OBJECTIVE: Penile erection following epidural anesthesia is an infrequent occurrence of unclear etioogy. It is very troublesome to perform the procedure during penile erection and attempting it leads to various complications. Three such cases are reported herein. The literature on the pathophysiology and management of intraoperative penile erection is discussed.

Penile erection is mediated by the corpora cavernosa, a trabecular-like vascular bed that enlarges upon vasodilation, but its regulation is not completely understood. Here, we show that perivascular fibroblasts in the corpora cavernosa support vasodilation by reducing norepinephrine availability. The effect on penile blood flow depends on the number of fibroblasts, which is regulated by erectile activity. An extensive review of the literature suggests that priapism or prolonged penile erection is extremely rare in the newborn. The effect of perineal nerve stimulation on penile erection was studied in ten dogs. Through a paraanal incision, the nerve was exposed in the ischiorectal fossa and a bipolar electrode was applied to it. A radiofrequency receiver was implanted subcutaneously in the abdomen. Upon perineal nerve stimulation, the corporeal pressure and EMG activity of the bulbo- and ischiocavernosus muscles increased; penile erection occurred. In an effort to determine if artificial penile erection has any harmful effect in children intracorporeal pressures were measured in 12 boys (mean age 18 months) with mid shaft hypospadias. Using a solid state intracompartmental pressure monitoring system, intracorporeal pressures were obtained during full penile erection and after removal of the tourniquet at detumescence. Penile erection (PE) is a hemodynamic event that results from a neuroendocrine process, and it is influenced by the cardiovascular status of the patient. However, it may also modulate an individual's cardiovascular events. The present study provides the mechanisms involved in the association of PE and cardiovascular function.
