Amaurotic mydriasis is characterized by larger than normal pupils in patients with visual loss. In sought to establish whether amaurotic mydriasis can reliably identify different kinds of visual loss and whether this static measurement might prove useful in discerning "balanced" bilateral optic neuropathies where no relative afferent pupillary defect (RAPD) is detected. Patients with binocular pregeniculate visual loss, patients with balanced binocular pregeniculate loss without RAPD, and patients with monocular pregeniculate visual loss had significantly larger pupils than age-matched control…

UNLABELLED: UNEQUAL PUPILS: Or anisocoria is frequent and can be physiological, benign or life threatening. VARIATIONS IN THE PUPIL: Constri … . Other particular cases exist, such as Adie's pupil, affecting young women (1/20,000), associating mydriasis and occasionally tendinous areflexia. Diagnosis is generally confirmed by the ophthalmologist, following a normal or evocative of the disease neurological examination.
IN PRACTICE: Rapid but careful clinical examination permits orientation of the d …

A 73-year-old white woman with bilateral congenital mydriasis had no other obvious abnormalities. The pupils reacted almost imperceptibly to light. There was no detectable reaction to accommodation and convergence. Pupillary response to pilocarpine 4% solution indicated the presence of the sphincter muscle. However, the failure of the pupil to react to a potent cholinesterase inhibitor (demecarium bromide 0.25%) suggested an abnormality relative to acetylcholine production in the iris. Rapid and marked pupillary response to phenylephrine 10% solution indicated the presence of the dilator muscl…

AIM: To compare mydriasis before the cataract surgery after the local application of mydriatics in to the conjunctival sac with mydriasis achieved using the mydriatic solution in soaked piece of cellulose applied in to the lower fornix. The same mydriatics (tropicamide 0.5 %, phenylephrine 10%, and homatropine 4 %) were used.
METHODS: In a prospective, non-randomized study the authors measured maximal artificial mydriasis in 100 patients (60 women, 40 men) in 100 eyes before the cataract surgery. The mean age of the patients was 70.17 years (range, 42 - 91 years). The patients were divided in…

Hydroxyamphetamine eyedrops are used to help localize the lesion in Horner's syndrome. Because normal variability in the response to the eye … effect of hydroxyamphetamine was symmetric in each pair of eyes. The mean interocular asymmetry of mydriasis as measured by the difference in dilation (right eye dilation minus left eye dilation) was -0.087 mm (+/- 0.29 S.D.). Thus, the variability of hydroxyamphetamine mydriasis from one eye to the other in a single subject was much lower than the variabi …

PURPOSE: To report a case of transient anisocoria and mydriasis in a 14 year old boy with toxoplasmosis retinochoroiditis.
OBSERVATION: The patient presented with panuveitis and mydriasis which persisted for 18 days and spontaneously resolved.
CONCLUSIONS AND IMPORTANCE: Mydriasis is a rare potential neurological manifestation of toxoplasmosis retinochoroiditis. Clinicians should be aware of this rare cause of anisocoria.

We studied hydroxyamphetamine hydrobromide 1% (Paredrine) mydriasis in 54 patients with Horner's syndrome to determine its effectiveness in distinguishing preganglionic lesions from postganglionic lesions. The difference in pupillary dilation between the unaffected and affected sides was used as a measure of the hydroxyamphetamine effect. We found that patients who had clinical evidence of damage to the postganglionic neuron of the oculosympathetic pathway had less pupillary dilation on the affected side. In contrast, almost all patients judged to have clinical evidence of preganglionic lesion…

Atropine derivatives. The presence of atropine derivatives in numerous combined medicines is often ignored or reflected. A case report of prolonged bilateral non reactive mydriasis with "cycloplegia", linked with the utilisation of a therapeutic dose of Lameline suppositories raises the question of individual susceptibility, with particular ocular sensitivity. This urge one not to ignore the presence of these products and to respect the contraindications, even with minute doses.

Although scopolamine is rarely intentionally applied directly to the eye, accidental contamination can occur after touching or handling a transdermal scopolamine patch, worn behind the ear to prevent motion sickness, and then rubbing one's eyes or handling contact lenses. An acute episode of prolonged cycloplegia with no admission of use of any medication by the patient was only resolved after direct questioning as to the use of a scopolamine patch revealed its removal within a 36 hour period.

Seven patients (all female, ranging in age from 5 to 53 years) had typical migraine and episodic unilateral mydriasis. These mydriatic episodes, usually occurring during the headache, lasted from 15 minutes to 24 hours and had occurred for three months to 35 years. Three patients had suffered from car sickness as children and there was a family history of migraine in six. Normal accommodation in five patients, normal responses to pilocarpine in two patients, and a return to normal or near-normal pupillary reaction in five patients with the cessation of headache made pharmacologic blockade unli…

PURPOSE: To compare the mydriatic efficacy and cardiovascular safety of the mydriatic options.
METHODS: This was a prospective randomized analysis of 90 eyes submitted to senile cataract surgery. Group A (30 eyes) included topical mydriasis, group B (30 eyes) included mydriasis with the ophthalmic insert (Mydriasert), and group C (30 eyes) included intracameral mydriasis. Measures were mydriasis value before ophthalmic viscoelastic device (OVD) injection and at the end of surgery, blood pressure and heart rate, need for antihypertensive medication, and subjective iris stability.
RESULTS: Bef…

Lidocaine (a sodium channel blocker) serves as an antiepilepsy drug in neonates. We present a neonate with a seizure disorder who acutely developed pupillary mydriasis secondary to lidocaine overdose. We suggest adding this side effect to the list of untoward effects of lidocaine and to the differential diagnosis of fixed dilated pupils in neonates treated with lidocaine.

In the literature congenital mydriasis is described as a very rare condition and explained as a result of isolated aplasia of the iris sphincter muscle. Aplasia of the ciliary muscle was assumed to cause congenital accommodation insufficiency. A case of congenital mydriasis with lack of accommodation is presented. The first ophthalmological check-up was 2 weeks after surgery for a persistent ductus arteriosus Botalli. The girl was 15 weeks old. Her parents had watched her dilated pupils since birth. The diameter of both pupils was 6.5 mm. They did not react to light, lid closure, or conjunctiv…

PURPOSE: To describe the clinical features of the syndrome of neurologically isolated episodic unilateral mydriasis.
METHODS: Information was obtained by reviewing the case records of 24 patients evaluated by the author and collected by polling other neuro-ophthalmologists.
RESULTS: Nineteen patients were women. The median age was 31 years. Fourteen patients had migraines. The median interval between onset of events and evaluation was 6 months. The median duration and frequency of events were 12 hours and two to three per month, respectively. Most patients did not identify any factor that co…
