We present the second reported case of a smooth muscle neoplasm involving the placental parenchyma. On gross examination, the tumor easily separated from the uterus and had a whorled cut surface with finger-like extensions into the villous parenchyma, very similar to the previously described case. Uterine smooth muscle tumors are a heterogeneous group of mesenchymal neoplasms with multiple histologic variants and distinct biological behaviors. One of 3 patients with peiomyosarcomas of the epididymis and spermatic cord treated by inguinal orchietomy was cured, 1 died 18 months after diagnosis and 1 was living but had a pulmonary metastasis 4 years after excision of the primary tumor. All 3 patients with leiomyoma of the testicular adnexa were cured. The literature concerning smooth muscle tumors of the adnexa is reviewed.

This paper presents an overview of smooth muscle tumors occurring in deep soft tissue. The ultrastructure of a series of primary and metastatic smooth muscle tumors is reviewed. Myofilaments and other smooth muscle features were present in all primary leiomyosarcomas of the soft tissues and uterus. Smooth muscle tumors are the most common among mesenchymal tumors in the female genital tract. The vast majority of these neoplasms are clinically benign and easy to diagnose. CD56 is used in gynecologic pathology, typically in the context of a neuroendocrine, sex cord or sex cord-like tumor.
