Sleepiness.

Sleep medicine can be a particularly rewarding clinical area for neurologists. After all, the driving organ of sleep is the brain. Many of the sleep disorders are primary brain disorders, whereas other sleep disorders (and their medications) influence brain function. But without an organized approach, even the engaged and motivated neurologist can easily become frustrated when dealing with patients whose concerns center on an undesirable degree of sleepiness. Lost in the thicket of information provided during the patient-doctor encounter, many physicians are unable to direct the history toward…

Excessive sleepiness.

Sleepiness is a commonly experienced symptom affecting up to one-fifth of the population chronically. Sleepiness contributes to systemic health effects, and has been shown to contribute to mood disorders and cardiovascular risks. Sleepiness also presents a significant public health risk: it has been associated with major catastrophes, motor vehicle accidents, and medical errors by health care workers. This article reviews the implications of sleepiness on public health, the evaluation of sleepiness, disorders that result in sleepiness, and the management of these disorders.

Sleepiness, troika of consciousness cycle, and the Epworth sleepiness scale.

Excessive daytime sleepiness (EDS) is an important indicator when diagnosing sleep-disordered breathing and evaluating its treatment results. However, there appears to be some confusion as to what exactly is sleepiness; Dorland's Illustrated Medical Dictionary does not help. The medical literature was reviewed in order to assemble a schematic model that would suggest a definition of sleepiness and how it can be measured. The derived model is entitled the troika of consciousness cycle (TCC). It assumes that the presence of wakefulness, nonrapid eye movement sleep (NREMS), and rapid eye movement…

Sleepiness in different situations measured by the Epworth Sleepiness Scale.

This investigation examines how the sleep propensity (SP) in one test situation, such as the Multiple Sleep Latency Test (MSLT), is related to sleepiness in daily life, as assessed by the Epworth Sleepiness Scale (ESS). This is a self-administered questionnaire, the item scores from which provide a new method for measuring SPs in eight different real-life situations. The ESS item scores were analyzed separately in four groups of subjects: 150 adult patients with a variety of sleep disorders, 87 medical students who answered the ESS on two occasions 5 months apart, 44 patients who also had MSLT…

Subjective sleepiness ratings (Epworth sleepiness scale) do not reflect the same parameter of sleepiness as objective sleepiness (maintenance of wakefulness test) in patients with narcolepsy.

OBJECTIVE: To evaluate whether subjective (Epworth Sleepiness Scale or ESS) and objective (Maintenance of Wakefulness Test or MWT) tests of sleepiness are equally useful in patients with narcolepsy. METHODS: Correlational study evaluating the relationship between ESS and MWT as measures of sleepiness. SETTING: Multi-center. PATIENTS: 522 patients (17-68 year old men and women) with a current diagnosis of narcolepsy. INTERVENTIONS: None. RESULTS: Correlations were: MSLT and MWT, r = 0.52 (P<0.001); MWT and ESS, r = -0.29 (P<0.001); MSLT and ESS, r = -0.27 (P<0.001). Regression curve estima…

Sleepiness in Narcolepsy.

Narcolepsy represents the best understood disorder of excessive daytime sleepiness (EDS). EDS presents in 100% of patients with narcolepsy; it is usually the first and the most disabling symptom, which varies in aspects, such as onset age, severity, and clinical characteristics. Early recognition of this symptom leads to early diagnosis and intervention. A diagnosis of narcolepsy is based on clarifying EDS through history, subjective evaluation using questionnaires, and objective measurement, including nocturnal polysomnography followed by a daytime multiple sleep latency test. The therapeutic…

Sleepiness in Adolescents.

Sleepiness and sleep deprivation among adolescents are increasingly being recognized as a public health concern. Many of the determinants of this growing problem lie beyond the biomedical scope of explanation. In this article, the authors begin with a review of the prevalence and consequences of sleepiness in adolescents and then present the approach to a sleepy adolescent and the underlying cause. The topic is discussed from clinical as well as public health perspectives.

Sleepiness in the Elderly.

Excessive daytime sleepiness has pathologic causes and numerous adverse consequences; therefore, it requires medical attention in older adults. Excessive sleepiness in older adults is often multifactorial and may signal an underlying sleep disorder, chronic medical condition, undiagnosed mood disorder, or side effects of medications. It is associated with increased risk for cognitive decline and dementia in the elderly. Excessive sleepiness often requires a multi-method assessment in this age group. It is pertinent to take a systematic, step-by-step treatment approach geared toward the underly…

The sleepy child.

Disorders of excessive sleepiness are uncommon in children. When they occur, the presentation is often consistent with the developmental age of the child and does not meet either historical or laboratory criteria determined with adults in mind. Because of these differences, the true diagnosis can be missed or inadequately treated. Knowledge of the differences in presentation, diagnosis, and treatment between adult and childhood sleep disorders is imperative to prevent serious cardiac, respiratory, psychiatric, and cognitive morbidity.

Excessive daytime sleepiness.

Excessive daytime sleepiness is one of the most common sleep-related patient symptoms, and it affects an estimated 20 percent of the population. Persons with excessive daytime sleepiness are at risk of motor vehicle and work-related incidents, and have poorer health than comparable adults. The most common causes of excessive daytime sleepiness are sleep deprivation, obstructive sleep apnea, and sedating medications. Other potential causes of excessive daytime sleepiness include certain medical and psychiatric conditions and sleep disorders, such as narcolepsy. Obstructive sleep apnea is a part…

Posturographic sleepiness monitoring.

Although reduced sleep often underlies traffic and occupational accidents, convenient sleepiness testing is lacking. We show that posturographic balance testing addresses this issue, because balance testing predicts hours of wakefulness, which could facilitate sleepiness testing. Here, we equate balance scores from separate trials, blinded to the experimenter, with those recorded as a function of known and increasing time awake (i.e. during sustained wakefulness); we show, that the time awake in separate trials is posturographically measurable: positive predictive value 69%, sensitivity 56%, a…

[Excessive daytime sleepiness].

Today excessive daytime sleepiness is recognized as a pathology of its own. It is both frequent and at the root of numerous complications for the individual and the society. It is usually identified with much delay, due to its progressive development and the fact that the patient gets used to it and does not foresee its deleterious consequences. The positive diagnosis relies on the symptoms reported by the patient and his relatives, and the use of introspective and behavioural subjective scales and objective tests. The aetiologic diagnosis is founded on a thorough clinical interview and a phys…
