The Malingering Intussusception.

While intussusception is rarely seen in adults, it is typically obstructive in nature when it does occur. Even less commonly seen is transient intussusception, which occurs without a radiological lead point or any evidence of bowel obstruction. Such findings consist of a "target pattern" seen on computed tomography (CT) but are incidental and do not require any surgical intervention. We report the case of a 31-year-old female who presented to the emergency department with abdominal pain, vomiting, and diarrhea. CT imaging revealed transient intussusception, a benign finding that is not well es

Retrograde amnesia and malingering.

PURPOSE OF REVIEW: Malingered anterograde amnesia is a phenomenon that has been exhaustively studied, whereas research on retrograde amnesia has tended to focus upon functional and organic accounts of impairment. The present review explores studies relevant to extending the malingering paradigm to retrograde amnesia. RECENT FINDINGS: In the period reviewed, very little work has directly addressed the area of malingered retrograde amnesia. Researchers have tended to explain apparent 'anomalies' in memory performance or individual presentation, as manifestations of unconscious or psychological …

Malingering in the Emergency Setting.

Malingering is the intentional fabrication of symptoms for material gain. Malingering among frequent utilizers and patients with psychiatric symptoms is suspected to be common in emergency settings but difficult to detect and manage. We present a case report of a 50-year-old man feigning psychosis and suicidality in order to obtain shelter. Strategies to identify malingered psychiatric symptoms are presented. Understanding how malingering is adaptational can help clinicians begin to manage these patients and symptoms in a compassionate manner that preserves healthcare resources, improves patie…

Malingering in the medical setting.

Malingering of mental illness has been studied extensively; however, malingered medical illness has been examined much less avidly. While in theory any ailment can be fabricated or self-induced, pain--including lower back pain, cervical pain, and fibromyalgia--and cognitive deficits associated with mild head trauma or toxic exposure are feigned most frequently, especially in situations where there are financial incentives to malinger. Structured assessments have been developed to help detect both types of malingering; however, in daily practice, the physician should generally suspect malingeri…

Detection of malingered mental retardation.

In a cross-validation of results from L. O. Graue et al. (2007), standard psychological assessment instruments, as well as tests of neurocog … st manuals or published articles, of the 11 feigning measures administered, only the Test of Memory Malingering (TOMM; T. N. Tombaugh, 1996) retention trial had a specificity rate >.90 in the MR group. However, the 2nd learning trial of the TOMM, as well as a short form of the Digit Memory Test (T. J. Guilmette, K. J. Hart, A. J. Guiliano, & B. E. Leininger, …

Estimated costs of malingered disability.

The feigning of disabling illness for the purpose of disability compensation, or "malingering," is common in Social Security Disability examinations, occurring in 45.8%-59.7% of adult cases. In this study, we estimated the costs of malingering based on mental disorder data published by the Social Security Administration. At the most widely accepted base rate of malingering in medicolegal cases involving external incentive, costs were high, totaling $20.02 billion in 2011 for adult mental disorder claimants. Moreover, these figures clearly underestimate the costs of the larger problem with feig…

A clinical approach to the malingering patient.

Malingering presents special challenges to the practicing clinician, including diagnostic uncertainty, the confrontation of potentially criminal conduct, and countertransference and personal reactions. Maintaining a traditional clinical approach to the patient suspected of malingering enables the therapist to draw analogies to other disorders and utilize customary diagnostic and therapeutic skills. Malingering has long been recognized in the military, and has predictably come to the forefront of clinical practice during a time of war. The art in military medicine is to find a way to make our e…

Evoked potentials in hysteria and malingering.

Evoked potentials are often helpful in confirming the nonorganic nature of sensory symptoms in hysteria and malingering. In a retrospective analysis, such cases were found to comprise approximately 1% of referrals to a university hospital laboratory. However, the diagnostic usefulness of electrophysiologic tests is limited in some clinical settings: normal responses may be encountered in subjects with certain organic deficits, and "abnormal" responses can sometimes be produced voluntarily by normal subjects. In malingering--as opposed to hysteria--the role of the technologist in monitoring pat…

Forced-choice testing provides evidence of malingering.

Assessment of motivation using forced-choice testing is described and illustrated with case reports. Two injured workers with protracted dis … culty levels, is proposed as a specific measure to detect faked poor performance. Identification of malingering reduces the risk of iatrogenic damage. The subjective complaints of malingerers must be viewed with skepticism.

Evaluation of posturography in the detection of malingering subjects.

OBJECTIVE: This study aimed to test the performance of proposed methods for detecting malingering subjects on computerized dynamic posturography using one subject group in three situations (normal, malingering, vestibular weakness). STUDY DESIGN: The study design was a prospective, blinded study. SETTING: The study was conducted at a university hospital. PATIENTS: Volunteer subjects aged 20-59 years of age participated. INTERVENTIONS: Computerized dynamic posturography was performed under three situations: best effort, faking vestibular weakness, and transient induced vestibular weakness w…

How to Approach Malingering in the Forensic Setting.

Malingering, the intentional production or exaggeration of symptoms for external gain, presents a significant diagnostic challenge in forensic psychiatry. Distinguishing malingering from genuine psychiatric illness is essential, as diagnostic errors can delay appropriate treatment, consume limited psychiatric resources, and affect legal proceedings. This article presents three clinical case vignettes illustrating different manifestations of malingering, including feigned psychosis and intellectual disability. Key diagnostic strategies include identifying inconsistencies between reported sympto…

Susceptibility of the trauma symptom inventory to malingering.

This study examined the sensitivity and specificity of the Trauma Symptom Inventory (TSI; Briere, 1995), a self-report measure of psychological sequelae of potentially traumatic events, to malingering. An optimal cutting score for a validity scale--Atypical Responding (ATR)--designed to identify exaggeration or other unusual response sets was developed in an analogue sample of 155 college students and subsequently applied to TSI profiles from several samples of patients with various psychiatric disorders. Use of a cross-validated T-score cutoff of 61 and below on the ATR scale produced good se…

[Granulomatosis with polyangiitis (previously Wegener's granulomatosis) mimicking malingering].

INTRODUCTION: ANCA vasculitis may involve the skin and develop slowly without specific histology, and without autoantibodies. CASE REPORT: We report a 50-year-old woman who experienced bilateral mastectomy because of ulcero-necrotic, non-specific inflammatory cutaneous lesions of the breasts. First considered by others as a malinger patient, she developed oto-neurological lesions leading to the diagnosis of Wegener's granulomatosis. Five years later, specific antibodies of the disease were present. CONCLUSION: Cutaneous involvement by ANCA vasculitis can be isolated for a long time. Physicia

An emotional Stroop effect to malingering-related words.

This study was designed to elicit an emotional Stroop effect in simulators of malingering. Student participants (mostly women) either did or did not feign mild brain trauma. A modified Stroop test was administered, using neutral and "malingering" words. Controls showed no difference on malingering versus neutral words; simulators performed significantly worse on malingering words. Further research must specify the best method of administration to elicit a malingering Stroop effect and assess the additional discriminative power it may provide when combined with other malingering-detection strat…
