Infantile nephrocalcinosis with chronic diarrhea.

A 3-month-old girl presented with symptoms indicative of dysuria, gross hematuria, chronic diarrhea, and nephrocalcinosis. She was born to fourth-degree consanguineous parents. The infant exhibited hypercalciuria and hyperoxaluria, and her stool tested positive for reducing sugars. She responded positively to dietary lactose restriction, and her hyperoxaluria improved over the course of 3 months. This suggests an enteric hyperoxaluria phenotype, although her hypercalciuria persisted. Whole exome sequencing identified a homozygous variant in the LCT gene, which is associated with congenital lac

[Balance technic in the syndrome of infantile diarrhea].

Ninety-six infants were studied with water balance technique at the Infant Unit of Hospital Roberto del Río, Santiago de Chile. Sixty-eight with acute diarrhoea and twenty-eight with protracted diarrhoea associated to carbohydrate intolerance. Total water losses in most of them were in the range of 150 to 225 ml/kg day. Stool loses were in the range of 25 to 75 gm/kg/day, with humidity percentage varing from 97.4 to 92.5%. In patients with acute diarrhoea, stool losses and humidity percentage showed slight decreasing tendency in the following balances. By contrast, those with protracted diarrh

[Infantile diarrhea in Libreville (Gabon). Ecological studies].

The identification of different infectious agents found in infantile diarrheas was undertaken in 237 children hospitalized in pediatric institutions in Libreville. In neonates between 0 and 18 months of age, the most common pathogen was Rotavirus (20.6%), followed by Shigella (10.8%), E. coli (9.24%), Salmonella (3.46%), E. histolytica (0.8%) and Y. enterocolitica (0.4%). The highest percentage of diarrheas caused by rotaviruses was found among children between 6 and 11 months of age. The frequency of Salmonella among children 2 to 4 years of age is almost the same at that in children under 2

Traditional enteropathogenic Escherichia coli of infantile diarrhea.

First described in 1885, Escherichia coli gradually emerged as a cause of infantile diarrhea. Currently, four pathogenic categories of diarrheagenic E. coli are recognized: enteropathogenic (EPEC), enterotoxigenic (ETEC), enteroinvasive (EIEC), and enterohemorrhagic (EHEC). Of these, ETEC and EPEC are important causes of pediatric diarrhea, especially in developing countries. ETEC strains are characterized by their production of well-defined heat-labile or heat-stable enterotoxins. In the absence of other identifying characteristics, EPEC strains have been identified by serotyping only; recent

[Cryptosporidium as a co-pathogen in infantile diarrhea and pneumonia].

The authors describe a case of cryptosporidiosis in a 10 month old immunocompetent infant, who suffered from prolonged diarrhoea and pneumonia. Cryptosporidium oocysts, Giardia lamblia were detected by using modified Kinyoun stain, and klebsiella was identified in the stool culture. According to the clinical presentation and the laboratory data the streptococcus infection is presumed as cause of the pneumonia, but the authors could not exclude the role of cryptosporidium in predisposing and/or causing the pneumonia.
