[Nutritional support in haemodialysis].

Malnutrition is a common severe problem in dialysis. The prevalence of malnutrition has been estimated as between 30% and 70%. Although malnutrition is not normally listed among the most frequent causes of mortality in these patients, it contributes to cardiovascular mortality through the MIA syndrome (Malnutrition, inflammation and arteriosclerosis) and the severity of infections. The cause of malnutrition in dialysis is frequently due to a multiplicity of factors. The use of the term malnutrition has been criticized when the cause is not scant dietary intake, for practical reasons: if the ca

Assessment of utilization of nutritional support scheme vis a vis nutritional status of tuberculosis patients on nutritional support scheme.

INTRODUCTION: Tuberculosis and undernutrition is a well-known association and form a vicious cycle. A nutritional support scheme under the name of NIKSHAY POSHAN YOJANA (NPY) was launched in April 2018 where a direct bank transfer of INR 500 was made to the patients enrolled in the NIKSHAY portal monthly to support the nutritional needs of the patients. The study was undertaken to study the implementation and utilization of the benefit among tuberculosis patients. METHODS: A total of 1000 patients were enrolled in the study who have completed their treatment under the Tuberculosis unit of Rao…

Enteral Access for Nutritional Support.

There are a variety of methods for obtaining enteral access for patients requiring short- or long-term enteral feeding. This article reviews indications, contraindications, and techniques for placement of nasoenteric access, percutaneous endoscopic gastrostomy, open and laparoscopic gastrostomy and jejunostomy, and gastrojejunostomy. The diagnosis and management of complications from these procedures is also discussed as are pearls and pitfalls for each technique.

Nutritional Support in Pancreatic Diseases.

This review summarizes the main pancreatic diseases from a nutritional approach. Nutrition is a cornerstone of pancreatic disease and is sometimes undervalued. An early identification of malnutrition is the first step in maintaining an adequate nutritional status in acute pancreatitis, chronic pancreatitis and pancreatic cancer. Following a proper diet is a pillar in the treatment of pancreatic diseases and, often, nutritional counseling becomes essential. In addition, some patients will require oral nutritional supplements and fat-soluble vitamins to combat certain deficiencies. Other patient

Nutritional Support in Esophagogastric Cancers.

Esophageal and gastric cancers are common malignancies, both in the United States and worldwide, that carry significant morbidity and mortal … y, or parentally. In patients who do not undergo surgery but receive chemotherapy and/or radiation, nutritional support is also an important aspect of the multidisciplinary care approach.

Nutritional support in acute pancreatitis.

Acute pancreatitis (AP), mainly the severe necrotizing type, results in extreme energy demands which might lead, if prolonged, to severe malnutrition. Besides that, starving during AP contributes to gut barrier dysfunction, the main cause of bacterial translocation and sepsis. The aim of nutritional support in AP is to prevent malnutrition and protect the gut by maintaining mucosal integrity. Traditionally, nutritional support during the acute phase of the disease has been provided through total parenteral nutrition (TPN) solutions. However, recent animal and human studies have identified new …

Nutritional support in liver disease.

Protein-energy malnutrition is highly prevalent in patients with chronic liver disease, especially in those with advanced cirrhosis, and seems to influence their survival. Enteral and parenteral approaches to the treatment of alcoholic hepatitis have yielded controversial results. The use of polyunsaturated lecithin might be a promising alternative for the future. Enteral nutrition is the preferred way of providing artificial nutrition to patients with advanced cirrhosis. In some trials, it appears to improve the short-term survival of these patients. The long-term use of oral nutritional supp

Nutritional support by tube jejunostomy.

Access to the jejunum by orogastric intubation or insertion of a surgical tube provides a method for nutrient infusion in clinical conditions that involve abnormal upper gastrointestinal function. Despite the physiologically extraordinary nature of jejunostomy intubation-with enteral infusions beyond the stomach, biliary tract, pancreas, and duodenum--the current view is that jejunal feeding is an acceptable alternative to parenteral nutritional support. Because jejunostomies utilize the gastrointestinal tract, administering feedings by this route can minimize some of the major adverse effects…

Nutritional support for acute pancreatitis.

The current review has summarized current data relevant to the nutritional support of patients with acute pancreatitis. Selection of the most appropriate form of nutritional support for patients with acute pancreatitis is intimately linked to a thorough understanding of the effects of various forms of enteral and parenteral nutrition on physiologic exocrine secretory mechanisms. Two basic concepts have emerged from the multiple studies that have addressed these issues to date: 1, enteral feeds should have low fat composition and be delivered distal to the ligament of Treitz to minimize exocrin…

[Nutritional support in laryngectomized patients].

Pathologies involving tumours of the neck are an important group of neoplasias. The tumours themselves are often the cause of malnutrition, and this may be aggravated in these patients by other factors such as: excess of alcohol and cigarettes, anatomical location of the tumour next to the digestive tract and the application of cyto-reducing treatment applied previously. The main purpose of the present study is to analyze the possible effect of hospital diets during the postoperative period in laryngectomized patients, on the development of malnutrition. For this purpose 49 patients were studi
