Medical records and issues in negligence.

It is very important for the treating doctor to properly document the management of a patient under his care. Medical record keeping has evolved into a science of itself. This will be the only way for the doctor to prove that the treatment was carried out properly. Moreover, it will also be of immense help in the scientific evaluation and review of patient management issues. Medical records form an important part of the management of a patient. It is important for the doctors and medical establishments to properly maintain the records of patients for two important reasons. The first one is tha

Electronic medical records and patient privacy.

Networks of health care providers and payers increasingly rely upon electronic databases to coordinate and analyze medical information. Benefits of linking databases include saving lives and reducing costs. However, access to and use of medical records raise serious privacy concerns. Potential misuse of records may harm patients and undermine the quality of health care. In the clinical setting, stringent security procedures must be instituted, including limited access to electronic records, audit trails, and security audits. Patient consent should be obtained before personal information is dis

Facebook language predicts depression in medical records.

Depression, the most prevalent mental illness, is underdiagnosed and undertreated, highlighting the need to extend the scope of current screening methods. Here, we use language from Facebook posts of consenting individuals to predict depression recorded in electronic medical records. We accessed the history of Facebook statuses posted by 683 patients visiting a large urban academic emergency department, 114 of whom had a diagnosis of depression in their medical records. Using only the language preceding their first documentation of a diagnosis of depression, we could identify depressed patient

Discrepancy between medical conditions self-reported by bus drivers and medical records.

BACKGROUND: Medical incapacity at the wheel is a rare but high-profile factor in accident causation. The UK Driver and Vehicle Licensing Agency (DVLA) does not require a review of medical records on the application for a bus licence, but applicants sign a self-declaration of medical history. There is debate over what, if any, verification of medical information is required for doctors who complete the medical assessment. AIMS: To assess how self-declaration compares against General Practitioner (GP) patient records for a series of bus drivers undergoing driver licensing assessment in a 12-mon

Medical records in litigation: the Dalkon Shield story.

The Dalkon Shield was manufactured by A.H. Robins Inc. in the early and mid seventies, before it was withdrawn from sale because of the influx of lawsuits against the manufacturers. The case has become the largest tort case in history, with approximately 200,000 claimants worldwide and will not be wound up for years to come. Slater and Gordon is an Australian firm of solicitors with offices in three states. They have the largest Dalkon Shield practice in the world and represent almost 3,000 claimants. One of their most difficult tasks in preparing the cases is the gathering of medical evidence

Discovering novel disease comorbidities using electronic medical records.

Increasing reliance on electronic medical records at large medical centers provides unique opportunities to perform population level analyses exploring disease progression and etiology. The massive accumulation of diagnostic, procedure, and laboratory codes in one place has enabled the exploration of co-occurring conditions, their risk factors, and potential prognostic factors. While most of the readily identifiable associations in medical records are (now) well known to the scientific community, there is no doubt many more relationships are still to be uncovered in EMR data. In this paper, we

Predicting burn patient mortality with electronic medical records.

BACKGROUND: Although there exists robust literature on mortality-associated factors in burn patients, it is not known how electronic medical records affect outcomes. Using burn injury as a surgical care model of information and communication, we hypothesized that functionality and interoperability of the electronic medical record could serve as determinants of outcome. METHODS: We used the state inpatient databases for New York, Washington, California, and Florida for the years 2009 and 2010 for all states, with the additional years of 2012 and 2013 for New York (n = 6,002), and the respectiv

Learning bundled care opportunities from electronic medical records.

OBJECTIVE: The traditional fee-for-service approach to healthcare can lead to the management of a patient's conditions in a siloed manner, inducing various negative consequences. It has been recognized that a bundled approach to healthcare - one that manages a collection of health conditions together - may enable greater efficacy and cost savings. However, it is not always evident which sets of conditions should be managed in a bundled manner. In this study, we investigate if a data-driven approach can automatically learn potential bundles. METHODS: We designed a framework to infer health con

Surgeon bias in the medical record.

The medical chart is presumed to be an impartial record of a patient's history, examination, and treatment. This study was designed to determine if hand surgeons introduce selective bias into the medical record. Fifty consecutive new patients were referred to a hand surgery practice for presumed carpal tunnel syndrome. Forty-four patients were diagnosed with carpal tunnel syndrome and were eligible for study inclusion. Prior to evaluation, all patients wrote in free-response fashion and diagrammed their symptom quality and location. Surgeon dictations from these visits were then evaluated agai

Medical Marijuana Documentation Practices in Patient Electronic Health Records: Retrospective Observational Study Using Smart Data Elements and a Review of Medical Records.

BACKGROUND: Medical marijuana (MMJ) is available in Pennsylvania, and participation in the state-regulated program requires patient registration and receiving certification by an approved physician. Currently, no integration of MMJ certification data with health records exists in Pennsylvania that would allow clinicians to rapidly identify patients using MMJ, as exists with other scheduled drugs. This absence of a formal data sharing structure necessitates tools aiding in consistent documentation practices to enable comprehensive patient care. Customized smart data elements (SDEs) were made av

DOME: Directional medical embedding vectors from Electronic Health Records.

MOTIVATION: The increasing availability of Electronic Health Record (EHR) systems has created enormous potential for translational research. Recent developments in representation learning techniques have led to effective large-scale representations of EHR concepts along with knowledge graphs that empower downstream EHR studies. However, most existing methods require training with patient-level data, limiting their abilities to expand the training with multi-institutional EHR data. On the other hand, scalable approaches that only require summary-level data do not incorporate temporal dependenci

Data-driven approach for creating synthetic electronic medical records.

BACKGROUND: New algorithms for disease outbreak detection are being developed to take advantage of full electronic medical records (EMRs) that contain a wealth of patient information. However, due to privacy concerns, even anonymized EMRs cannot be shared among researchers, resulting in great difficulty in comparing the effectiveness of these algorithms. To bridge the gap between novel bio-surveillance algorithms operating on full EMRs and the lack of non-identifiable EMR data, a method for generating complete and synthetic EMRs was developed. METHODS: This paper describes a novel methodology

Electronic medical records and prevention quality: the prevention index.

BACKGROUND: The Prevention Index is a methodology for using electronic medical records to identify and evaluate practice variations in the delivery of preventive care. METHODS: The Prevention Index was used to evaluate the provision of 10 recommended adult preventive services using electronic medical record data for the years 1999 through 2002 among the 450,000 members of a large Northwest integrated care system. The analyses were conducted in 2005. The Prevention Index determines the proportion of person-time that is covered using consensus guidelines as a standard of care. It is analyzed at

Clinical large language model centered on electronic medical records.

In the quest to enhance medical consultation, our study introduces AI4Doctor, a sophisticated large-language model (LLM) tailored for the clinical domain. At the heart of AI4Doctor is an innovative integration strategy that synergizes distilled data extracted from electronic medical records (EMR) with empirical insights gathered from practicing physicians during the supervised fine-tuning. Although existing platforms offer informative responses, they fall short of replicating the nuanced decision-making processes of medical professionals, particularly in complex, integrative diagnostic scenari

National Cancer Grid initiative for electronic medical records, India.

PROBLEM: Inefficient workflows, incomplete data and lack of interoperability can hinder the uptake of electronic records systems, challenges particularly relevant in cancer treatment with its complex longitudinal and multidisciplinary nature. Further, products developed in high-income countries are not designed for compatibility with the workflows of low- and middle-income countries, which face additional issues of cost. APPROACH: We evaluated centres with different resources and geographical locations to develop the requirements of our product. We published an invitation to potential vendors
