During gonioscopy the aqueous humor can be shifted by appropriate indentation with the goniolens (Forbes test). The iris root is affected in a variety of ways: it may sink in, troughlike, or be deformed to an S-shape, or remain unchanged. The different findings depend on the configuration of the ciliary processes and the position of the lens equator; they enable conclusions to be drawn concerning the risks of inverse reaction to miotics or of ciliary lens block (malignant) glaucoma.

PURPOSE: To describe an infrared gonioscopy system designed to observe the anterior chamber angle under natural mydriasis in a completely darkened room.
METHODS: An infrared light filter was used to modify the light source of the slit-lamp microscope. A television monitor connected to a CCD monochrome camera was used to indirectly observe the angle.
RESULTS: Use of the infrared system enabled observation of the angle under natural mydriasis in a completely darkened room.
CONCLUSION: Infrared gonioscopy is a useful procedure for the observation of the angle under natural mydriasis.

Herein we describe 2 cases of persistent mydriasis after gonioscopy-assisted transluminal trabeculotomy for open angle glaucoma. Both surgeries were uneventful, but the patients experienced postoperative hyphema and intraocular pressure elevation. They then developed persistent fixed and dilated pupils resistant to pilocarpine that led to intolerable photosensitivity and glare. An iris cerclage pupilloplasty was performed with adequate relief of symptoms in one case.

This case report outlines the successful management of angle recession (AR) with gonioscopy-assisted transluminal trabeculotomy (GATT). A 53-year-old man with a history of blunt trauma to the left eye resulting in AR presented with an IOP of 38 mm Hg on 4 antiglaucoma agents. After GATT, a notable reduction in IOP to 10 mm Hg was observed on the first postoperative day. Subsequent 1-year follow-up visits demonstrated sustained improvement, with IOP stabilizing at 18 mm Hg without the need for antiglaucoma medication. PURPOSE: To report a case of gonioscopy-assisted transluminal trabeculotomy (GATT) in a patient with primary congenital glaucoma.
OBSERVATIONS: A three-year-old boy who presented with buphthalmos and elevated intraocular pressure. Despite the presence of iris and iris processes extending to Schwalbe's line, GATT was performed successfully.
CONCLUSIONS: GATT may be successful with primary congenital glaucoma even when angle structures are not initially visible.

METHODS: A 71-year-old male patient with advanced pseudoexfoliation glaucoma and dense cataract was planned to perform combined GATT and cataract extraction.
