In June 1962 at the age of 87 years, Sir Winston Churchill (1874-1965) fell over in his hotel room at the Hotel de Paris in Monte Carlo and sustained a fracture to the neck of his left femur. Experiments show that the cancellum of some femoral heads and of the trochanteric region is too weak to support a nail. A simple classification of traumatic femoral neck fractures is presented. These are either vertical or, less commonly, transversely disposed. Either type may be undisplaced, impacted, or displaced. For adequate fixation of undisplaced vertical and all displaced fractures a nail plate is essential. For undisplaced transverse fractures and impacted vertical fractures two screws usually suffice. We introduce a simple method of sonographic determination of femoral neck anteversion in children with which the even difficult angels in terms of measurement, those up to 70 degrees, can be measured with sufficient reliability: The examination was done with a defined internal 40 degree rotation of the legs reached by a supporting wedge. We proved this method on 55 children's hips of which we already had an existing radiologic determination of the anteversion angle. Compared to the radiological results we found an average deviation of 3.6 degrees (standard deviation = 3.2).

A detailed cost analysis of fractures of the neck of femur in elderly patients has been conducted at Aberdeen Royal Infirmary. In 29 patients with fractures of the femoral neck (group A) and 22 patients operated on for osteoarthritis of the hip (group) B), plasma levels of 25-hydroxycholecalciferol (25-OH-D3) were compared with the results of histological bone tests. A third group of 19 patients at the Geriatric hospital served as a control in this age group for 25-OH-D3 values (group C). Necrosis of the head of the femur with segmentary collapse after transcervical fractures occurs in 30 to 50% of the cases. Diagnosis of avascular necrosis should be made early before the head collapses. In a clinical and experimental study, we attempted to perform early screening for this complication.
