischemic time, inadequate graft preservation, and reperfusion injury. We evaluated lung preservation with use of an extracellular solution, with or without the addition of blood, as compared with preservation with the intracellular Euro-Collins solution. METHODS: With use of an isolated, whole blood perfused/ventilated rabbit lung model, we studied three groups of animals. Lungs were flushed with Euro-Collins, low-potassium dextran, or 20% blood-low-potassium dextran solution. Lungs were harvested en bloc, stored inflated at 4 deg

Lactate versus acetate buffered intravenous crystalloid solutions: a scoping review.

BACKGROUND: Buffered crystalloid solutions are increasingly recommended as first-line intravenous resuscitation fluids. However, guidelines do not distinguish between the different types of buffered solutions. The aim of this scoping review was to assess the evidence on the use of lactate- vs acetate-buffered crystalloid solutions and their potential benefits and harms. METHODS: We conducted this scoping review in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. We searched PubMed, Embase, Epistemonikos, and the Cochrane Lib…

Effectiveness of different crystalloid i.v. solutions in establishing urine flow.

There are several situations in Emergency Medicine when it is desirable to promote a prompt diuresis to fill the bladder or obtain urine for diagnostic tests. We attempted to determine which of 3 commonly used intravenous solutions is most effective in establishing urine flow. In a prospective, randomized double-blind crossover study of 12 healthy male volunteers, we rapidly infused 20 cc/kg of D5W, D51/2NS, or 1/2NS immediately after voiding. Voided urine volumes were then recorded at 30, 60, 90, and 120 min postinfusion and the degree of glycosuria, if any, was noted. Total mean urine volume

Effect of pH shifts induced by oxygenating crystalloid cardioplegic solutions.

Oxygenation of a bicarbonate-containing crystalloid cardioplegic solution alters the partial pressure of both oxygen (O2) and carbon dioxide (CO2). Therefore, oxygenating St. Thomas' Hospital II plus glucose (11 mmol/L) cardioplegic solution with 95% O2 + 5% CO2 induces a pH shift to 7.0 (10 degrees C) as opposed to pH 9.3 with 100% O2. In an isolated working rat heart model, we show that pH 7.0 (10 degrees C) improves mechanical postischemic recovery in the absence or presence of O2. However, in the absence of O2, pH 7.0 appears to inhibit glycolysis and diminish the stability of cellular mem

Preventing superficial phlebitis during infusion of crystalloid solutions in surgical patients.

Thirty patients who received routine intravenous therapy (3.3% dextrose and 0.3% normal saline at a rate of 100 mL/h) were randomized prospectively into two groups to receive an antiphlebitic mixture of heparin-hydrocortisone-sodium bicarbonate or placebo in a double-blinded fashion for 4 to 7 days. The code was then broken and the data were analysed using chi 2 analysis with respect to the clinical grade and incidence of phlebitis. Phlebitis developed in 2 of 14 study and 10 of 16 control patients. All study patients tolerated their intravenous lines longer and needed less restarting. The ant
