The pylorus controls the flow between a reservoir dedicated to mechanical and chemical digestion (the stomach) and a conduit dedicated to the absorption of nutrients (the intestines). The pylorus adjusts gastric outflow resistance to physiological needs. It allows the outflow of isotonic fluids yet selectively retains particles too large for delivery to the intestines and in concert with the antrum further processes them (gastric sieving). Unlike most gut sphincters, the pylorus, at least of man, maintains a patent lumen most of the time. It only intermittently becomes a tightly closed barrier…

Double pylorus is a rare abnormality and it occurs most commonly secondary to chronic peptic ulcer disease. It is usually treated conservatively with anti-ulcer therapy. However, refractory cases may require surgery.

Double pylorus (DP) or duplication of the pylorus is an uncommon condition that is either congenital or acquired. Acquired double pylorus (DP) results from a peptic ulcer eroding through and creating a fistula between the duodenal bulb and the distal stomach. We report a case of an acquired double pylorus in an adult gentleman that resulted from the erosion of a duodenal and prepyloric ulcer.

We report a 55-year-old man presenting with postprandial epigastric pain and vomiting. Barium meal study suggested two openings from the stomach to the duodenum. Endoscopy revealed double pylorus with chronic duodenal ulcer, suggesting the second opening as an acquired one.

It is reported on the endoscopic proof of a double pylorus in a 57-year-old patient. The anamnesis of ulcer of many years and a florid prepyloric gastric ulcer refer to the ulcerous origin of this anomaly. It is referred to the diagnostic importance of the gastroscopy of the acquired or congenital duplication of the pylorus channel which is difficult to be represented radiologically with regard to the avoidance of possible diagnostic errors (e.g. carcinoma of the antrum).
