Cryopreservation of parathyroid glands.

The risk of permanent hypoparathyroidism following thyroid and parathyroid surgery is around 1% in the hands of experienced endocrine surgeons. Although this complication is rare, rendering a patient permanently aparathyroid has significant consequences on the health and quality of life of the patient. Immediate autotransplantation of parathyroid glands that are injured or unintentionally removed offers the best possibility of graft viability and functionality. However, since the majority of cases of hypoparathyroidism are transient, immediate autotransplantation can complicate postoperative s…

Multiple ectopic parathyroid glands.

Parathyroid surgery to correct primary hyperparathyroidism is successful in 80 to 97 per cent of initial explorations. Failures are often linked to inability to locate ectopic parathyroid glands. Although ectopic parathyroid glands are relatively common (15%) multiple ectopic glands are rarely reported. We describe a case of multiple ectopic parathyroid glands and the intraoperative approach to their localization and review the anatomy and embryology of ectopic parathyroid glands. A 39-year-old woman presented with fatigue, lethargy, and depression. On biochemical evaluation she was noted to b…

Pathologic intrathyroidal parathyroid glands.

BACKGROUND: Parathyroid glands originate from the third and fourth branchial pouches and migrate caudally to their final positions. Aberrations during migration result in anomalous locations. Intrathyroidal location is not common. METHODS: We reviewed cervical explorations performed from 1974 to 1993 in hyperparathyroidism patients. RESULTS: We found pathological intrathyroidal glands in six patients. Three patients had adenomas (left superior, left inferior and right inferior glands). The hyperplastic glands were left inferior in one patient and right inferior in the remaining two. Intraope…

Hyperparathyroidism: ectopic parathyroid glands.

A patient with a parathyroid adenoma located beneath the throid capsule and within a benign thyroid adenoma is reported on. To the best of our knowledge, this lesion has not previously been reported. This case points out the difficulties that may be encountered when it is necessary to locate a missing parathyroid gland in a patient with multinocular goiter. In this clinical setting there may be a role for use of the various technics recommended for preoperative localization of a parathyroid adenoma.

Retropharyngeal Parathyroid Glands: Important Differences.

INTRODUCTION: In primary hyperparathyroidism (PHPT), parathyroid ectopia is seen in up to 22% leading to more difficult surgery. We aimed to determine the rate and characteristics of retropharyngeal (RP) parathyroid glands. METHODS: A prospective database was queried for patients with sporadic PHPT who had surgery from 1997 to 2016. The data of RP patients were compared to those who had surgery for sporadic PHPT over the same time period with hyperfunctioning parathyroids in anatomically normal positions (N). RESULTS: RP glands occurred in 47/3006 (1.6%) patients and were more common at reop…

[Inflammation of the parathyroid glands].

Inflammation of the parathyroid glands is rare when compared to other endocrine organs. This leads to the use of descriptive terms as well as the lack of a generally accepted classification for inflammatory disorders of the parathyroid glands. This review article proposes that parathyroid inflammation be subdivided morphologically into (a) non-specific lymphocytic infiltration, which is more an expression of damage to small vessels, due to e. g. severe systemic inflammation or myocardial infarction, (b) autoimmunogenic lymphocytic parathyroiditis, (c) nonimmunogenic inflammation caused by gran…

Disorders of the parathyroid glands.

The three calcitropic hormones, parathyroid hormone (PTH), 1,25-dihydroxycholecalciferol and calcitonin are together responsible for calcium homeostasis in the mammal. Feline PTH is an 84 amino acid, single chain polypeptide with a molecular weight of 9449, which is secreted by the parathyroid glands. The principle secretagogue for PTH is a low plasma ionised calcium concentration, although both 1,25-dihydroxycholecalciferol and phosphate have significant roles in regulating PTH secretion. The ability to accurately measure circulating PTH in the cat has simplified the evaluation of disorders o…

Immunocytochemical localization of parathyroid hormone in rabbit parathyroid glands.

Immunocytochemical localization of parathyroid hormone was examined in the rabbit parathyroid gland by means of protein A-gold technique. Protein A-gold particles were observed on the secretory granules and the large secretory granules thought to be storage granules. No protein A-gold particles were observed on cisternae of the endoplasmic reticulum and the Golgi apparatus.

Intermediate-filament proteins in parathyroid glands and parathyroid adenomas.

The intermediate-filament proteins of normal, hyperplastic, and adenomatous parathyroid glands were analyzed immunohistochemically and by immunoblotting with monospecific antibodies. In both normal and adenomatous parathyroid glands, we found keratins with molecular weights of 52, 45, and 40 kilodaltons (Nos. 8, 18, and 19, respectively). Vimentin proteins could be identified only in stromal cells, while glial fibrillary acidic protein was not found. In normal parathyroid glands, neurofilament positivity was seen only in nerve axons. In five of 15 parathyroid gland adenomas some keratin-positi…

Imaging of the parathyroid glands.

Parathyroid imaging is performed in the setting of primary hyperparathyroidism with intent to localize adenomatous parathyroid tissue before minimally invasive surgery. Available imaging options currently consist of ultrasound, radionuclide scanning, magnetic resonance imaging, and both conventional and "4 D" computed tomography. Various combinations of these modalities are used based primarily on institutional bias, and although highly successful in localizing the common single adenoma, they are less effective when there is multiglandular disease or glandular ectopia.

Lipohyperplasia of the parathyroid glands.

Parathyroid lipohyperplasia is an extremely rare cause of hyperparathyroidism. According to a MEDLINE search, there have been only 9 previously reported cases of parathyroid lipohyperplasia in the English-language literature. We report a new case of parathyroid lipohyperplasia, with a brief discussion of this rare disease process.

[Ultrasonographic diagnosis of parathyroid glands].

Enlarged parathyroid glands are, if their localization is typical behind the thyroid gland, during basic USG examination with 5 MHz to 10 MHz using a linear probe, hypoechogenic or anechogenic, larger than 0.5 cm, oval in shape, elongated in a cranoiocaudal direction, separated from the thyroid gland. During swallowing they move upwards. Such a picture may be simulated also by enlarged lymph nodes, sympathetic ganglia a cranked vessel or oesophagus. By using coloured Doppler sonography in the algorithm of examinations the percentage of falsely positive findings may decline. Between 90 degrees …

Imaging of the parathyroid glands.

Primary hyperparathyroidism resulting in hypercalcemia is the most common presentation of parathyroid pathology and usually is related to a parathyroid adenoma and, less commonly, to hyperplasia. Imaging of the parathyroid glands focuses on the detection of adenomas in patients with primary hyperparathyroidism. The role of imaging (as well as the modality used) for preoperative localization of the parathyroid glands continues to be controversial. The embryology, anatomy, and physiology of the parathyroid glands are reviewed. The diagnostic utility of radiological imaging-including ultrasonogra…
