OBJECTIVE: To evaluate the benefit of iris biopsy in cats with iris hyperpigmentation to differentiate melanosis from early feline diffuse iris melanoma (FDIM).
METHODS: The medical records of cats with unilateral iris hyperpigmentation that had undergone iris biopsy between February 2013 and September 2016 at Willows Veterinary Centre & Referral Service were reviewed.
RESULTS: Seven cats with unilateral iris hyperpigmentation were included in this retrospective study. The biopsy procedure was performed under general anesthesia (n = 7) with neuromuscular blockade (n = 6) following pre-operat…

PURPOSE: To report an ocular side effect of topical latanoprost therapy.
DESIGN: Single interventional case report.
METHODS: A 73-year-old woman on latanoprost for primary open-angle glaucoma developed an iris cyst simulating an iris melanoma.
RESULTS: The lesion disappeared over 8 weeks when latanoprost was stopped.
CONCLUSIONS: In managing patients with iris-pigmented lesions, the list of medications should be reviewed. If the patient takes latanoprost, a trial off latanoprost is warranted.

PURPOSE: To report a patient with a unilateral presentation of glaucoma, pain, and acute iris transillumination syndrome simulating iris melanoma.
OBSERVATIONS: A 53-year-old male presented with blurred vision and pain in his right eye several weeks following a respiratory sinus infection managed by oral azithromycin. Examination of the right eye was notable for elevated intraocular pressure of 46 mm Hg, an irregular mid-dilated pupil, and diffuse iris transillumination with pigmentary seeding on the iris surface, in the anterior chamber angle, and on the sclera, suspicious for diffuse iris m…

The flexible iris prosthesis according to Koch with a customized iris design serves for anterior segment reconstruction in patients with partial or complete aniridia. It is designed for implantation in the ciliary sulcus and is recommended only for pseudophakic or aphakic eyes. The multilayered implant consists of a core of a silicone matrix with incorporated color pigment coated with another layer of medical grade silicone. As a design variant the implant is available with or without an additionally embedded tissue layer. Without tissue the implant is more flexible, can be easily folded and p…

The iris is the least common site of primary uveal melanoma. The prognosis of iris melanoma is better than that of melanoma of the ciliary body and choroid, but the reason for this difference is unclear. One possible explanation is that iris melanoma is smaller than its posterior segment counterparts at the time of diagnosis. Most iris melanomas are spindle cell types, according to a modified Callender classification system. There is evidence that the proliferation of melanocytes of the anterior iris surface (iris plaque) and diffuse stromal invasion may be risk factors for local recurrence an…

PURPOSE: To study artificial iris intraocular lens (IOL) or ring specially designed for correction of aniridia or iris deficiencies associated with cataract or aphakia.
PATIENTS AND METHOD: Nine patients with partial or total aniridia underwent the surgical procedure. A black (Morcher 67F) or brown (Ophtec 311) diaphragm intraocular lens was used for the seven patients with aphakia. Morcher 50C rings were used for the patients with cataract.
RESULTS: The esthetic results were satisfactory. Photophobia was reduced in all cases. Postoperative complications included one case of transient hypert…

PURPOSE: To evaluate the safety and efficacy of surgical implantation of prosthetic iris devices in patients with iris deficiency.
METHODS: Nine patients with traumatic iris defects, congenital aniridia or iris coloboma, and surgical or optical iridectomies were included in a noncomparative case series. Cataract surgery with intraocular lens and prosthetic iris implantation was performed in 10 eyes. The visual acuity, subjective degree of glare disability, postoperative anatomic results, and intraoperative and postoperative complications were evaluated.
RESULTS: The mean follow-up was 17.75 …

Immune reconstitution inflammatory syndrome (IRIS) in patients with human immunodeficiency virus (HIV) and Pneumocystis jirovecii pneumonia infection reflects an exaggerated inflammatory response of the host immune system to an antigen, which is temporally related to recovery of the immune system. Clinical manifestations include fever, cough, dyspnoea and hypoxia following the commencement of antiretroviral therapy. Diagnosis is made on clinical and radiological criteria with exclusion of other infective and non-infective causes. Unrecognized, IRIS may be associated with significant morbidity …

Background/Objectives: To compare the clinical outcome of suture and sutureless iris fixation techniques for dislocated intraocular lenses (IOLs). Methods: Retrospective cohort study including patients who underwent surgery for late IOL dislocation over a 10-year period. IOL repositioning was achieved either by suturing the original IOL to the iris using the Siepser slipknot technique or by replacing it with a retropupillary sutureless iris claw IOL. Data collected during surgery included the type of dislocation, the need for anterior or posterior vitrectomy, duration of surgery, and intraoper…

The case is presented of a 32 year-old male who arrived with acute bilateral symptoms with blurred vision, red eye, severe photophobia and severe ocular pain after suffering from a flu-like syndrome. The patient presented with a clinical picture of bilateral involvement characterised by pupils in mid-mydriasis, scarcely reactive to light, iris transillumination, diffuse depigmentation of the iridian stroma, pigment dispersion in the anterior chamber, and ocular hypertension. After the eye examination an inflammatory syndrome and pigmentary glaucoma were ruled out. The patient showed depigmenta…

PURPOSE: To discuss the limitations and benefits of the BrightOcular prosthetic artificial iris device in management of aniridia associated with aphakia or cataract.
METHODS: This is a retrospective study including 5 eyes of 4 patients who underwent implantation of the BrightOcular iris prosthesis (Stellar Devices) for total or partial aniridia. The cases included 2 eyes of 1 patient with congenital aniridia associated with congenital cataract and 3 eyes with traumatic aniridia: 1 with subluxated cataractous lens and 2 with aphakia. In all cases, the iris prosthesis was implanted after a 3-pi…

Intraoperative floppy iris syndrome (IFIS) as well as lens-iris diaphragm retropulsion syndrome (LIDRS) may negatively influence the course of the modern cataract procedure. In case of simultaneous presence of both syndromes, the LIDRS may paradoxically ease the surgery by means of partial or transient elimination of the IFIS. It causes the dilatation of the pupil, pushes the iris back, and deepens the anterior chamber and this way reduces the fluttering of the iris and makes possible to perform important phases of the surgery, during that we are working in the proximity of the pupil. In case …
