Hereditary Hyperferritinemia.

Ferritin is a ubiquitous protein that is present in most tissues as a cytosolic protein. The major and common role of ferritin is to bind Fe … egulation of ferritin synthesis, secretion, and uptake, and in its central role in iron metabolism, hyperferritinemia is a relatively common finding in clinical practice and is found in a large spectrum of conditions, both genetic and acquired, associated or not with iron overload. The diagnostic strategy to reveal the cause of hyperferritinemia includes family and …

Hyperferritinemia and inflammation.

Understanding of ferritin biology has traditionally centered on its role in iron storage and homeostasis, with low ferritin levels indicativ … protective, limiting the production of free radicals and mediating immunomodulation. Additionally, hyperferritinemia is a key acute-phase reactants, used by clinicians as an indication for therapeutic intervention, aimed at controlling inflammation in high-risk patients. One school of thought maintains that hyperferritinemia is an 'innocent bystander' biomarker of …

Hyperferritinemia in autoimmunity.

Controlling iron/oxygen chemistry in biology depends on multiple genes, regulatory messenger RNA structures, signaling pathways and protein … , Alzheimer disease, and restless leg syndrome. Mutations in the ferritin gene cause the hereditary hyperferritinemia-cataract syndrome and neuroferritinopathy. Hyperferritinemia is associated with inflammation, infections and malignancies, and in systemic lupus erythematosus correlates with disease activity. Some evidence points to the importance of hyperferritine …

Febrile Exanthem with Hyperferritinemia.

Diagnosis of an adult with acute generalized exanthem coupled with spiking fevers, arthralgias, and myalgias requires careful consideration. History and physical examination are basic necessities, while laboratory studies can be valuable adjuncts. The authors present a case of adult onset Still's disease, discuss the differential diagnoses, and highlight the utility of high serum ferritin in identifying this febrile exanthem.

[Differential diagnosis of hyperferritinemia].

Hyperferritinemia is a common finding in clinical practice, and its origin is associated with more than one etiology in 40% to 70% of cases. This multifactorial nature requires a comprehensive differential diagnostic approach. A thorough clinical evaluation is crucial to guide the diagnosis, as seric ferritin (SF) is also an acute phase reactant and can act as a biomarker in diseases where inflammation is central. Most cases with elevated FTs do not have iron overload. It is essential to distinguish iron overload from other causes of hyperferritinemia, as the management, treatment, and prognos…

Hyperferritinemia-A Clinical Overview.

Ferritin is one of the most frequently requested laboratory tests in primary and secondary care, and levels often deviate from reference ranges. Serving as an indirect marker for total body iron stores, low ferritin is highly specific for iron deficiency. Hyperferritinemia is, however, a non-specific finding, which is frequently overlooked in general practice. In routine medical practice, only 10% of cases are related to an iron overload, whilst the rest is seen as a result of acute phase reactions and reactive increases in ferritin due to underlying conditions. Differentiation of the presence…

[Hereditary hyperferritinemia cataract syndrome].

HISTORY: A 42-year-old man was found to have a four to six fold increase in the level of plasma ferritin since four years. In the age of 10 he had undergone unilateral resection of a dysplastic kidney associated with systemic hypertension. He had also developed recurrent venous thromboses caused by atresia of the inferior vena cava with azygos continuation, known since 23 years. Iron overload or hemochromatosis had been excluded, but despite numerous investigations the exact cause of the hyperferritinemia had not been elucidated. The patient, his grandfather, his mother and a brother had under…

Pathogenesis of hyperferritinemia cataract syndrome.

Hereditary hyperferritinemia-cataract syndrome (HHCS) is an autosomal dominant disorder characterized by bilateral cataracts and increased serum L-ferritin, in the absence of iron overload. Under physiological conditions, ferritin synthesis is finely regulated at the translational level by iron availability. This regulation is achieved by the high-affinity interaction between cytoplasmic mRNA-binding proteins (iron regulatory proteins, IRPs), and mRNA stem-loop structures, known as iron responsive elements (IREs), located in the untranslated regions (UTRs) of the mRNAs. A single IRE is located…

Extreme Hyperferritinemia: Causes and Prognosis.

The significance of extreme hyperferritinemia and its association with certain diagnoses and prognoses are not well characterized. We performed a retrospective analysis of adult patients with at least one total serum ferritin (TSF) measurement ≥ 5000 µg/L over 2 years, in three university hospitals. Conditions associated with hyperferritinemia were collected, and patients were classified into 10 etiological groups. Intensive care unit (ICU) transfer and mortality rates were recorded. A total of 495 patients were identified, of which 56% had a TSF level between 5000 and 10,000 µg/L. There were …

Hyperferritinemia in Critically Ill Patients.

OBJECTIVE: Hyperferritinemia is frequently seen in critically ill patients. A rather rare though life-threatening condition related to severely elevated ferritin is hemophagocytic lymphohistiocytosis. We analyze ferritin levels to differentiate hemophagocytic lymphohistiocytosis from other causes of hyperferritinemia in a mixed cohort of critically ill patients. DESIGN: Retrospective observational study. SETTING: Adult surgical, anesthesiologic, and medical ICUs of a university hospital. PATIENTS: Critical care patients (≥ 18 yr old) admitted to any of the adult ICUs at Charité - Universitä…

[Hereditary hyperferritinemia syndrome and cataract].

UNLABELLED: Hereditary hyperferritinemia cataract is a recently described autosomal dominant syndrome, characterized by bilateral cataracts and elevated level of serum ferritin. PATIENTS: Three members of a family were investigated for cataract and hyperferritinemia. A 30-year-old woman had elevated serum ferritin levels and bilateral cataracts. She was treated for hemochromatosis, but serum iron and transferrin saturation were normal. Her two sons, nine and five years old, also had a high ferritin level and bilateral cataracts. RESULTS: The ferritin level was 1200 micrograms/L in the woman'…

Hyperferritinemia as a Clue to Neuroendocrine Carcinoma.

Ferritin is the protein that serves as the main mechanism for iron storage. It can also be an acute-phase reactant, which may rise in response to inflammation, infection, injury, autoimmune disease, or malignancy. Therefore, when evaluating ferritin levels, it is important to consider both iron storage and potential underlying conditions that could cause hyperferritinemia. We report a case of an elderly man in his late 80s who had elevated ferritin levels for years before ultimately being diagnosed with metastatic neuroendocrine carcinoma. The initial diagnosis was suspected to be due to iron …

Hyperferritinemia: Important Differentials for the Rheumatologists.

Ferritin is commonly used as a marker for iron status, aiding in diagnosing iron deficiency anemia. However, it is also an acute phase reactant often elevated in various inflammatory conditions. Marked hyperferritinemia, defined as ferritin levels above 10,000 μg/L, can indicate severe underlying conditions, including infections, cardiovascular like heart failure, endocrinological, autoimmune, and malignancies. This case report highlights the differential diagnoses and clinical implications of hyperferritinemia from a rheumatological perspective. Here are two case reports illustrating the use …

Hyperferritinemia in dogs with splenic hemangiosarcoma.

Serum ferritin concentration increases in dogs in association with various diseases. In this study, we measured serum ferritin levels in dogs with splenic masses, using a sandwich ELISA assay. Eleven dogs with hemangiosarcoma (HSA), six with hematoma, 1 with hemangioma and 3 with lymphoma were enrolled. All dogs with HSA had serum ferritin concentrations above the normal limit (1,357 ng/ml, mean + 2× standard deviation of normal). Increased serum ferritin concentrations have also been observed in few cases of hematoma, hemangioma and lymphoma. Therefore, hyperferritinemia is not specific for s…

Hyperferritinemia: Pathophysiology, Etiologies, and Diagnostic Approach.

OBJECTIVES: Hyperferritinemia is common in adults, yet most lack true iron overload. Confusion among metabolic, inflammatory, genetic, and reactive causes drives under-investigation and over-treatment, compounded by SGLT2 inhibitors and, less certainly, GLP-1 receptor agonists. We synthesize the hepcidin-ferroportin axis as the unifying mechanism and a transferrin saturation (TSAT)-guided pathway for workup. METHODS: Narrative review with structured PubMed, Embase, and Cochrane searches through April 2026, written to address the SANRA quality domains. RESULTS: Ferritin above 10 000 μg/L flag…

Possible dysmetabolic hyperferritinemia in hyperinsulinemic horses.

BACKGROUND: Hyperinsulinemia associated with equine metabolic syndrome and pituitary pars intermedia dysfunction is a risk factor for laminitis. Research in other species has shown elevated body iron levels as both a predictor and consequence of insulin resistance. In humans, this is known as dysmetabolic hyperferritinemia. AIM: To explore the relationship between equine hyperinsulinemia and body iron levels. METHODS: We reviewed case histories and laboratory results from an open access database maintained by the Equine Cushing's and Insulin Resistance Group Inc. (ECIR). We identified 33 hor…

Hyperferritinemia in children hospitalized with scrub typhus.

BACKGROUND: Hyperferritinemia is increasingly associated with mortality in sepsis. Studies estimating the prevalence of hyperferritinemia in pediatric scrub typhus are limited. METHODS: This was a secondary analysis of a prospective observational study (FERRIS) from a tertiary care teaching hospital in North India where 72 children with confirmed scrub typhus, 4 (5.5%) PCR positive, 55 (76.4%)-IgM ELISA positive, and 13 (18.1%)-both PCR and ELISA positive, were analyzed. Serum ferritin was measured in 62 children to identify the prevalence of hyperferritinemia and determine its association wi…
