Nipple discharge.

By performing a thoughtful evaluation including a detailed history and careful physical examination, appropriate studies can be selected to allow the practitioner to arrive at the correct diagnosis of nipple discharge in a nonmorbid, expeditious, and inexpensive manner. This article has presented a simple, cost-effective, minimally morbid algorithm for the evaluation of nipple discharge.

Nipple discharge.

The evaluation and management of nipple discharge can be undertaken with minimal difficulty by performing a careful history and examination and following a logical thought process in linking the type of discharge with the suitable mode of treatment. In this manner, the patient's discharge can be efficiently and thoroughly evaluated.

[Nipple discharge].

The Authors examine the various types of breast discharge concentrating in particular on the secretions due to inherent pathology. After having studied origin, they concentrate on the diagnostic significance and the limits of exfoliative cytology and contrast mammography. The Authors conclude by presenting an original protocol of treatment of the afflicted breast illustrating in addition, the various surgical techniques proposed for the cure of the sicknesses of intramammary origin that cause abnormal discharge.

Nipple discharge.

A milky discharge may be of a benign endocrine nature, drug induced or secondary to a pituitary lesion. A clear or bloody discharge may be functional or related to benign or malignant breast lesions. Carcinoma is a very unusual cause of nipple discharge in the absence of a palpable mass or an abnormal mammogram. A unilateral bloody discharge without evidence of a tumor may stem from a benign intraductal papilloma. Careful follow-up evaluations are sufficient for a persistent nipple discharge, after serious pituitary and breast disorders have been ruled out.

Bloody nipple discharge.

Bloody nipple discharge is a complaint that worries both the patient and the physician. Unlike other types of nipple discharge, it signals intrinsic breast disease. A benign etiology is the rule rather than the exception, but this symptom warrants careful evaluation. If there is no palpable mass or mammographic abnormality, bloody discharge from a single duct in a nonpregnant young woman is usually due to a benign intraductal papilloma.

Nipple discharge screening.

Nipple discharge is a common complaint among women. It is classified as normal or abnormal depending on features such as laterality, cycle variation, quantity, color or presentation (i.e., induced vs spontaneous). It can be related to benign conditions, such as intraductal papilloma, duct ectasia, plasma cell mastitis or galactorrhea; or to malignant conditions such as ductal, lobular or papillary carcinoma. Techniques used in nipple discharge evaluation include mammography, ultrasound, cytology (which could be assisted by a mammary pump), duct endoscopy, ductography, immunochemical methods an…

Management of Nipple Discharge.

Nipple discharge is the third most common breast-related complaint but is rarely the presenting symptom of breast cancer. Distinguishing patients with physiologic versus pathologic nipple discharge, and treating the later according to the underlying pathologic condition is of utmost importance. Nipple discharge is categorized as lactational, physiologic, or pathologic. Physiologic nipple discharge (galactorrhea) is typically caused by hyperprolactinemia due to medications (ie, antipsychotics), pituitary tumors, and endocrine disorders. When a suspicious radiologic lesion is identified, patholo…

Management of nipple discharge.

In a series of 9,312 women who consulted one of the authors with a complaint of disease of the breast between 1959 and 1991, nipple discharge was the presenting symptom in 448 (4.8 percent). Nipple discharge was spontaneous in 243 (2.6 percent) and provoked in 205 (2.2 percent) of the patients. The ages of the patients ranged from 13 to 75 years (mean of 42.5 years) in the spontaneous and 16 to 70 years (mean of 37.8 years) in the provoked discharge group. When a palpable mass was found, biopsy was undertaken, while in instances of nipple discharge only, subareolar exploration was performed. O…

Nipple discharge: surgical significance.

Nipple discharge is an important clinical entity ranking second only to a lump as the most common complaint among 7,588 patients having breast surgery (560/7,588, or 7.4%). In the office and clinic it is even more common, since many patients can be treated medically and do not require an operation. To be significant, a discharge should be true, spontaneous, persistent, and nonlactational. Nipple discharge can be milky, multicolored and sticky, purulent, clear (watery), yellow (serous), pink (serosanguineous), or bloody (sanguineous). Watery, serous, serosanguineous, and sanguineous discharges …

Epidemiology of nipple discharge.

The Authors report their personal observations of nipple discharge in gynecologic patients admitted to the Obstetric and Gynecologic Department of Padua University or applying to the Senologic Diagnostic- or Endocrinology-Departments of the same. After dividing breast secretions into two groups - the neuroendocrine and the strictly mammary ones - the Authors discuss their etiopathogenesis, incidence, and clinical significance (mainly as to their possible association to breast malignancies), and present their personal results. At the end, a careful examination of all nipple discharges is recomm…

Bloody nipple discharge in infants.

Though milky nipple discharge is frequently seen in neonates, blood stained discharge from the nipple is an exceptionally rare phenomenon. We noted a case of a three-month-old baby girl who presented with bilateral blood stained nipple discharge without signs of inflammation; engorgement or hypertrophy and which subsided without any intervention. This case is reported along with literature review about managing this rare condition.

Nipple discharge from the breast.

Abnormal nipple discharge is rare, constituting only 3-5% of mammary consultation. In the present paper 267 patients with primary nipple discharge operated on at Institut Gustave-Roussay (IGR) in Villejuif, France, between January 1, 1960 and December, 1974 were evaluated. In all cases, the nipple discharge was symptomatic, spontaneous, and represented the primary reason for the patient's consultation. During the same period 1,145 cases of nipple discharge were treated at IGR. Of the 1,145 cases with symptomatic nipple discharge, 267 patients (23%) required surgical intervention. Among these, …

[Bloody nipple discharge: duct ectasia].

Bloody discharge from the nipple (thelorrhagia) is a rare symptom in childhood and is a cause of great concern due to the mistaken association with carcinoma in adults. However, in children thelorrhagia is due to benign mammary duct ectasia in the majority of cases. Up to date, only isolated case reports or case series have been published and, although different theories have been postulated, no definite etiology has been identified. We report two cases with similar clinical features and outcome, in whom bleeding resolved spontaneously 4 and 6 weeks after onset. We present a review of the lite

Surgical management of nipple discharge.

PURPOSE: Nipple discharge is reported in 2.5-3% of women with breast carcinoma. Breast carcinoma is found in approximately 8% of surgically treated patients presenting with bloody nipple discharge (ND). METHODS: In the present study 110 women with ND as a presenting symptom were examined. The discharge was spontaneous in 76 and elicited in 11 patients. RESULTS: After surgical intervention benign breast disease was found in 85% of patients. Thirteen women (15%) were found to have malignancy and underwent additional surgery. Cytology of the discharge was positive or suspicious for malignancy i…

[Diagnostic management of nipple discharge].

Nipple discharge may be a symptom of a variety of diseases. The importance of nipple discharge is its possible association with breast carcinoma. Not only the precise clinical evaluation but also particularly imaging evaluation by mammography, ultrasound, galactography or MR-galactography is necessary to establish the exact diagnosis and plan proper treatment. Depending on the underlying pathology--most commonly intraductal papilloma, fibrocystic changes or breast carcinoma--the optimal therapeutic regimen, such as single lactiferous duct excision on the one end and modified radical mastectomy…

[Nipple discharge: differential diagnostic considerations].

Each uni- or bilateral secretion of the nipple--spontaneous or easy to trigger--has to be examined thoroughly, independent of the colour or the amount of the secretion. Colours of the secretions do not refer to a specific breast disease. Main causes are functional disease with and without endogenous increase of prolactin secretion and benign and malignant breast diseases. Mastopathia, papilloma, fibroadenoma, nonpuerperal mastitis, and carcinoma of the breast are the most common pathological findings associated with secretions of the mammary gland.

The diagnostic value of nipple discharge cytology: breast imaging complements predictive value of nipple discharge cytology.

BACKGROUND: Papilloma is the most common finding associated with pathologic nipple discharge. In the absence of breast imaging abnormalities, the incidence of occult malignancy is <3%. OBJECTIVE: To determine the predictive value of nipple discharge cytology in conjunction with breast imaging. METHODS: Retrospective review of 160 charts; inclusion criteria of clinically pathologic nipple discharge, subsequent excisional biopsy, and absence of palpable abnormalities. Nipple discharge cytology categorized as negative, atypical, suspicious, and papillary. Breast imaging was analyzed. Preoperati…
