Presbyopia.

The need for bifocals causes more angst, fear, depression, denial, searching for alternatives, and needless self-pity than any other benign intervention in all of medicine. To many, this is the first sign they are on the other side of the hill of life. I will try to explain, with a minimum of numbers and equations, what presbyopia is and how it is managed (and perhaps even cured in the near future) in an understandable way. I will also present a few clinical situations which the primary care physician often meets. I suspect many of you are already wearing bifocals or are delaying the acquisiti…

Prandial presbyopia.

Loss of accommodation amplitude during eating (prandial presbyopia) is a rare phenomenon that has been reported in only 1 patient who had had head trauma. We report 2 patients who had not had head trauma and whose accommodative amplitudes, measured by dynamic retinoscopy, became markedly diminished within 1 minute of starting a meal and did not recover for 55-60 minutes. Apart from this abnormality, there appeared to be no autonomic or other neurologic dysfunction. The cause of this isolated disturbance in these patients is a mystery.

[Presbyopia surgery].

Presbyopia surgery is a compensation option which aims at creating an increased depth of field. Monovision preserves a good close-vision quality for myopes. Multifocality is well tolerated by hyperopes if it is compatible with good far-vision quality. Conductive keratoplasty and Lasik multifocal ablation patterns are newly emerging methods. Monovision and multifocality can be applied to surgery by using corneal or intraocular implantation techniques. Multifocal intraocular lenses (IOLs) are growing in popularity among patients and surgeons, and opened the way to refractive lens exchange. Still…

[Presbyopia treatment].

Presbyopia, i.e. the loss of accommodative amplitude with age, results from increasing stiffness and thickness of the lens and leads to inability of the lens to change shape, refractive power and focus of the eye in response to the contraction of the ciliary muscle inside the eye. Multiple remedies are available, the most common being reading glasses. Alternative options include a monovision combination of monofocal lenses or the use of multifocal lenses, in the form of contact lenses or intraocular implant lenses, corneal implants or corneal refractive surgery. This review provides an update …

Treatments for Presbyopia.

BACKGROUND: Accommodation is the eye's ability to dynamically adjust its refractive power in order to focus images at varying distances sharply onto the retina. Presbyopia is the progressive deficit of accommodation that develops as an individual becomes older. An estimated 1.8 billion people worldwide have presbyopia. METHODS: This narrative review is based on pertinent publications retrieved by a PubMed search using the terms "presbyopia," "presbyopic," "intraocular lens," "corneal inlay," and "presbyLasik," with particular attention given to prospective and retrospective studies, systemati…

[TREATMENTS FOR PRESBYOPIA].

Presbyopia is the physiological, gradual, progressive loss of the ability to see clearly at near point which affects people as they age. It is primarily caused by the thickening and stiffening of the lens leading to an inability to adjust its shape to become convex enough to induce adequate plus power to see at short distances. Symptoms usually begin affecting individuals around 40 years of age, the most common being discomfort in or around the eyes after prolonged near work, blur at distance after near work, and eventually progressing to near blur, often with a natural tendency to distance th…

The mechanism of presbyopia.

Accommodation in humans refers to the ability of the lens to change shape in order to bring near objects into focus. Accommodative loss begins during childhood, with symptomatic presbyopia, or presbyopia that affects one's day to day activities, striking during midlife. While symptomatic presbyopia has traditionally been treated with reading glasses or contact lenses, a number of surgical interventions and devices are being actively developed in an attempt to restore at least some level of accommodation. This is occurring at a time when the underlying cause of presbyopia remains unknown, and e…
