[Advanced Resuscitation Measures: Extracorporeal Cardiopulmonary Resuscitation].

An extracorporeal cardiopulmonary resuscitation (eCPR) is considered as a therapy option for cardiovascular failure that is refractory to therapy. It can significantly improve the survival rate with favourable neurological results in highly selected patients. The initially defibrillatable heart rhythm and the short low-flow time < 60 minutes are of particular prognostic value. An essential prerequisite for deciding on eCPR is the existence of a reversible cause for cardiac arrest. Whether an eCPR directly at the emergency site (out-of-hospital variant) or in the clinic, e.g. in the cardiac cat…

[Resuscitation].

The factor "time" is probably the greatest enemy in a situation of cardio-respiratory arrest; rapid recognition and competent management are the duty of any physician, in the future eventually also of paramedical or even of non-professional persons. In a short overview the diagnostic and practical procedures of reanimation are repeated. Finally the chances for and results of resuscitated patients are discussed.

[European Resuscitation Council guidelines for resuscitation 2010].

All rescuers trained or not, should provide chest compressions to victims of cardiac arrest. The aim should be to push to a depth of at leas … specific interventions. Removal of the recommendation for a pre-specified period of cardiopulmonary resuscitation before out-of-hospital defibrillation following cardiac arrest unwitnessed by the EMS. The role of precordial thump is de-emphasized. Delivery of drugs via a tracheal tube is no longer recommended, drugs should be given by the intraosseous (IO) rout …

Adult basic life support and automated external defibrillation – Interactions between the emergency medical dispatcher, the bystander who pr … ecial patients are those with severe comorbidities and with specific physiological conditions. Post resuscitation care is new to the ERC Guidelines. Targeted temperature management remains, now aiming at 36°C instead of the previously recommended 32 – 34°C. Pediatric life support – For chest compressions, the lower sternum should be depressed by at least one th …

Resuscitation and auto resuscitation by airway reflexes in animals.

Various diseases often result in decompensation requiring resuscitation. In infants moderate hypoxia evokes a compensatory augmented breath - sigh and more severe hypoxia results in a solitary gasp. Progressive asphyxia provokes gasping respiration saving the healthy infant - autoresuscitation by gasping. A neonate with sudden infant death syndrome, however, usually will not survive. Our systematic research in animals indicated that airway reflexes have similar resuscitation potential as gasping respiration. Nasopharyngeal stimulation in cats and most mammals evokes the aspiration reflex, char…

Pediatric Resuscitation.

This article describes evidence-based nursing practices for detecting pediatric decompensation and prevention of cardiopulmonary arrest and outlines the process for effective and high-quality pediatric resuscitation and postresuscitation care. Primary concepts include pediatric decompensation signs and symptoms, pediatric resuscitation essential practices, and postresuscitation care, monitoring, and outcomes. Pediatric-specific considerations for family presence during resuscitation, ensuring good outcomes for medically complex children in community settings, and the role of targeted temperatu…

Haemostatic resuscitation.

Recommendations for resuscitation of patients in early haemorrhagic shock, with active ongoing bleeding, have evolved in recent years. This review covers current theories of the pathophysiology of shock and recommended treatments, including damage control surgery, deliberate hypotensive management, administration of antifibrinolytics, early support of the coagulation system, and the possible role of deep anaesthesia. Future directions for resuscitation research are discussed.

Fluid resuscitation.

Contention over fluid resuscitation is not new. The issues however have changed considerably. The crystalloid/colloid debate has largely reached a stalemate with little to define clear differences between the two especially early in traumatic shock when increased capillary permeability is a minor issue. Blood, despite its difficulties and its cost is now safer than ever before and we will have to wait a number of years before routine use of blood substitutes becomes possible. Hypertonic saline and hypertonic saline dextran solutions used as an early bolus of 4-6 ml/kg and in conjunction with i…

Hypotensive resuscitation.

While the mechanism remains unclear, a growing body of experimental and clinical evidence suggests that aggressive crystalloid resuscitation in near fatal uncontrolled hemorrhage is associated with poor outcome. Limited attempts to restore blood pressure improve cardiac output, tissue perfusion, and survival while attempts to restore normal tension with crystalloid result in increased hemorrhage volume and higher mortality. The current standard of therapy for treatment of hemorrhagic shock includes initial aggressive crystalloid resuscitation. This mini-review summarizes some of the experiment…

Cardiopulmonary Resuscitation.

Here we describe the evidence for point-of-care ultrasonography application in cardiac arrest. Most commonly, transthoracic echocardiography and transesophageal echocardiography imaging may identify reversible etiologies for arrest, guide chest compression quality, and provide prognostic information. Many arrest ultrasound protocols have been described, and emphasize qualitative assessment of cardiac and respiratory pathology while minimizing disruption to chest compressions.
