The great majority of sterile products commercially available as well as prepared in compounding pharmacies are sterilized by sterile filtration during aseptic processing. This brief and basic review will highlight the nature, action, and use of sterilizing filters. Special emphasis is given to how filters are validated in producing a sterile filtrate while being compatible with the filtered solution, as well as how filters are integrity tested during aseptic processing.

Among 10 patients with primary open-angle glaucoma with a filtration "pseudo-cushion" that prevented compensation of intraocular pressure after trabeculectomy, normalization of intraocular pressure by means of medicamentous treatment (thymolol) could be achieved in 1 patient; in the rest of the patients surgical intervention was required. It is stated that the most effective is early surgical intervention: dissection of the Tenon's capsule "cyst" and resection of the episclera in the zone of the primary surgery.

Priapism has been observed during two out of 3,680 filtration leukapheresis procedures in male donors and has been reported during hemodialysis. Both procedures are associated with enhanced granulocyte adhesion and aggregation presumably due to C5a. During both procedures, heparin is administered and this drug has been shown to cause heparin-dependent anti-platelet antibodies. It is suggested that complement mediated venous leukostasis or immune-induced platelet aggregates might impair the normal blood flow from the penis and result in a state of priapism.

For pharmacologic screening to identify hyperaldosteronism in patients after renal transplantation, spironolactone (Soludactone, Searle) was administered intravenously in 12 subjects (mean dose 1.2 g in the course of 3 days). Increased intraocular pressure (IOP) in encapsulated filtration blebs was evaluated in 49 eyes of 49 patients followed for 6 to 48 months (mean +/- standard deviation, 19.7 +/- 12.6 months).
