Desflurane: a reappraisal.

Desflurane use is associated with carbon monoxide production and sympathetic activation. These side effects can be eliminated taking into account certain recommendations such as closing fresh gas flow at the end of anaesthesia, regular replacement of carbon dioxide absorbents and the use of new absorbents for carbon monoxide production and opioid administration with slow increases of inspiration concentrations during desflurane initiation for sympathetic activation.

[Application of desflurane in anaesthesiology].

Desflurane (Des)--is a modern inhalation anaesthetic available in Russia since August, 2013. Des is a halogenated ether; its chemical structure is 2-difluoromethoxy-1-1-1-2-tetrafluoroethane (C3H2F6O). Special thermocompensated evaporators are used for Des dosing. Low solubility in blood and tissues of an organism causes fast absorption and elimination of Des. Blood/gas distribution ratio of Des is 0.42. Des distinctive properties are high saturated vapor pressure, super short duration of action and average power. Furthermore it is characterized by the minimal metabolism and lack of interactio

Desflurane analgesia for vaginal delivery.

The use of subanaesthetic concentration of inhalational anaesthetic for vaginal delivery offers many advantages to the mother and newborn. Desflurane, with the characteristics of rapid onset and minimal metabolism, may provide better analgesia and safety for labour pain control. Eighty healthy parturients were randomly assigned to receive either desflurane 1.0-4.5% and oxygen (n = 40) or nitrous oxide 30-60% in oxygen (n = 40). Analgesia was assessed using a score from 0 (no relief) to 4+ (excellent analgesia), amnesia for the delivery, blood loss were recorded. Neonates were evaluated by Apga

Inhalational anesthetics: desflurane and sevoflurane.

This article reviews the physico-chemical properties and performance characteristics of the two new potent inhaled anesthetics, desflurane and sevoflurane. Both drugs provide a greater degree of control of anesthetic depth and a more rapid immediate recovery from anesthesia than is currently available with other inhaled agents because of their decreased solubility. Desflurane is currently in widespread clinical use in the United States and parts of Europe. Compared with sevoflurane, it has the additional advantage of being extremely resistant to degradation and biotransformation. However, its

[Malignant hyperthermia in a pig anesthesized with desflurane].

A large-white pig that had not been genetically selected to develop malignant hyperthermia (MH) during anesthesia nevertheless suffered an episode of severe MH after repeated exposure to increasing concentrations of desflurane. MH is a hypermetabolic alteration that may develop in susceptible patients who have inhaled certain drugs or agents that act as triggers. Early identification and appropriate treatment are essential to reduce the likelihood of death associated with this severe alteration. We report a case of late-developing MH triggered by low concentrations of inhaled desflurane.

Malignant hyperthermia involving the administration of desflurane.

PURPOSE: This report describes an episode of malignant hyperthermia (MH) in a ten year old boy receiving desflurane anaesthesia. CLINICAL FEATURES: Following induction of general endotracheal anaesthesia with thiopentone and succinylcholine, desflurane was administered for maintenance of anaesthesia. Ten minutes after commencing desflurane administration, heart rate and PETCO2 increased to 165 bpm and 50 mmHg, respectively. Initially, the tachycardia was attributed to a sympathetic response secondary to desflurane. Desflurane was discontinued and isoflurane was started. Minute ventilation was

Effects of desflurane on the central nervous system.

Desflurane causes dose-dependent decreases in cerebrovascular resistance and cerebral metabolic rate of oxygen consumption (CMRO2), suggesting that desflurane is a cerebral arteriolar dilator with global flow-metabolism coupling similar to halothane and isoflurane. Desflurane is also similar to isoflurane in that cerebrovascular responsivity to carbon dioxide appears to be maintained. In the dog, arterial hypotension to 40 mm Hg induced with 2.4 MAC desflurane resulted in global decreases in cerebral blood flow of 60% and CMRO2 of 20%. Concentrations of cerebral metabolites of high-energy phos

Desflurane versus sevoflurane: a comparison on stress response.

BACKGROUND: Neurohumoral, immunologic and metabolic alteration characterize surgical procedures in relation with the intensity of injury, the total operating time and the anesthetic technique. We, therefore, compared the effects of desflurane versus sevoflurane anesthesia on intra and postoperative release of the stress hormones and inflammatory cytokines. METHODS: Fifty Caucasian women undergoing laparoscopic surgery for benign ovarian cysts were randomized to receive inhaled anesthesia with desflurane (DES group; N.=25) or sevoflurane (SEVO group; N.=25), with fentanyl bolus and remifentani

Maintenance of anaesthesia in sheep with isoflurane, desflurane or sevoflurane.

Rapid recovery from anaesthesia is advantageous in small ruminants, to reduce the risk of regurgitation. Theoretically, the least soluble inhalation agents should result in the fastest recoveries, but using additional injectable agents may negate this advantage. This study compared three inhalation agents for the maintenance of anaesthesia in sheep. Eighteen ewes that were to undergo orthopaedic surgery were allocated to one of three groups. Each group was premedicated with xylazine (0.1 mg/kg intramuscularly), anaesthesia was induced using ketamine (2 mg/kg) and midazolam (0.03 mg/kg) intrave

Desflurane is a trigger of malignant hyperthermia in susceptible swine.

Desflurane (difluoromethyl 1-fluoro 2,2,2-trifluoroethyl ether: CF2-H-O-CFH-CF3) is a potent inhalation anesthetic agent being investigated for possible clinical use. The authors examined the effects of this agent on normal swine and those from a special breeding program that were considered purebred for susceptibility to malignant hyperthermia (MH). Animals were exposed to 1 or 2 MAC or both doses of desflurane and observed for changes in end-tidal CO2, arterial blood gases, lactate, catecholamines, core temperature, blood pressure, and heart rate. All normal swine tolerated exposure to desfl

The science behind banning desflurane: A narrative review.

Potent inhaled anaesthetics are halogenated hydrocarbons with a large global warming effect. The use of fluorinated hydrocarbons (most are not anaesthetics) are being restricted but volatile anaesthetics have been exempted from legislation, until now: the EU has formulated a proposal to ban or at least severely restrict the use of desflurane starting January 2026. This narrative review addresses the implications of a politics-driven decision - without prior consultation with major stakeholders, such as the European Society of Anaesthesiology and Intensive Care (ESAIC) - on daily anaesthesia pr

Effects of sevoflurane and desflurane on otoacoustic emissions in humans.

Otoacoustic emissions (OAEs) are non-invasive, easy to apply and objective test methods which are widely used to determine the presence of hearing in audiology clinics. Under certain circumstances, the study should be applied under general anesthesia. The aim of this study was to determine the influence of new short-acting inhalation agents, desflurane and sevoflurane, on OAE in humans. These short-acting agents are widely used in general anesthesia. Thirty-one healthy patients who underwent septoplasty and turbinoplasty surgery were included in this study. Unpremedicated patients were anesthe
