[Pneumonia, pneumocystis pneumonia].

Pneumonia (almost equally bacterial) is the most common and serious adverse events of the patients with rheumatoid arthritis (RA) prescribing anti-tumor necrosis factor(TNF) agents. Furthermore, it has been shown that Pneumocystis jirovecii should be one of the important causal microorganisms by the results of postparketing all-patients registration surveillance in Japan. Older age, pulmonary comorbidities, diabetes mellitus, or dosage of co -prescribing glucocorticoid has been extracted as a predictable risk factor respectively and synergistically by the surveillance. When prescribing anti-TN…

[Viral pneumonia. COVID-19 pneumonia].

Viruses are becoming more and more important as etiological agents of pneumonia, mainly due to improvements in diagnostic techniques. At present, they account for approximately one-third of community-acquired pneumonia cases. They tend to occur in children and elderly patients, causing anything from mild cases to severe cases which require intubation and intensive care. The main causative agents include rhinovirus, influenza virus A or B, metapneumovirus, respiratory syncytial virus, parainfluenza virus, coronavirus, or adenovirus. Infection produced by SARS-CoV-2, the cause of the current COV…

When is pneumonia not pneumonia?

A 34-year-old man was admitted to hospital via the accident and emergency department with severe right-sided abdominal pain and raised inflammatory markers. His pain settled with analgaesia and he was discharged with a course of oral co-amoxiclav. He was readmitted to the hospital 7 days later reporting cough and shortness of breath. His chest X-ray showed a raised right hemi-diaphragm, presumed consolidation and a right-sided effusion. As a result, he was treated for pneumonia. Despite antibiotic therapy his C reactive protein remained elevated, prompting an attempt at ultrasound-guided drain…

[Necrotizing pneumonia due to Streptococcus pneumonia].

We present a patient with a necrotizing pneumonia due to Streptococcus pneumoniae. This complication often affects children and is relatively rare and unrecognized in adults. Its diagnosis is often difficult on chest-X-ray and need a chest computed tomography. No risk factors predisposing to necrosis are described in literature and its mortality is not different from pneumonia without necrosis. The serotype 3 is the most common type implicated in pneumococcal necrotizing pneumonia. Study of virulence factors of S. pneumoniae and various genetic polymorphisms of the host should allow a better u…

Varicella pneumonia: another 'steroid responsive' pneumonia?

Varicella-zoster virus (VZV) pneumonitis remains an often-fatal complication of VZV infection. Antiviral agents and supportive care are widely accepted therapies. Cautious use of corticosteroids in life-threatening VZV pneumonitis may be justified. Appropriate patient selection factors are as yet unidentified and the decision to commence corticosteroid therapy in this setting is clinical.

Cryptococcal pneumonia simulating chronic eosinophilic pneumonia.

We report two cases of cryptococcal pneumonia in which the roentgenographic picture closely mimicked that for chronic eosinophilic pneumonia. Caution in use of corticosteriods based on this roentgenographic pattern is stressed.

[Comparison of clinical presentation of mixed pneumonia with Chlamydia pneumoniae and Streptococcus pneumoniae and S. pneumoniae pneumonia].

Chlamydia pneumoniae is a significant cause of both lower and upper acute respiratory illnesses, including community-acquired pneumonia. Furthermore, C. pneumoniae has been reported to frequently cause pneumonia in association with other respiratory pathogens, mainly Streptococcus pneumoniae. In this study, we investigated the clinical presentation of mixed pneumonia with Chlamydia pneumoniae and S. pneumoniae and compared it with S. pneumoniae pneumonia. A total of 13 cases of mixed pneumonia and 58 cases of S. pneumoniae pneumonia identified at Kawasaki Medical School and related hospitals b…

Pneumonia.

Pneumonia is the leading cause of mortality and a common cause of morbidity in children below five years of age. Commonly, pneumonia is caused by bacterial agents. The diagnosis of pneumonia is usually made on clinical features. A child with tachypnea with no chest in-drawing or difficulty in feeding is labeled as pneumonia. Presence of chest in-drawing, difficulty in speech, feeding or cyanosis classifies a child as suffering from severe or very severe pneumonia. Factors that may help in selection of appropriate antibiotics include: knowledge of etiological agents, sensitivity of pathogens to…

Comparative analysis of chlamydia pneumoniae pneumonia and Mycoplasma pneumoniae pneumonia in children.

BACKGROUND: Chlamydia pneumoniae pneumonia (CPP) in children often presents with mild clinical manifestations, leading to less clinical attention. This study aimed to compare the clinical features of Mycoplasma pneumoniae pneumonia (MPP) and CPP in pediatric patients and to identify risk factors for lobar involvement in CPP. METHODS: We conducted a retrospective analysis of 145 children with CPP and 145 contemporaneously hospitalized children with MPP. Clinical characteristics were compared between the two groups. Patients with CPP were further stratified into lobar pneumonia and bronchopneum…

[Clinical comparison of Chlamydia pneumoniae pneumonia, ornithosis, and Mycoplasma pneumoniae pneumonia].

To elucidate the differences between the clinical aspects of Chlamydia pneumoniae (C. pn) pneumonia and those of two other atypical pneumonias, Chlamydia psittaci (C. ps) pneumonia and Mycoplasma pneumoniae (M. pn) pneumonia, we analyzed the symptoms and laboratory data on the cases of these three types of pneumonia: 46 cases of C. pn pneumonia, 39 cases of C. ps pneumonia, and 131 cases of M. pn pneumonia. C. pn pneumonia was significantly more frequent among the elderly (mean 70 +/- 16 years, p < 0.01) and patients were significantly more likely to be male (76%, p < 0.05). A white blood cell…

Pneumonia.

Pneumonia is commonly encountered in both outpatient and inpatient settings and continues to pose diagnostic and therapeutic challenges for pediatricians. Although bacterial, viral, and fungal pathogens can all cause pneumonia, the etiology can vary depending on the child's age, underlying chronic medical disease, and other risk factors and predictors. However, the epidemiology of pneumonia continues to be evolving given newly emerging viral etiologies and available vaccinations. The diagnosis of uncomplicated, community-acquired pneumonia continues to be based on patient history, physical exa…

Daptomycin-Induced Eosinophilic Pneumonia Mimicking Multifocal Pneumonia.

A 71-year-old female presented to the emergency department with worsening dyspnea, dry cough, malaise, weight loss, fever, chills, and diaph … t X-ray revealed new patchy bilateral interstitial and airspace opacities concerning for multifocal pneumonia. Labs were pertinent for mild peripheral eosinophilia. CT chest revealed moderate diffuse ground glass opacities involving both lungs, with subpleural predominance and some areas of septal thickening seen as well. Daptomycin-induced pneumonitis was …

[Cryptogenic organizing pneumonia versus secondary organizing pneumonia].

Organizing pneumonia (OP) describes a histological pattern of acute or subacute lung damage. Clinically, patients present with cough, fever, and dyspnea. A distinction is made between idiopathic or cryptogenic organizing pneumonia (COP) and secondary organizing pneumonia (OP). In COP, neither clinical/radiological nor histological causes can be determined. It is classified as an interstitial idiopathic pneumonia (IIP) according to the criteria of the American Thoracic Society (ATS) and the European Respiratory Society (ERS). Secondary organizing pneumonia has a known triggering mechanism, such…

Chlamydia pneumoniae pneumonia.

Chlamydia pneumoniae is a common cause of community-acquired pneumonia. At present there is no "gold'' standard for diagnosis and there is no easily accessible means of rapid diagnosis available. The best indication of acute C. pneumoniae infection is a fourfold rise in antibody titer, accompanying a positive polymerase chain reaction or culture. C. pneumoniae is usually associated with nonsevere clinical manifestations but the features will vary depending upon the occurrence as primary or reinfection syndrome, the presence of co-pathogens, or the existence of co-morbid conditions. C. pneumoni…
