Fraternal keloid.

This report concerns the cases of three European caucasian brothers who developed keloid scars as teenagers. The eldest brother's keloid followed surgery, the second brother's followed ear piercing and the youngest brother's followed chicken pox scarring. No other member of any generation of their family is known to have suffered from the condition. Such a family as this, with an inherited tendency to form keloids, provides an interesting source of material for genetic research. An understanding of the genetics of keloids, particularly the isolation of a specific gene or group of genes could p…

A surgical approach for earlobe keloid: keloid fillet flap.

Earlobe keloid can form after cosmetic ear piercing, trauma, or burns, and it poses several difficulties in treatment and distinctive cosmetic implications. Treatment methods for earlobe keloids include both surgical and nonsurgical methods. After excision of the earlobe keloid, healing by secondary intention, primary suture, skin graft, or local flap has revealed some disadvantages. The authors approached this problem with a new excision and covering method. The surgery was performed under local anesthesia. Skin over the keloid was dissected from the keloid mass as a flap, which they termed a…

Penile Keloid Successfully Cured in a Keloid High-Tendency Patient.

Keloid scars are a relatively common condition but extremely rare in the penis. This case aims to be added to the previous 34 cases of penile keloid reported in the literature. We present the case of a 15-year-old patient with a high keloid-forming tendency who was successfully cured of penile keloid scarring with excision alone and without any adjuvant therapy. No recurrence was reported over eight years of follow-up. In contrast, at the same time, recurrence is frequently observed in other body parts after excision, making this particular part of the body an area of less keloid occurrence an…

Keloid Skin Flap Retention and Resurfacing in Facial Keloid Treatment.

OBJECTIVE: Facial keloids commonly occur in young patients. Multiple keloid masses often converge into a large lesion on the face, representing a significant obstacle to keloid mass excision and reconstruction. We describe a new surgical method that excises the keloid mass and resurfaces the wound by saving the keloid skin as a skin flap during facial keloid treatment. METHODS: Forty-five patients with facial keloids were treated in our department between January 2013 and January 2016. Multiple incisions were made along the facial esthetic line on the keloid mass. The keloid skin was dissecte…

Congenital corneal keloid.

The clinical course and pathological findings in an infant with a congenital corneal opacity, aniridia, and cataract of the right eye and anophthalmia of the left orbit are reviewed. Although the lesion was thought to represent a dermoid pre-operatively, the keratoplasty specimen revealed disorganized and vascularized connective tissue, with thickened, keratinized epithelium. Bowman's and Descemet's membranes as well as the endothelium were absent. These histopathologic findings were felt to represent a congenital corneal keloid. The co-existence of aniridia in the involved eye and anophthalmi…

Development of Postsurgical Pyoderma Gangrenosum with New Keloid after Keloid Resection.

Keloids are a dermal fibroproliferative disorder and can arise from trauma, acne, vaccination, and herpes zoster. Pyoderma gangrenosum (PG) is a painful ulcerative skin disorder that is associated with neutrophilic dysfunction. However, the pathophysiologies of keloids and PG are not fully understood. The authors encountered an unusual case of a 24-year-old woman who presented with an anterior chest keloid that bore an ulcer. The keloid was resected along with the ulcer, and histology revealed the ulcer to be a neutrophilic PG ulcer. A year after surgery, another ulcer developed in the scar. T…

Recurrent plantar keloid.

Keloids of the plantar foot present a unique challenge to the surgical dermatologist. Many of the established regimens often fall short of their desired goals. Some of the obstacles to overcome include the repetitive nature of ambulation, the inability to primarily close the plantar foot, and the exquisite tendency for even fine suturing of skin grafts to form keloids. The use of excision, postoperative electron beam therapy, and secondary intention healing provides a useful approach in the management of plantar keloids.

[Tumor characteristics of keloid].

In clinical work, it is observed that keloid has significantly different characteristics from other types of scar such as hypertrophic scar. The growth of keloid usually exceeds the margin of original wound and continuously invades the surrounding skin, and keloid has a certain recurrence rate after various treatment measures such as surgery and glucocorticoid injection, etc. The above phenomenon suggests that keloid has certain tumor characteristics, and we cannot judge keloid from the perspective of scar alone. This article attempts to re-describe the pathogenesis of keloid from the perspect…

Digital retractile keloid in a context of spontaneous keloids - a case report.

We report the extremely rare case of a digital and plantar retractile keloid in a 32-year-old patient with a history of spontaneous keloids. Since childhood, she has developed hypertrophic scars following minor trauma, extending beyond the original wound margins. She currently presents a left IV-V syndactyly and a complete flexion contracture of the fifth finger (Tubiana stage IV), as well as a retraction of the right hallux, resulting in functional impairment. The goal of surgical treatment is to restore extension. Retractile keloids occurring in the context of spontaneous keloids are unusual…

Advancing Keloid Treatment: A Novel Multimodal Approach to Ear Keloids.

BACKGROUND/OBJECTIVE: Management of keloids of the pinna, in particular, those located in the helix and antihelix and lobule that occur as complications of ear piercing. MATERIALS AND METHODS: Retrospective analysis of 49 patients treated with extralesional surgical excision of keloids localized to the ear followed by the application of autologous platelet-rich plasma (PRP) to wound site and postoperative in-office superficial radiation therapy (SRT). Radiation protocol consisted of 1 to 3 fractions, with cumulative dosage ranging from 1,300 to 1,800 cGy. Average follow-up was 24 months to as…

The epigenetics of keloids.

Keloid scarring is a fibroproliferative disorder of the skin with unknown pathophysiology, characterised by fibrotic tissue that extends beyond the boundaries of the original wound. Therapeutic options are few and commonly ineffective, with keloids very commonly recurring even after surgery and adjunct treatments. Epigenetics, defined as alterations to the DNA not involving the base-pair sequence, is a key regulator of cell functions, and aberrant epigenetic modifications have been found to contribute to many pathologies. Multiple studies have examined many different epigenetic modifications i…

Keloidal scleroderma.

Progressive systemic sclerosis (PSS) can involve the skin diffusely or in a limited way. Rarely cutaneous involvement can assume a hypertrophic form, described in the literature as keloidal scleroderma. The authors describe a PSS patient who presented cutaneous manifestations compatible with the diagnosis of this clinical situation. The clinical features, pathogenic mechanism and therapeutic approach are discussed and a broad review of the literature is given.
