Covering: 2000 to 2022Natural products are a vital source of compounds for use in agriculture, medicine, cosmetics, and other fields. Macrolides are a wide group of natural products found in plants and microorganisms. They are a group of polyketides constituted of different-sized rings and characterized by the presence of a lactone group. These compounds show different biological activities, such as antiviral, antiparasitic, antifungal, antibacterial, immunosuppressive, herbicidal, and cytotoxic activities. This review is focused on macrolides isolated from fungal sources, examining their biol…

Macrolides and lincosamides are first choice bacteriostatic antibiotics used in veterinary dermatology. The main antibiotics in these classes are erythromycin, lincomycin, clindamycin and tylosin. They are well absorbed if given orally and are able to penetrate well into infected skin. Their spectrum of action comprises bacteria commonly associated with skin infections, including staphylococci. Their main disadvantages are the rapid development of bacterial resistance and occasional gastroenteric upset, most often seen with erythromycin. More recently developed macrolides, such as azithromycin…

Macrolides are widely used antibiotics, but their clinical benefits in the setting of infection and sepsis span beyond their antimicrobial spectrum. They possess immunomodulatory properties, characterized, across preclinical models, by effects on macrophage M1/M2 polarization, autophagy, neutrophil extracellular trap formation, cytokine release, and bacterial virulence factors. In hospitalized CAP, macrolide and beta-lactam combination therapy confer survival benefit, as supported by observational studies and meta-analyses, and reinforced by a recent randomized trial. In non-CAP sepsis, three …

Macrolides and ketolides are two families of antibiotics that share the same mechanism of action. They bind to different bases of the peptidyl transferase center of 23S RNA. The antibacterial spectrum of these drugs virtually overlaps, but dissimilarities in the affinity and number of binding sites results in differences in the intensity of their antibacterial effects (bacteriostatic or bactericidal) and their activity against strains with acquired resistance mechanisms. These agents are active against most gram-positive microorganisms and many intracellular microorganisms. Over the last ten y…

BACKGROUND: Diffuse panbronchiolitis (DPB) is a chronic airways disease predominantly affecting East Asians. Macrolides, a class of antibiotics, have been used as the main treatment for DPB, based on evidence from retrospective and non-randomised studies.
OBJECTIVES: To assess the efficacy and safety of macrolides for DPB.
SEARCH METHODS: We searched CENTRAL (2014, Issue 6), MEDLINE (1966 to July week 1, 2014), EMBASE (1974 to July 2014), Chinese Biomedical Literature Database (CBM) (1978 to July 2014), China National Knowledge Infrastructure (CNKI) (1974 to July 2014), KoreaMed (1997 to Jul…

BACKGROUND: Asthma is a chronic disease in which inflammation of the airways causes symptomatic coughing, wheezing, and difficult breathing. The inflammation may have different underlying causes, including a reaction to infection in the lungs. Macrolides are antibiotics with antimicrobial and antiinflammatory activities that have been used long-term to control asthma symptoms.
OBJECTIVES: To assess the effects of macrolides for managing chronic asthma.
SEARCH METHODS: We searched the Cochrane Airways Group Specialised Register up to April 2015. We also manually searched bibliographies of pre…

This article is an attempt to analyze and discuss the role and the purported mechanisms of azithromycin (AZM) in non-eosinophilic severe asthma, including antineutrophil activity, an effect on gastroesophageal reflux or antibacterial activity against an underlying chronic infection, such as Chlamydia pneumoniae. Macrolides have an expanding role in the therapy of chronic inflammatory diseases based on their additional anti-inflammatory and immunosuppressive properties. Many studies have been performed in lung transplantation field and maintenance treatment has been proved to be effective in cy…

Following the first outbreaks of legionnaire's disease, erythromycin emerged as the treatment of choice without the foundation of rigorous clinical trials. The number of therapeutic failures with erythromycin, as well as the side-effects and drug interactions, led to the consideration of other drugs such as the new macrolides and quinolones for the treatment of legionnaire's disease in the 1990s. In this article, 19 studies in in-vitro intracellular models and seven animal studies that compared macrolides to quinolones were reviewed. Quinolones were found to have greater activity in intracellu…

We have noticed that macrolides can improve the main symptoms of many patients with chronic pharyngitis. We feel that the efficacy of macrolides for chronic pharyngitis can be due to the anti-inflammatory effect of this group of antibiotics. However, we think that at present the use of macrolides is not the therapy of choice for chronic pharyngitis.

The antimicrobial spectrum of azithromycin and clarithromycin suggests a number of further uses for these newer macrolides. Favorable clinical and bacteriologic responses have been reported with both antibiotics in children with community-acquired pneumonia. Response rates were high for overall patient populations and for subgroups with infection caused by Mycoplasma pneumoniae and Chlamydia pneumoniae. Treatment with azithromycin or clarithromycin has resulted in a reduction in mycobacteremia and an improvement in clinical symptoms in adult AIDS patients with disseminated Mycobacterium avium-…

Macrolides, lincosamides, tetracyclines and chloramphenicol are structurally unrelated antibiotics which share protein synthesis inhibition as their common mechanism of action. Despite their individual differences, they can all be considered broad spectrum antibiotics with practical use for a wide variety of infections. Due to their similarities in function, however, concurrent or sequential administration of these agents must be undertaken with caution in order to prevent antagonism and induction of bacterial resistance. Full understanding of their function and potential interactions are, the…

Macrolide antibiotics are largely used in pregnancy for different bacterial infections. Their fetal safety has been studied by several groups, yielding opposing results. In particular, there have been studies claiming an association between macrolides and cardiovascular malformations. Exposure in early infancy has been associated with pyloric stenosis and intussusception. This has led to an avoidance in prescribing macrolides to pregnant women in several Scandinavian countries. The Objectives of the present study was to investigate the fetal safety of this class of drug by linking a large admi…

Three pharmacokinetic/pharmacodynamic parameters--(i) the peak concentration to the minimum inhibitory concentration ratio (C(max)/MIC); (ii … erally have their efficacy linked to T>MIC, and these include the beta-lactams and the conventional macrolides. Antibiotics with weak concentration-dependent effects, but with prolonged persistent effects, such as tetracyclines and azithromycin, have their activity mostly related to the AUC(24)/MIC. By applying these concepts to current antibiotics, and also …
